Description
This is a SOURCES SOUGHT NOTICE in support of VISN 20 for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described below. Potential offerors are invited to provide feedback via e-mail to Tracey Linna at tracey.linna@va.gov. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. All responses are due by August 16, 2016, at 1600 PDT. Potential contractors shall provide, at a minimum, the following information: 1) Company name, address, and point of contact, phone number, e-mail address, and DUNS. 2) Is your firm eligible for participation in one of the following small business programs? If so, please indicate the program. Anticipated North American Industry Classification System (NAICS) code is 562112 - Hazardous Waste Collection Services. The largest a firm can be and still qualify as a small business for Federal Government programs is no larger than $38.5 Million. The Capabilities Statement for this sources sought is not expected to be a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding the company's ability to provide the services outlined in the draft SOW below. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. Submission of capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business (SB) Community or to conduct as an Unrestricted Procurement. Other than small businesses may respond to this notice in the event the market does not indicate SB interest. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for information solicited. Respondents will not be notified of the results of the evaluation. Note: Do not include Proprietary, classified, confidential, or sensitive information in responses. Not responding to this Sources Sought does not preclude participation in any future or potential solicitation, or Request for Quotation. It is the intent of VISN 20 use the information gathered for market research purposes only. If a formal solicitation is released, it will be posted via the Federal Business Opportunity (http://www.fbo.gov). DESCRIPTION OF THE REQUIREMENT: WWVAMC CBOC Medical Waste Collection Removal and Disposal Part I: General Information A.Introduction The Jonathan M. Wainwright Memorial VA Medical Center, 77 Wainwright Drive, Walla Walla, WA, Community Based Outpatient Clinics (CBOCs) requires services to perform Medical Waste Collection Removal and Disposal as required by local State and Federal Regulations and Laws. The CBOCs are located at: -Yakima, WA VA Medical Center 717 Fruitvale Blvd 98902-1465 -Richland, WA VA Medical Center 825 Jadwin Ave 2nd Floor 99352-3589 -Lewiston, ID VA Medical Center 1630 23rd Ave Bldg #2 83501-6350 -La Grande, OR VA Medical Center 202 12th Street 97850 B.Scope Contractor shall provide all necessary labor, equipment and supplies to collect, remove, transport, and dispose of medical waste according to all applicable local state and federal regulations and laws every 30 days. Services shall be required for the CBOCs at Yakima WA, Richland WA, and Lewiston ID, La Grande OR. Contractor shall provide medical waste approved collection/disposal containers at each CBOC location and shall invoice per medical waste containers removed and disposed. Containers shall be 40-50 gal size. Contractor shall report to the Clinic Manager when arriving to perform service. When the service is complete the Contractor will verify quantities and check out with Clinic Manager. C. Deliverables Contractor shall provide documentation that all medical waste removed from the CBOCs was collected, removed, transported, and disposed of in accordance with local state and federal regulations with monthly invoices per location (i.e. Yakima, Richland, Lewiston, and La Grande). Invoices will be sent to: Attn: Facilities Management Service 77 Wainwright Drive Walla Walla, WA 99362 Documentation to include Medical Waste Weights will be sent to: Attn: Environmental Coordinator - Randy Stujenske 77 Wainwright Drive Walla Walla, WA 99362 Part II: Bid Items 1.Medical Waste Collection Removal and Disposal Yakima, WA VA Medical Center 2.Medical Waste Collection Removal and Disposal Richland, WA VA Medical Center 3.Medical Waste Collection Removal and Disposal Lewiston, ID VA Medical Center 4.Medical Waste Collection Removal and Disposal La Grande, OR VA Medical Center Part III: Supporting Information A.Security Contractor shall be escorted to medical waste storage room for container drop-off and pickup. B.Safety Requirements In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default. C.Qualifications of Key Personnel: The Contractor's staff shall include a fully qualified Field Service Representative assigned to this area and a fully qualified Field Service Representative who shall serve as the backup. Fully qualified is based upon training and on experience in the field. For training, the Field Service Representatives must have successfully completed a formalized training program for the equipment covered under this contract. For field experience, the Field Service Engineers must have a minimum of five years of experience. Contactor shall be licensed in the state where the service is provided for medical waste transportation and disposal. D.Hours of work Hours of Work are defined as Monday through Friday from 8 a.m. to 4:30 p.m., excluding federal holidays or as otherwise arranged with the Contracting Officers Representative. Work performed outside of normal working hours of coverage at the request of the Contracting Officer or Contracting Officer's Representative shall be billed separately. The ten holidays observed by the Federal Government are New Year's Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Also, any other day declared by the President of the United States to be a national holiday. E.Payment for Services Invoices will be paid in arrears on a monthly basis upon completion of the services. Each invoice shall include VA purchase order number, invoice number, the month of the service being invoiced, description of container type and size, quantity of containers, weight of medical waist, and price per container.
