- SUBJECT: Medical Readiness and Training Command (MRTC) Human Worn Surgical Simulators, United States Army Medical Command (MEDCOM), Fort Sam Houston, TX SOL#: W81K04-16-T-HWSS THIS IS A REQUEST FOR INFORMATION (RFI) ONLY; it is not a Solicitation (i.e. Request for Proposal, Request for Quotation, or Invitation for Bids) or an indication that the Health Readiness Contracting Office (Provisional) (HRCO(P)) will contract for the supplies contained in the RFI. This RFI is part of a Government market research effort to determine the scope of industry capabilities and interest and will be treated as information only. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI are strictly voluntary and the Government will not pay respondents for information provided in response to this RFI. Responses to this RFI will not be returned and respondents will not be notified of the result of the review. If a competitive solicitation is issued, it will be announced on the Federal Business Opportunities website http://www.fbo.gov at a later date, and all interested parties must respond to that Solicitation announcement separately from any response to this announcement. This RFI does not restrict the Government's acquisition approach on a future Solicitation. The anticipated effective date for award is September 23, 2016, with an expected delivery date of 60 days after the receipt date of the order (RDO). The corresponding North American Industry Classification System (NAICS) Code is 339112 -- Surgical and Medical Instrument Manufacturing, with a Small Business Size Standard of 1,000 employees. DESCRIPTION OF REQUIREMENT: This is a supply requirement to provide medical training equipment to the Army Reserve (AR), (the Department of Defense (DoD)) through the Medical Readiness Training Command (MRTC). The MRTC requests to purchase a quantity of six (6) human worn surgical simulators. Human worn surgical simulators provides training opportunities from point of injury through roles 1-3 of care (role 1 point of injury care by the medic and/or battalion aid station; role 2 is basic primary care to include forward resuscitative damage control surgical care and role 3 is care provided in a fully equipped medical treatment facility/hospital). The purchase of the Surgical Simulators should also include training for twelve (12) end-users, two end-users will be designated for each simulator purchased, on the proper operation and maintenance of the equipment. Shipping shall be included, in the quantity identified to six (6) separate locations throughout the Continental United States. The Contractor shall provide a copy of the standard commercial warranty/warranties with its offer. Each Cut Suit comes with a limited warranty for one year against defects in material and workmanship subject to conditions of use. RESPONSES: Respondents to this RFI are to describe their interest and ability to perform the requirements summarized within this notice and their ability to deliver all equipment in 60 days. Please specify if the equipment is commercially sold within the Private or Government Sector. Please include specification sheets on the suggested items offered to include photos, features and abilities, and any related warranty information. In addition, responses are to contain (1) company name, (2) DUNS number and CAGE code, (3) business size pursuant to NAICS code 339112 (size standard, 1,000 employees), (4) mailing address, (5) primary point of contact information, to include telephone number and email address. (6) If applicable, current GSA contract number and applicable SINs. Do not include any pricing as this is only a request for information only. Proprietary/Competition Sensitive information (appropriately marked) will be protected from disclosure to the greatest extent practical, however it is preferred that respondents do not provide proprietary or otherwise restricted responses. Responses should be formatted as either MS Word (.doc) or Adobe Portable Document Format (.pdf) and should be limited to a maximum of five (5) pages. No faxes, courier delivered, or telephone inquiries/submissions will be accepted. All interested vendors should respond in accordance with the above paragraph. Please include the RFI Identification Number and Title CUTS in the subject line of the email along with the RFQ identifier number, W81K04-16-T-CUTS. Responses must be received no later than 2:00 PM CDT on Thursday, August 16, 2016. Failure to submit all information requested on a timely basis will result in a contractor not being considered as an interested business. Respondents will not be notified of the results.
