Description
The Department of the Interior, Bureau of Reclamation, Great Plains Region has a requirement for several Associated Facility Reviews (AFR) located in Kansas and Nebraska. The period of performance is 90 calendar days. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-89. In accordance with the procedures in FAR Part 12 and 13, RFQ Number R16PS01618 is available for viewing through electronic commerce via https://www.fbo.gov and via Reclamation's website at http://fedconnect.net. Solicitation number R16PS01618 is issued as a request for quotation (RFQ). This RFQ is set aside for small business under NAICS 517110 with an associated small business size standard of $15 million. The resulting purchase order will be a firm-fixed price type purchase order. The line items are listed as follows: Line Item 00010: Temporary Service for Associated Facility Reviews in accordance with Performance Work Statement; 1 Lump Sum. Bureau of Reclamation Great Plains Region Associated Facilities Reviews PERFORMANCE WORK STATEMENT 1. SCOPE The work consists of performing all the tasks of conducting facility reviews in accordance with Bureau of Reclamation Directives and Standards (D&S) (FAC 01-04 for Associated Facilities and FAC 01-07 for High- and Significant-Hazard Dams). D&S Manual. Locations and Type of Reviews include: ¿Associated Facility Reviews: oKirwin Irrigation District, KS oTwin Loups Irrigation District, NE oFrenchman-Cambridge Irrigation District, NE 2. TASKS 2.1 Coordination with the Area Office and/or the facility to schedule a facility examination. 2.2 Records research to prepare for the examination, including a review of past recommendations and significant events since the last facility review. Contractor will be given past reviews and reports. 2.3 Travel to and conduct facility examination in accordance with the D&S. 2.4 Preparing a facility review report in accordance with the D&S requirements and format that documents the status of outstanding recommendations and the need for any new recommendations. A draft copy of the report shall be distributed to the Area Office and the Quality Assurance Evaluator (QAE). The review period shall be a minimum of 2 weeks. 2.5 A final report shall be submitted to the QAE for signature approval. 2.6 The contractor shall keep a log of hours work and task completed and provide a weekly copy to the QAE All tasks shall be completed with no deviations from the D&S or stated requirements. 3. PERIOD OF PERFORMANCE The term of the purchase order shall be 90 calendar days from the date of the Notice to Proceed. RECOGNIZED HOLIDAYS. The contractor is not required to provide services on recognized Federal holidays established by Federal law (5 U.S.C. 6103). GOVERNMENT FURNISHED PROPERTY Description of PropertyQuantity and Unit of MeasureAcquisition Cost if Damaged or LostUnique Item IdentifierCondition TBDTBDTBDTBDTBD The following government equipment will be provided as determined necessary by the Government: ¿Pit gage ¿Camera ¿Air monitor ¿Any other equipment that is determined necessary by the Government to conduct an inspection. Personal protective gear, such as steel-toed boots, prescription safety glasses, harnesses, etc. will not be provided and shall be provided by the contractor. The contractor will be provided direct access to hard copy files. Electronic files will be provided by the government The contractor shall obtain any needed electronic information by requests through the QAEor designated staff. 5. SAFETY The contractor shall perform the work in accordance with Reclamation ¿s Safety and Health Manual. A safety meeting shall be held between the contractor and the QAE prior to conducting any facility examinations to discuss safety to insure the contractor is aware of safe working practices. 6. DELIVERY The contractor shall provide a draft electronic report for a 2-week review period to the appropriate Area Office and QAE within 60 days of the site examination. A final review report shall be submitted to the QAE for signature from the Facility Operation Services Group Supervisor of the Great Plains Regional Office prior to the conclusion of the performance period of the contract. 7. QUALITY ASSURANCE EVALUATOR (QAE) The Contracting Officer (CO) has designated the employees listed in Section 11 of this PWS as the QAEs to act as the authorized representative, which assists in the technical administration of the purchase order. The CO has not authorized the QAE to make any contractual commitments, or to authorize any contractual changes on the Government ¿s behalf. If any doubt exists as to the extent of the authority of the QAE, the contractor shall contact the CO for guidance before taking action on the matter. The QAE will determine whether the contractor has consistently satisfied the expected quality level as per the contractual requirements. 8. PERFORMANCE CRITERIA The QAE will document acceptable and unacceptable performance. When the QAE considers the contractor's performance to be unsatisfactory, the QAE will report the information to the CO. The CO may require the contractor to explain, in writing, the cause of the poor performance, any corrective actions the contractor has already taken to obtain an acceptable level and any future corrective actions the contractor will utilize to prevent unacceptable performance. The contractor ¿s administration personnel may be required to meet with the QAE, as determined necessary, to discuss the contractor ¿s performance, to include actual or potential problems. All parties shall make a mutual good faith effort to resolve any issues. All parties to the discussion shall review and sign written minutes of the meeting. Should the contractor not concur with the minutes, the contractor shall state any areas of non-concurrence in writing to the CO within 7 business days of receipt of the minutes. Whenever the CO issues a Purchase Order Discrepancy Report, the QAE will hold a meeting if the contractor requests such a meeting. 9. PERFORMANCE REQUIREMENTS SUMMARY TABLE (PRST). GOVERNMENT REMEDIES All work shall be subject to acceptance by the Government. The Government will consider work incomplete when any one of the following conditions exits: - The contractor did not perform any or all of the tasks. If the performance of any required service is unsatisfactory, and poor performance is clearly the fault of the contractor, the Government may reduce the purchase order price to reflect the reduced value of defective services, if the Government gives the contractor a written Purchase Order Discrepancy Report prior to executing such payment deductions. 11. (SEP 2002) 52.212-2 Evaluation Commercial Items (OCT 2014) (a) The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, considering the following evaluation factors: (1) Employee Experience (2) Price The above evaluation factors are listed in order of priority (in accordance with FAR 12.602, Reclamation will not describe the relative importance of the evaluation factors). Prospective Offerors shall provide enough information in order to make an adequate determination regarding the employee experience: (1) Employee Experience - Employee experience is an objective criterion that determines whether the contractor has or has not performed similar work. Therefore, the contractor shall furnish a list (no more than five) of Federal, State, local government, or commercial projects similar in scope, complexity, and magnitude to the work required under this solicitation that the contractor has completed within the last three years. These projects must clearly demonstrate the contractor ¿s successful ability to accomplish projects in a timely and satisfactory manner. At a minimum, the contractor shall provide the following information for each project: (i) Name of the Customer (Government Agency or Commercial); (ii) Purchase order number, type, date, name of project; (iii) Description of the work; (iv) Initial purchase order amount and final purchase order amount; and (v) Name, telephone number, and e-mail address of a point of contact for the customer that the Government may contact to confirm and verify the information provided. The Government may not evaluate references with incorrect or missing phone numbers and contact names, which may potentially result in a less favorable rating of the contractor ¿s offer. The Government may also use information obtained from other than the sources identified by the offeror. Information obtained from the Past Performance Information Retrieval System (PPIRS) database located at http://www.ppirs.gov is one of the sources that will be utilized. PPIRS functions as the central warehouse for performance assessment reports received from several Federal performance information collection systems and is sponsored by the DOD E-Business Office and administered by the Naval Sea Logistics Center Detachment Portsmouth. (2) Price ¿ Contractor shall provide pricing in a lump sum for 720 hours (90 days). (b) Options - [RESERVED] (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Quotes are due by Tuesday, August 23, 2016 by 4:30 p.m. Mountain Time. Reclamation will consider all responsible sources who submit a timely quote for award.
