Description
JUSTIFICATION FOR SINGLE SOURCE AWARDS IAW FAR 13.106-1 (OVER MICRO-PURCHASE THRESHOLD($3.5K) BUT NOT EXCEEDING THE SAT ($150K)) IAW FAR13.104, COs must promote competition to the maximum extent practicable to obtain supplies and services from the source whose offer is the most advantageous to the Government, considering the administrative cost of the purchase. When competition is not practicable, IAW FAR13.106-1(b), COs solicit from a single source for purchases not exceeding the simplified acquisition threshold. COs may solicit from one source if the CO determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). IAW FAR13.106-3(b)(3),COs are required to include additional statements explaining the absence of competition (see 13.106-1 for brand name purchases) if only one source is solicited and the acquisition does not exceed the simplified acquisition threshold (does not apply to an acquisition of utility services available from only one source) or supporting the award decision if other than price-related factors were considered in selecting the supplier. This template when completed can be used to document single source awards IAW FAR13.106-3(b)(3). Note: Statements such as "only known source" or "only source which can meet the required delivery date" are inadequate to support a sole source purchase. 1. ACQUISITION PLAN ACTION ID: VA259-16-AP-50301A. PROJECT/TASK No. None1B. ESTIMATED AMOUNT: redacted 2. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: DESCRIPTION: The VASOVIEW PRO 6 Harvesting Cannula is designed for use in conjunction with the Maquet 7mm endoscope. The Harvesting Cannula has four lumens which house the endoscope, C-Ring, distal lens washer tube and Bipolar Bisector for ligation and division of vessel branches. The C-Ring/distal lens washer is independently controlled by a C-Ring Slider on the handle of the device for retraction of the vessel as well as washing of the distal tip of the endoscope. The Bipolar Bisector can be extended/retracted/rotated with the Bisector Carriage. Mechanical cutting is achieved with the Bisector Toggle located on the Bisector Carriage. Bipolar coagulation is achieved using electrosurgical energy. Positioning of the device, coagulation and mechanical cutting are performed under endoscopic visualization. The device is intended for use with bipolar outputs of the Maquet Endoscopic Vessel Harvesting (EVH) System. INTENDED USE: The VASOVIEW PRO 6 Harvesting Cannula is a disposable item used in minimally invasive surgery allowing access for vessel harvesting, and is used on patients undergoing endoscopic surgery for arterial bypass. It is indicated for cutting tissue and controlling bleeding through coagulation, and for patients requiring blunt dissection of tissue including dissection of blood vessels, ducts and other structures in the extra peritoneal or subcutaneous extremities and thoracic space. Extremity procedures include tissue dissection/vessel harvesting along the saphenous vein for use in coronary artery bypass grafting and peripheral artery bypass or the radial artery for use in the artery bypass grafting. Thorascopic procedures include exposure and dissection of structures external to the parietal pleura, including nerves, blood vessels and other tissues of the chest wall. 3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: The VASOVIEW PRO 6 Harvesting Cannula must be compatible with existing Maquet equipment and Storz Camera System. On 9/20/2013, the Salt Lake VAMC awarded PO #VA259-13-P-4092 for the purchase of a Maquet Endoscopic Vessel Harvesting (EVH) System. On 3/9/2016 the Salt Lake VAMC awarded PO #VA259-16-P-1891 for the purchase of Maquet endoscopes. The Maquet EVH System and endoscopes are only compatible with Maquet Vessel Harvesting Cannulas. 4. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: A review of national contracts and prime vendors showed that this requirement is not available from a VA NAC contract source. A review of current WSNC BPAs as well as VISN, regional and local IDIQs did not find any vendors able to fulfill this requirement. Other sources including, AbilityOne, Federal Prison Industries (FPI) and excess stock from other agencies returned negative results. An internet search showed that these items are available via open market from small business distributors.
