- Amendment 1 - Cancellation Notice 8/2/2016 THIS SOLICITATION IS CANCELLED - See the attached modification document for details. I. This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items," as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only notice. Written proposals are being requested and offerors should be submitted using RFP# DHHS-NIHAO2016046. This is a request for proposals (RFP). The solicitation documents and incorporated provisions and clauses are attached and in effect through Federal Acquisition Circular (FAC) 2005-89 effective July 14, 2016. The associated North American Industry Classification System (NAICS) code for this procurement is 541611 with a small business size standard of $15M. This acquisition will be processed under FAR Part 13 and Part 15 as applicable. This is NOT a total small business set aside. It is anticipated that one Indefinite Delivery Indefinite Quantity contract will be awarded. II. DESCRIPTION: See the attached document for the Statement of Work as well as all relevant attachments, Clauses and Provisions. III. PERIOD OF PERFORMANCE: The estimated Total Period of Performance is from award date of September 1, 2016 through August 30, 2019. IV. DELIVERY: FOB Point shall be Destination, Rockville, Maryland. Delivery location is: 5601 Fishers Lane, Rockville, MD 20852 unless otherwise indicated. Proposals and Past Performance Surveys must be submitted via email to the Contract Specialist, LaDonna Stewart on or before August 9, 2016 at 10:00 AM EST. By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the System for Award Management (SAM) https://www.sam.gov/portal/public/SAM/ (Previously CCR at www.ccr.gov) prior to award, during performance, and through final payment of any resultant contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. [Note: Lack of registration in the SAM will make an offeror ineligible for award]. All responsible sources may submit a proposal which shall be considered by the agency
