Description
Good Afternoon Contractors: the following are questions asked by contractors in regards to solicitation VA250-16-R-0043 for Radiology Reading Services for the Chalmers P. Wylie VA Ambulatory Care Center in Columbus, Ohio 43219. The answers are highlighted in yellow. 1. Please provide the 2015 imaging volumes by month and by modality for the CHALMERS P. WYLIE VA AMBULATORY CARE CENTER Answer: See the embedded Attachment highlighted in yellow. 2. Is there any flexibility on which voice recognition software is used to interpret exams if the radiology provider is able to provide final reports back to the VA's worklist solution in real-time? Answer: No, there is no flexibility on using outside voice recognition software. 3. Please confirm the hours that the radiologist will be required to be onsite versus remotely providing interpretations. Answer: Radiologist must be onsite from 8am until 4:30pm. Radiologist services may be required between 4:30pm-7pm or 7am-8am to cover surgery or urgent care. 4. Is there any evening or night time volume which will need to be interpreted? Answer: Radiologist must be onsite from 8am until 4:30pm. Radiologist services may be required between 4:30pm-7pm or 7am-8am to cover surgery or urgent care. 5. From a network security perspective, will the VA allow us to interpret exams remotely via our unified worklist solution? Or can we add workstation to a few of our locations to assist in the reads? Answer: No. Readings will be completed at the VA from 8am to 4:30pm. 6. How will invoicing and payment occur? Answer: The vendor will place monthly invoices into the VA software, Invoice Payment Processing System. After certification, payment will be made. 7. Can you please tell me if this is a new requirement? If not, who is the incumbent, and what is the contract number? Answer: No, this is not a new requirement. Since there are proprietorial information regarding the incumbent no information will be share about them or the current contract. " Is the pricing, while submitted in a separate binder, to be entered in both of the SF 1449 cover page blocks 23 and 24 AND under the Key Personnel section of each of the base period and the four succeeding option periods of the Price/Cost Schedule? Answer: Yes, the cover page of the SF1449 can be submitted with only the unit price and amount in blocks 23 and 24 on it for the base year only. The price for the base year and the four succeeding option period shall be into in a separate binder label price proposal. " Under the Performance Work Statement for Radiologist Services, page nine, 1.4, I am unable to locate the provided spreadsheet which delineates annual workload and serves as a historical representation of the annual workload and trending for future growth. Will you please provide/clarify? Answer: See the attached embedded workload above. " Will it be possible for HL-7 order messages to be sent to the contractor's voice recognition system? The contractor's voice recognition system will meet all VA security requirements and policies and return an HL-7 result message that can be uploaded into the VA's voice recognition and other patient information systems. Answer: No, all reading will occur on station using the VA Power scribe software. "Will it be possible for the contractor's radiologist workstation to pull exams from the VA's PACS and be transferred to an off-site radiologist workstation for sub- specialized radiology support? The contractor's workstation will comply with all VA security requirements and policies. Answer: No. It is expected that readings will be completed onsite. " Regarding Section D Attachment 5, Subcontracting Plan Model, if we have no need for a subcontractor, how do we respond to this portion? Contractor would furnish radiologist services to include readings for General Diagnostic, Computed Tomography, Ultrasound, Magnetic Resonance Imaging, Bone Density, and Nuclear Medicine per the contract description. Answer: If you do not use a subcontractor, state so in the response within the Subcontracting Plan Model. " within Section E, Special Solicitation Instructions to Contractor, General Proposal Format (2) c) Quality of Offers Facility. We are unclear as to the meaning of "Quality of Offers Facility." Will you please elaborate. Answer: Explain your facility laid-out, your Organizational Chart, and staffing, etc. My questions are related to my confusion relative to the overall proposal format that you are seeking. Within the Addendum to FAR 52.212-1 Instructions to Offerors- Commercial items, Special Solicitation Instructions to Contractor, as you lay out in (a) General Proposal Format, I understand that you are seeking the Price Proposal in its' own, separate binder. Is the second binder what you are considering to be the Technical Proposal? Answer: Yes, the price proposal is in a separate binder and the second binder is for the Technical Proposal; " If yes to the above question, is the Technical Proposal to include tabs a through g? Answer: Yes, the Technical Proposal to include tabs a through g. " The Technical Proposal that is to be in accordance with FAR 52.212-2, Evaluation-Commercial Items, is it to include the Technical Capabilities detailed within E.13? Answer: Yes, the Technical Capabilities detailed within E. 13 are already embedded within tabs a through g, so they do not need to be restated again. " If yes to the above question, are then the responses to these capabilities to be embedded within tabs a through g? Answer: Same answer as reference above " the Technical Capabilities detailed within E. 13 are already embedded within tabs a through g, so they do not need to be restated again". Does the Combined Synopsis/Solicitation under solicitation number VA25016R0043 contain requirements similar to a current contract? If possible, please provide the current contract number. Or, is this a new requirement for the government? Answer: Yes, the requirements are similar to a current contract. No, we do not provide current information on current contracts as they may contain proprietary information about the contractor and/or their method of doing business. Can you tell me where to find the spreadsheet? 1.1.Workload in Radiology fluctuates by as much as 25% on any given day. The Contractor shall provide a sufficient workforce of onsite personnel to interpret 100% of the workload. A spreadsheet is provided which delineates annual workload and serves as a historical representation of the annual workload and trending for future growth. Answer: See the embedded attachment for the workload above. 1.Can I hand deliver the contracts to your office as I did last time? I don't trust delivery services. Answer: Yes, your proposals can be hand delivered to this office at 6150 Oak Tree Blvd, Suite 300, Independence, Ohio 44131. 2.Is the cover page (SF 1449) to be submitted only with the price proposal since it has the unit price and amount (blocks 23 and 24) on it? Answer: Yes, the cover page of the SF1449 can be submitted with only the unit price and amount in blocks 23 and 24 on it for the base year only. 3. Should the proposal have the entire text of the bid submitted, since there are little fields throughout that need to be filled in (like QASP acknowledgement)? If you do want the entire body of the text, which section should it go in of the specified sections below? Also, which section do the attachments go in? a. Company Overview/History b. Education/Qualifications c. Quality of Offers Facility d. Management Capabilities and Approach e. Past Performance Reference List f. Label of Acknowledgment of all Amendments g. List/ Copies of Licensures and Certifications of Insurance Answer: Use a tab for each section and list the information that is associated with that section under that tab. E.g. The Company overview/History; then provide that information regarding the company overview and history, the next tab - Education/ Qualifications; then list that information regarding the education/qualifications of your physicians, their medical background, etc. Note to contractors: The Government is more interested in the content and detail of your information or have you omitted any important facts in the proposal and not the format that you use so long as it follow the above layout. 3.In the Price/Cost Schedules it has spaces for 3 radiologists; I am assuming I can add my 4th radiologist as that is how many we have. Also, they rotate through the VA, so do you want me to just split the total hours between the four radiologists, knowing that this is just a guess as to how the total 3120 annual hours will be split up between them? Answer: The fourth Radiologist can be added and the invoice would be totaled. The invoice should indicate total hours as reflected from daily handwritten logs. 4.In 3.2 it says we must be available "24 hours a day, 7 days a week, and 52 weeks a year for stat, urgent and routine exams". This is different from the hours specified as 3.1 which are specified as 8am to 4:30 pm and then between 7am to 8am and 4:30pm to 7pm for STAT service. This seems to conflict- can you clarify? Answer: Please disregard the available "24 hours a day, 7 days a week, and 52 weeks a year for stat, urgent and routine exams". The correct verbiage is the hours specified as 3.1 which are specified as 8am to 4:30 pm and then between 7am to 8am and 4:30pm to 7pm for STAT service. 5.Past performance surveys- it is unclear to me if these are to be filled out by the Reference person and sent by them directly to you (as it says to do in Section D. attachment 4) OR filled out and sent back to me to be submitted by me with the bid in section e to you. Maybe in section e. you just want the names of the references? Also, how many references should there be (as this is not specified) Answer: To clarify, all past performance surveys are to be attached to your proposals and just add the contact person names and addresses under Section E within the binder with tab "Past Performance Reference List. 6.On the General Proposal Format, can you tell me what section "c. Quality of Offers Facility" and "F. Label of Acknowledgement of all Amendments" means? It is not clear to me what those things are. Answer: Quality of Offers Facility - Explain your facility laid-out, your Organizational Chart, and staffing, etc. Answer: Label of Acknowledgement of all Amendment -when amendments are posted be sure to acknowledge that you have read them by their number and dates posted. This should be done on page 4 of the solicitation and tab (f) Label of Acknowledgement of all Amendments. 7.What section do the Attachments 1, 2, and 3 go into? Answer: You can add Attachment 1,2 and 3 into binder D, which is label Management Capabilities and Approach for Attachment 1: Key Personnel Spreadsheet, Attachment 2: Contractor Certification and Attachment 3: 852.209-70 Organizational Conflicts of Interest (JAN 2008). Finally, Attachment 5: Subcontracting Plan Model.
