- This Sources Sought Notice is issued for information and planning purposes only. This is not a solicitation or a request for proposal. An award will not be made on offers received in response to this notice, and it shall not be implied the Government is committed to providing any solicitation or award following this notice. The Government will not pay for any information received in response to this request, nor will the Government compensate a respondent for any costs incurred in developing information provided. Department of Veterans Affairs, VISN 19, Eastern Colorado Health Care System (ECHCS) Denver VA Hospital is seeking a potential qualified contractor to provide the following: Responding companies shall include any relevant information (specifications, cut sheets, brochures, capability statement, etc.) confirming their ability to meet the service requirements in the Performance Work Statement and supply product with the salient characteristics outlined in this request. This notice shall not be construed as a commitment by the Government to issue a solicitation, or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. MICROBIOLOGY MEDIA, KITS, REAGENTS, and SUPPLIES 1. SCOPE: A five-year blanket purchase agreement (BPA) to provide a full range/wide array of microbiology media, supplies, identification kits, biochemical reagents for rapid/full identification, ATCC quality control (QC) organism, and disposable supplies related to processing and complete work-up of specimens for microbiology cultures. These supplies, kits, reagents, and media must be delivered on 1st and 3rd Wednesday of the month, or next business day, in case of a holiday according to the estimated quantities listed on the attached schedule. All media, supplies and peripherals must perform according to specifications and represent a bona-fide need for patient care on behalf of the Denver VA Medical Center (ECHCS), 1055 Clermont Street, Denver, CO 80220. Supplies shall be required for a base year period, and four option years. Media, kits, reagents, and QC organisms are specific for required microbiology testing for patient specimens, and quality control requirement. Supplies shall be delivered to Denver VA Pathology & Laboratory Medicine Service. 2. DEFINITION: The price shall include costs covering (a) full spectrum of microbiology media listed, (b) identification kits, (c) biochemical/chromogenic media, (d) rapid identification tests, (e) transport/collection kit, (f) consumables, (h) antigen kit testing, and any other item required for the proper operation of the contractor's supplies and software necessary for data management. (f) ATCC QC organisms, (h) Contractors are required to provide prompt delivery according to the schedule. 3. GENERAL REQUIREMENTS: a.Offered media, kits, reagents, and supplies shall be capable of producing accurate and reproducible results. Supplies shall provide accurate result up to the manufacturer's defined guidelines without failures or media, kits or software will be replaced at no additional charge to the DENVER VA MEDICAL CENTER. 1.Offered media, kits, and reagents shall be Clinical Laboratory Standard Institute (CLSI), Clinical Laboratory Improvement Amendments (CLIA), & FDA approved for patient testing. 2.The contractor shall provide technical standard operating procedure templates required for patient testing for microbiological culture and assays. 3.A technical manual and quality control documents shall be furnished with supplies supplied to VA. 4.Failure to provide a quality media, kits, reagents, antimicrobial susceptibility disks, and supplies may result in review of the contract. 5.The contractor should provide the quality control standards letter at the beginning of year as required by College of American Pathologists (CAP) and CLIA. 6.Notification of backordered media, supplies, and reagents as soon as possible (ASAP) by email or a phone call. 7.Provide a list of a replacement for backordered media without any delay for patient testing. Media should be made available as requested. 8.Required: Refer to appendix 1 for a list of media, kits, reagent, and supplies for estimated quantities of minimum and maximum per year, delivery schedule of minimum and maximum media and supplies. Assigned laboratory staff (Kevin Ibershoff, Patricia Cuddy, Rose Martinez, Marlon Coggins, Neelam Ojha, and/or Diane Svoboda) should be able to call the order for supplies to vendor or place the order on-line for delivery. b.The quality of the media, kits, reagents, and supplies shall be high enough to satisfy proficiency testing standards of the College of American Pathologists (CAP), CLSI, CLIA, FDA and OSHA. 1. With the exception of the specific quality control requirements stated herein, which shall have precedence; credit for failure to provide quality media, kits, reagents, and supplies, and replacement shall be performed and provided in accordance with the terms, conditions, and provision of contactor's existing Federal Supply Service Contract for performance as stated in Subsection III of the VA National Acquisition Center contract requirements Solicitation (RFP 797-FSS-03-0001). 4. SPECIAL REQUIREMENTS: a.Technical performance based on following criteria: "Wide spectrum/arrays of microbiological media, kits, reagents, biochemical, and QC organisms supplies for bacteriology, mycology, mycobacteriology, virology/PCR, parasitology, serology, transport media, and consumables etc. "Media should meet the performance for: observing defined hemolysis, depth (deep media), freshness, colony morphology, cracks, dehydration, sterility, clear chromogenic observation of colony growth, formulation appropriate to the growth of specific organisms, performance and pH of susceptibility media. "Long expiration of kits, reagents, and antimicrobial disks for susceptibilty. "Peer review information. "Availability & Selection of packaging (option for small & big package as needed) "Long shelf-life at the time of shipment. "Direct delivery from manufacturer, not from the stocked supplies from warehouse to ensure freshness of media/reagents. "Next day delivery of media/supplies to meet the above requirements. b.Replacement of products, without additional charges, when failure of performance is observed. c.Immediate notification when a product is backordered, in-house QC fails, or if there will be any delay in product shipments. 6. TRANSITION PERIOD: The transition period for the awarded contractor to have microbiology media, kits, reagents, and supplies delivered and fully operational shall be within 7 days.
