- Required Provisions_please complete Plan Provisions and clauses RFQ Solicitation number HU000116Q0717 Fifty (50) Wood Lockers and Eight (8) End Panels The Uniformed Services University of the Health Sciences (USU) is the Nation's federal health sciences university and is committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense and the United States Public Health Service and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USU is located in Bethesda, MD on the grounds of the Naval Support Activity Bethesda. Additional information regarding USU can be found on our website: www.usuhs.mil. USU has a requirement for (50) Wood Lockers and (8) End Panels including delivery and installation. This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 - Acquisition of Commercial Items and FAR Part 13 - Simplified Acquisition Procedures. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 05-89 (July 14, 2016) and Defense Federal Acquisition Regulation Supplement (DFARS) (August 2, 2016). This requirement is set-aside for small business concerns under NAICS code 337215 (Showcase, partition, shelving, and locker manufacturing) with a size standard of (500). In order to be eligible for award, offerors must be registered in www.SAM.gov, with completed representations and certifications by the close date of the solicitation. Offerors responding to this RFQ, must submit a quote for a for (50) Wood Lockers and (8) End Panels meeting the following specifications. SPECIFICATIONSQUOTED SPECIFICATION - COMPLETED BY OFFEROR 1.Single full standing wood locker configuration 2.Approximately 78"h x 17"w x 20"d 3.1-coat rod, 1-coat hook 4.Raised Panel wood door, ¾'' kiln dried solid wood stile and raised center panel made of maple. Moisture resistant glue will be used to hold the stile and rail joints together 5.Two coats of natural stain with hand sanding in between coats. Sealed with a moisture-proof resistant top coat 6.Wood should be warp free and the finished faces should be free of chips and scratches 7.Locker frame will be approximately 5/8'' almond melamine with particleboard substrate 8.Chrome hardware 9.Heavy duty hinges 10.Key lock with 2 keys per locker and 2 master keys per order. Keys need to correspond to number on locker. 11.Numbered lockers/disc with black engraved brass disc routed flush with locker door. Installed in numerical order. 12.4'' base trim and end panels to cover exposed sides 13.½" Venting on top between door and frame 14.Installation of lockers with approved plans 15.Lockers must be structurally sound and free from defects in materials and workmanship for a period of 3 years from installation. The government intends to award a single firm-fixed priced (FFP) purchase order to the offeror representing the best value to the government based on the evaluation factors listed below. Evaluation factors are listed in order of importance, with most important listed first. The government will conduct a trade-off between price and non-price factors. The Government is not required to award to the lowest priced offeror or the highest technically rated offeror. The government reserves the right to award without discussions. Evaluation Factors (Listed in order of importance): •Price - Offerors must include all applicable costs (Example: Components, Shipping and Installation) in their quote. The total quoted price will be the evaluated price. •Technical - Offerors must provide product literature, which demonstrates the capabilities of the quoted item(s). At a minimum the product literature must demonstrate how the quoted item meets the required specifications and the desired specifications quoted. Information on warranty and delivery lead-time must be included in the quote submission and will be evaluated. The government reserves the right to utilize technical (i.e, trade magazines) and customer reviews/references, if needed, as part of the technical evaluation for the quoted equipment/supply. FAR clause 52.217-6 (Option for Increased Quantity) is included in this requirement. The Government reserves the right to unilaterally exercise this option within six (6) months after award for additional lockers at the unit price listed in the order. Offerors must also submit the following with their quote, if not completed in SAM registration: •Completed copy of provision 252.209-7991 •Completed copy of provision 252.225-7000 •Completed copy of provision 52.209-2 •Estimated delivery lead-time See attached list for applicable provisions and clauses for this requirement. Quoted items must be new. Quotes for refurbished equipment will not be considered. Offerors must also indicate in quote where item is manufactured. Items will be shipped to Bethesda, MD. Deadlines: Questions/clarifications regarding this solicitation must be submitted via email to polina.klimenkova@usuhs.edu by 8/5/16. Any question received after 8/5/16 will not be answered. Questions will not be answered over the phone or submitted by any other means. All questions will be answered in an amendment to the solicitation posted on or around 8/8/16. Quote packages are due by 8 pm Eastern Local Time on Thursday, 8/11/16. Quotes must be submitted via email to polina.klimenkova@usuhs.edu. Quotes will not be accepted by any other means. Late quotes will not be considered. Offerors will receive an email confirmation that they quote was received and whether it was on time or late within two (2) business days. Quote packages should include a cover sheet that provides the following information: •Company Name •Point of Contact, to include email and phone number •DUNS Number •Statement that includes acknowledgement of latest amendment number (if applicable). •Preferred method of payment (Visa or EFT)
