- The Gulf Coast Veterans Healthcare System (GCVHCS), Network Contracting Office (NCO) 16, Biloxi, MS, is issuing a combined synopsis / solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number VA256-16-Q-1153 is issued as a Request for Quote (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88 effective May 16, 2016. The NAICS code is 541350 and the Small Business Size Standard is $7.5 million. This requirement is 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB) and will be firm-fixed-price. The closing date for this notice is September 2, 2016 at 3:00 p.m. EST. The GCVHCS is seeking SDVOSB businesses capable of providing elevator and dumbwaiter maintenance services for the Overton Brooks VA Medical Center, Shreveport, Louisiana for the period of Oct 1, 2016 through September 30, 2017 with four (4) each 12-month option periods. The contractor shall furnish all labor, tools, supplies, materials, equipment and supervision necessary to perform all work associated with elevator and dumbwaiter maintenance services in accordance with the Performance Work Statement (see below). Description of Services 1.1. The contractor shall provide full elevator and dumbwaiter maintenance service, in compliance with the ANSI ASME Safety Code requirements A17.1; A17.2 and A17.3, the manufacturers recommendations, the Elevator Industry Field Employees' Safety Handbook, National Electrical Code, NEIT Vertical Transportation Standards for Elevators, Escalators and Dumbwaiters and all other applicable laws, regulations, rules, ordinances, codes, etc., for all of the elevators and dumbwaiters listed in Appendix A. Full maintenance service is defined as all services, repairs and testing necessary to maintain all elevators, appurtenances and accessories in a fully operational mode at all times except for prescheduled downtime including all labor, parts and materials. Measurements of elevator speed, door opening and closing times, performance time, door closing force, floor to floor performance times, mean times between service calls, ride quality, stopping accuracy, downtime, customer surveys, and review of preventive maintenance and inspection reports shall be the governing factors in determining the adequacy of the elevator maintenance at Overton Brooks VA Medical Center (OBVAMC), Shreveport, LA. 1.2. The contractor shall be responsible for all costs associated with accomplishing repairs and replacements, including labor, equipment, and supplies for all equipment and systems specified or recommended by the system manufacturer, The Contractor shall not be required to make repairs made necessary by reason of negligence or misuse of the equipment by persons other than the Contractor, his/her representatives and employees except ordinary wear and tear. Other items excluded are stationary architectural features and underground piping for hydraulic elevators. This does not exempt deficiencies from coverage under this contract that developed, or was caused, prior to the contract effective date. 1.3. The contractor shall be fully responsible and accountable for the operation of all equipment beginning on the date specified in the contract performance period. These specifications are a statement of the minimum level of work and services that are to be provided in certain areas under this contract. They are not intended to be, nor shall they be construed as, limiting specifications or requirements, at a minimum, the contractor shall be required to take all steps and measures, which would be taken by a prudent building owner, to maximize the life expectancy of the elevators, related systems and ensure safe and reliable elevator operation. 1.4. The contractor shall develop and implement a preventive maintenance (PM) program, performed during normal hospital working hours acceptable to the Contracting Officer Representative (COR). All equipment shall be maintained in accordance with the manufacturer's recommendations, the best practices of the industry, and applicable codes, standards, and regulations. If a conflict arises between these standards, the most stringent will prevail. Within five (5) days after award, a copy of the PM program shall be provided to the COR for acceptance. The PM program must be acceptable to the COR prior to the commencement of the contract period. 1.5. The contractor shall maintain a log in a designated VA location. Log shall be annotated at the conclusion of each maintenance/inspection. The fog shall identify each elevator, date and time of inspection maintenance/ inspection performed, repairs needed, name of inspector, and overall condition of the elevator. If repairs require work outside the scope of this contract, the contractor shall notify the COR for appropriate decision and action. 1.6. All scheduled work that requires an elevator to be taken out of service shall be coordinated with the COR and the contractor shall report to the COTR the status of elevator equipment or systems not operating by the close of each workday. 1.7. In order to provide for uninterrupted Government business and elevator service, unnecessary out-of-service periods for elevators shall not be tolerated. Therefore, it is essential that elevator repairs be performed immediately. 1.8. The contractor shall maintain the machinery spaces and storage areas in a clean and orderly manner. When work is performed in these areas, the Contractor's personnel shall clean up all debris and leave the area in a presentable condition. The Contractor must obtain the approval of the COR before storing anything in machinery spaces. Operating supplies such as lubricants, rags, cleaners, etc., shall be properly secured in containers approved by the COR. Storage shall not negatively impact the means of egress, fire protection systems, and emergency lighting; nor, shall it significantly increase the amount of combustible material in the machinery space. Clearances from electrical equipment shall be maintained as required by NFPA 70, the National Electrical Code. 2. Scheduled Maintenance 2.1 Cleaning, Lubrication, and Adjustment Services. As required, motors, generators, controllers, valves, pumps, tanks, jack units, relay panels, selectors, leveling devices, operating devices, signal devices, switches on car and in hoist way, hoist way door and car door or gate operating device, interlocks and contacts, guide shoes, guide grooves in hoist way and car door sills, hangers for all doors and/or gates, signal system, car safety devices, governors, tension frames and sheaves in pit shall be cleaned, lubricated and adjusted. Hoist motor and generator brushes shall be cleaned, lubricated and adjusted. Hoist motor and generator brushes shall be checked for wear at least every two weeks. Accumulated carbon dust shall be removed from commutators, brush rigs and windings at the same time. 2.2.Cleaning Services. Guide rails, overhead sheaves and beams, counterweight frames, tops of cars, bottoms of platforms, elevator pits, and machine room floors shall be brushed clean and rubbish removed at least once a month. A complete cleaning of the machine room equipment, hoist way equipment, machine room floors and walls, hoist way walls, supporting structures, etc. shall be accomplished once per year. The Contractor shall furnish all necessary cleaning supplies and equipment necessary to accomplish cleaning service. 2.3. Lubrication Services. All sheave, machine, and motor bearing and hoisting ropes shall be lubricated, machine stuffing box and bearing on motor-operated brakes shall be repacked and gear cases and guide lubricators shall be refilled when required. Hydraulic fluid sufficient to maintain the best operating levels shall be furnished on all hydraulic elevators, 2.4. Adjustment Services. All hoisting ropes shall be examined and the tension equalized. Whenever necessary to insure maintenance of adequate safety factors, all hoisting ropes and governor ropes shall be replaced. The exterior of the machinery and any other parts of the equipment subject to rust shall be kept properly painted and presentable at all times. The motor windings shall be kept free of rust and carbon dust, Guide shoe rollers and gibs shall be renewed as required to insure smooth, stable, and satisfactory operation. All electrical wiring and conductors extending to elevators and dumbwaiters from circuit breakers and main line switches in machine rooms and from outlets in the hoist way shall be repaired and/or replaced-when required: 2.5. Materials to be Furnished, The Contractor shall furnish all lubricants, hydraulic fluid, cleaning supplies, parts and tools necessary to perform the work described above. All lubricants shall be recommended by the manufacturer of the equipment. 2.6. Parts to be Furnished and Installed or Repaired. The Contractor shall furnish and install or repair when and as necessary, machine, hydraulic pumps, motor generators, controllers, selectors, worms, gears, thrusts, windings, commutators, hydraulic pumps, "VEE" belts and pulleys, hydraulic valve components and coils, circuits, magnet frames, hoist and governor ropes, relays, contactors, control fuses, resistors, cams, car door and hoist way door hangers, tracks and door guides, door operating devices, door gibs, interlocks and contacts, car gates, safety devices, governors, car and hoist way lighting, push buttons, annunciators, hall lanterns and indicators, lamp replacement in signal systems and all other elevator signal and accessory equipment complete. Contractor shall furnish, install or repair parts and/or make renewals as necessitated by reason of normal use, equipment malfunction, and wear and tear by all parties. Damage caused by proven vandalism or misuse shall be corrected outside the scope of this statement of work. The Contractor shall provide the COR with a detailed cost estimate of the needed to bring the damage back to working order. Then the government may offer the work to the contractor through a contract tool outside this contract or solicit the repair work as a new contract. 2.6.1. Parts installed on the elevators covered by this specification, shall be new and genuine parts supplied by, or certified by the Contractor as equivalent to, the original equipment manufacturer, or its successor prior to the installation of the part. The Contractor shall maintain a stock of applicable replacement parts to reduce to a minimum the interruption of elevator service. When replacing existing parts or installing new parts, the Contractor shall install energy efficient parts to the maximum extent possible. 2.7. Publications to be Furnished. The Contractor shall, as a cost included in the various bid items, provide the following complement of publications to the COR at the commencement of the Contract: "1 copy - ANSI/ ASME 17.1, most recent edition: "Safety Code for Elevators and Escalators" "1 copy - ANSI/ ASME 17,1 Handbook, latest edition: "Elevators, Escalators, and Moving Walks Handbook" "1 copy ANSI/ ASIVIE 17.2, latest edition, "Elevators and Escalators - Inspectors Handbook" "1 copy -ANSI/ ASME 17.3, latest edition, "Elevators and Escalators, Safety Code" "4 copies - ANSI/ ASME 17,4, latest edition, "Emergency Evacuation of Passengers From Elevators 3.Un-Scheduled Repairs. The Contractor shall provide 24 hour, seven day a week un-scheduled repair services for all elevators listed in Appendix A, when notified by authorized personnel between regularly scheduled inspections and maintenance visits. The Contractor shall respond within two (2) hour of the initial notification call. Un-Scheduled Repair service includes equipment certification testing, as may be applicable, after completion of repairs and before placing the elevator back in service. 4.Emergency Service. The contractor shall promptly respond within one (1) hour of the time of notification in the event of a trapped passenger emergency service 24 hours per day, 7 days per week. The contractor shall report to the site of the emergency and shall remain on the job until the emergency has been resolved, If the situation cannot be resolved immediately, the Contractor shall promptly notify the COR or designee of the time and date corrective action shall be taken. Requests for service may be made by the COR or designee by telephone or other means. If the repairs cannot be completed, the elevator shall be secured, with the contractor promptly notifying the COR or designee of the problem and the time and date corrective action will be completed. 5.Inspection Services. Semi-annual inspections of the elevators required by the National American Standard Safety Code for Elevators and Escalators latest edition shall not be included in this SOW. However, the results of said inspection will be provided to the contractor for which he shall be responsible for correcting deficiencies noted by the report. An FSE employee shall be made available by the contractor to accompany the inspector during the inspection. The third party inspector shall furnish a written report of each inspection. The report shall be submitted on the following checklists from the American Society of Mechanical Engineers (ASME): Checklist for Inspection of Electric Elevators, Checklist for Inspection of Hydraulic Elevators, or Checklist for Inspection of Escalators and Moving Walks, as appropriate. The contractor shall be responsible for the correction doll listed deficiencies within the scope of this Statement of Work. These deficiencies must be corrected within 15 working days after inspection, unless otherwise agreed to by the Contracting Officer. Any deficiencies marked "emergency" must be corrected immediately. When all listed deficiencies have been corrected the contractor shall sign and date the inspection report and return it to the COR. Under no circumstances, shall the Contractor allow reported deficiencies to go uncorrected beyond the contract performance period, 6.Testing. In accordance with ASME A17.I/CSA B44 Handbook-2004 (or latest edition) - The maintenance contractor shall perform the periodic (annual) elevator safety tests of all elevators. The tests shall be witnessed by an ASME QEI-1 qualified inspector contracted by the VA. The maintenance contractor shall tag the tested devices after the inspection and safety tests have been performed and certified by the inspector. Routine (semi-annual) elevator tests shall be performed by an ASME qualified inspector contracted by the VA each year. The elevator maintenance contractor shall provide qualified personnel to assist the inspector during the routine elevator inspections and tests during normal VA working hours at no additional cost to the VA. The elevator maintenance contractor shall be responsible to correct deficiencies found during the routine and periodic inspections and tests. All safety deficiencies shall be repaired immediately and the remaining deficiencies shall be corrected within fifteen (15) working days of receipt of report from the VA. If a deficiency cannot be corrected within fifteen (15) working days, the contractor must submit an explanation, for approval by the COR, stating the reason why the deficiency cannot be corrected and a time when it shall be corrected. 7.Firefighter's Service Requirements. All elevators provided with firefighters' service shall be subject to monthly Phase I recall and a minimum of one floor operator on Phase II to assure the system is maintained in a proper operating condition in accordance with rule 211.3 ASME Al 7NCSA B44 Handbook-2004 Edition or latest edition. 8.Contract Monitoring Procedures. Contracting Officer Representative (COR) to assist with the contract monitoring requirements. The COTR will monitor such items as quality of service, timeliness of performance, customer service, cost control, and business relations supporting documentation. In accordance with FAR 1,601 (a) "Contracts may be entered into and signed on behalf of the Government only by contracting officers." A COR may not modify the stated terms of the contract, including extending the length of the contract period. 9. Performance Standards. Contractor performance evaluation shall be based upon continuously acceptable operation of all elevators, Acceptable elevator operation is based on ensuring the original operation criteria are maintained at all times for each elevator. Such criteria includes the following: maximum capacity in pounds; rated speed in feet per minute; timely and reliable door operation; traffic handling capabilities; response times; and ride quality. Increased frequency in service calls is not considered acceptable performance. In addition, if the Contractor fails to repair or correct reported deficiencies, performance shall be unacceptable. Response time is dependent on the elevator classification as essential or non-essential, and whether passengers are trapped in the elevator. Al! elevators at OBVAMC are classified as essential. If passengers are trapped, the Contractor must respond immediately and in no event more than one (1) hours after telephone notification and request for un-scheduled emergency service. The contractor is required to keep the Contracting Officer and COR informed by providing a work order or repair order detailing the repair work to be performed and any corrective action. In addition, if the contractor fails to maintain or provide adequate supplies of spare parts sufficient to install necessary replacement parts within 24 hours of the establishment of need for such parts, performance will be classified as unacceptable unless the part(s) are related to a major and unforeseen repair requirement, and the part(s) cannot be obtained via overnight express delivery. 11. General Information 11.1. Competency of Personnel Servicing Equipment and Appearance. The contractor shall have in its employment at all times a sufficient number of capable and qualified employees to enable it to properly, adequately, and safely manage, operate, maintain and account for the elevator system. The Contractor shall have an established business, with an office and full time staff. The staff includes a "fully qualified" FSE and a "fully qualified" FSE who shall serve as the backup. "Fully Qualified" is based upon training and on experience in the field. For training the FSE(s) has successfully completed a formalized training program for the equipment identified in the schedule. The FSE(s) shall be authorized by the Contractor to perform the maintenance services. All work shall be performed by "fully qualified" and competent FSEs. The contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSEs for each make and model the contractor services at the medical center. The Contractor's personnel shall wear distinctive clothing bearing the name of the company. 11.2. Work Hours. Normal work hours are 8:00 AM to 4:30 PM (CST/CDT), Monday through Friday. The Contractor shall perform all scheduled services during normal work hours. The Contractor may be authorized and permitted to work outside these normal work hours, including extension of daily work hours and/ or weekend work, by first obtaining approvals from the affected COR or designee and Contracting Officer, and at no additional cost to the government. 11.3. Reports of Services/Documentation. Upon completion of any maintenance and/or repair services, the contractor shall report to the COR or designee and provide a written report detailing services performed and obtain an acceptable signature. (Service Ticket) 11.4. Contractor Personnel Reporting Procedures. Upon arrival at the station, the contractor shall check in with the COR or his designee so that the medical center will be aware of the contractor's presence at the facility at all times. Upon each departure, contractor will serve notice of disposition of work to the COR or designee. In addition the contractor shall furnish the COR or his designee a copy of a work order, service sheet or other such written notification of services performed. Such reports shall include the following information: "Name and address of contractor "Name of contractor employee(s) performing the work "Date(s) work performed and hours spent. "Brief description of work performed, including PMI's and identification of equipment worked on. "Signature of contractor's employee(s) and signature of COR or designee. "If the report is not submitted it will indicate that no inspection or maintenance was performed and payment may be withheld and/or delayed. 11.5. Security Requirements. In the interest of installation security, all vehicles entering and operating on the installation are subject to search at any time. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the installation, shall abide by all security regulations of the installation. The contractor shall not enter any controlled areas without prior coordination with ISO and Police Services and obtain the proper security escort if required. 11.6. Safety. The contractor shall adhere to all OSHA, EPA, NFPA Life Safety Codes, and all other regulatory requirements. In performance of this contract, the contractor shall follow medical center safety policy and standards for safe work practices, and take such safety precautions as the Safety Officer or designee may determine to be reasonably necessary to protect the lives and health of occupants of the building. The contractor shall comply with medical center smoking policy, which designates all interior space as non-smoking areas. Patient, employee, visitors, and contractor personnel safety shall be maintained at all times. The contractor shall replace all safety guards, equipment, devices, etc., removed to service or repair the elevator immediately after completion of work or when leaving the job unattended. 11.7. Hot Work Permit. The contractor shall obtain a Hot Work Permit from the Safety Office, through the COR, whenever soldering, welding, using a cutting torch, or other open flame, spark, or heat producing equipment. The contractor is required to follow ALL requirements outlined for the issuance of the Hot Work Permit. 11.8. Material Safety Data Sheets. The contractor shall submit to the contracting officer, within 15 days of contract performance, the Material Safety Data Sheets (MSDS) for all potentially hazardous materials (lubricants, cleaners, working fluids, etc.) to be used in the facility in performance of the contract, and will not use, in the facility, such materials which have not been so cleared in advance with the medical center. MSDS for new chemicals shall be furnished concurrently with arrival of the chemical on-site. The Contractor shall maintain a copy of all MSDS at the chemical storage site in a location accessible to medical center personnel to assure compliance with all laws and requirements regarding the "Right to Know" law. 11.9. Personal Protective Equipment. The contractor is responsible to identify, provide and maintain all personal protective equipment (PPE) required to perform the duties outlined in the contract. In addition, the contractor is responsible for identifying and providing all applicable safety programs (i.e. lockout/tag out, confined space entry, universal precautions, etc.) required to perform the work. Training on required safety programs and the proper use of PPE's shall be provided, and documentation maintained by the contractor. 11.10. Injury or Accident. Contractor shall submit a written report to the medical center within five work (5) days of any incident of a major malfunction, accident or injury involving an elevator. The report shall state the cause, what and when repairs and tests were or are to be made to correct the cause of the incident. 11.11. Non Compliance Notification. The contractor shall be notified of any non-compliance with the previous provisions. After receipt of such notice, the contractor shall immediately correct the condition and notify the contracting officer in writing that the condition has been corrected. If, in the opinion of the Safety Officer, or designee, the condition is life threatening he/she shall instruct the contractor to stop work until the condition has been corrected. If the contractor fails or refuses to comply promptly, the contracting officer may issue and order halting of all or any part of the work, and contractor may be held in breach. The contractor bears all costs of stop work time and correcting safety hazards, 11.12. Off-Site Repair of Government Property. If the Contractor and the Government mutually agree to remove equipment to the Contractors office location, when repairs cannot be made on site, the Contractor shall be responsible for any damage or loss from the time the equipment is removed from the Government installation until it is returned and reinstalled, 11.13. Disposal of Removed Materials. All waste and/or worn and replaced materials removed under this Contract shall become the property of the Contractor. The Contractor shall remove these materials from OBVAMC to a legitimate disposal location and the cost for this removal is considered included in the bid prices. 11.14. Submittals. Contractor shall prepare a detailed calendar schedule for his performance of monthly maintenance services' and annual load testing services at each elevator location, and submit the schedule with his/ her Bid, After review and approval of the successful bidder's schedule by the - Contracting Officer, it shall be binding (provided there are no emergency closures) upon both the Contractor and the Government as to the specific dates services will be performed by the Contractor.
