- Idaho Workers' Compensation Verification Form Experience Questionnaire Wage Determination RFQ Solicitation Cone Collection SYNOPSIS/SOLICITATION AG-02RC-S-16-0123 is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation (see attached Solicitation). This solicitation is issued as a Request for Quotation (RFQ) and the documents incorporate provisions and clauses in effect through Federal Acquisition Circular 2005-89. This procurement is set aside for Small Business. The NAICS code is 113210 and the Small Business Size Standard is $11 million average annual receipts of a firm over three years. DESCRIPTION OF REQUIREMENT AND LOCATION Work consists of collecting cones on the Nez Perce-Clearwater National Forests. The work sites may be located on any district of the forest. Maps of tree locations will provided at time of contract award. See attached Solicitation pages 9-10 under Scope of Work and Location and Description for details. SCHEDULE OF ITEMS The Schedule of Items includes a Base Year plus two (2) Option Years each with 16 line items for Operational Cone Collection (tree climbing and trees to be felled by Contractor and Government) for Douglas-fir, Lodge Pole Pine, Ponderosa Pine, White Bark Pine, Western Larch, White Pine, and Western Red Cedar. The Pay Unit is by Bushel and Tree. See attached Solicitation pages 3-7 for details. DATES Base Year plus two (2) option years from 2016-2019. See attached Solicitation page 9 under Period of Performance. (APR 1984) Quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. This form will be submitted to the Idaho Industrial Commission by the Government in order to assure that the Contractor in line for award has the required worker's compensation coverage or is exempt from the requirement for coverage. (b) A copy of the State of Idaho Certificate of Verification of Workers' Compensation Insurance form has been provided for use in the Attachments and shall be completed and submitted as part of the offer. (c) This certificate of verification shall be fully executed by the Contractor and signed by the Industrial Commission Compliance Officer prior to award of the contract. EVALUATION FACTORS The Government will award one or more contracts resulting from this solicitation to the responsible quoters whose quotes conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (a) RELEVANT PAST PERFORMANCE. Provide a list of all similar contracts performed by the individuals proposed for this project within the past three years. Include references with a contact name and current telephone numbers. We will assess your history of performing quality and timely work and your commitment to customer satisfaction. (b) KEY PERSONNEL QUALIFICATIONS AND EXPERIENCE. Identify the individuals who will perform the work under this contract and describe their experience and qualifications in this type of work. Also address specialized experience tree improvement skills and identifying tree phenology and morphology in accordance with Region 1 standards and knowledge of the field conditions to be encountered, including local species and habitat types. (c) WORK PLAN AND PRODUCTION SCHEDULE. Provide a work plan that demonstrates your ability to complete the work within the required performance period. This would include approximate dates of performance for each Item quoted on, number of workers, production rates anticipated, and equipment to be used. Address any limitations in the total amount of work you could accept. (d) SAFETY PLAN. A written safety plan shall be provided to the Forest Service specifying the Contractor's plans to ensure maximum safety during performance of the work for your employees and any public persons on your work site. (e) PRICE. Prices submitted in the Schedule of Items will be evaluated for reasonableness and the degree to which they reflect an understanding of the work requirements. The Government may screen the quotations on price and those quotations determined to be unreasonably high will not be further considered. EXPERIENCE QUESTIONNAIRE The Experience Questionnaire will be used to make an evaluation and arrive at a determination as to whether the quotation will meet the requirements of the Government. It must present sufficient information to reflect a thorough understanding of the requirements and a detailed description of the techniques, procedures, and program for achieving the objectives of the specifications/statement of work. Experience Questionnaires which merely paraphrase the requirements of the Government's specifications/statement of work or use such phrases as "will comply" or "standard techniques will be employed" will be considered unacceptable for award. As a minimum, the Experience Questionnaire must clearly provide the following criteria, which are approximately equal in importance.
