- 1 Aug 2016 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. 2. FA5209-16-Q-1012 for Portable Toilets Rental and Servicing is hereby issued as a request for quotes. This purchase obligates appropriated funds of the United States Government and non-appropriated funds that are generated by the military community through the sale of goods and services, and the collection of fees and charges for participation in military community programs. 3. Following solicitation document, incorporated provisions, and clauses are in effect through Federal Acquisition Circular 2005-88 effective 15 June, 2016 and the Air Force Federal Acquisition Regulation Supplement (AFFARS) Acquisition Circular 2015-1001 effective 01 Oct 2015. It is the responsibility of the offeror to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil/. 4. The NAICS for this requirement is 562991 Septic tank and Related Services. Small Business Size Standard for this NAICS code is $7.5 Million. 5. There is no set aside for this RFQ in accordance with FAR 19.000(b) as this contract will take place outside of the United States and its outlying areas. 6. The Government request quotes for the following items; CLIN DESCRIPTION QUANTITY 0001 Portable Toilets Renting and Services Per Performance Work Statement (See Attachment 1) 1 Each 7. Period of Performance for this service shall be 16 Sept, 2016 - 19 Sept 2016 reporting to the 374th Airlift Wing. 8. FAR 52.212-1 Instruction to Offerors -- Commercial Items (OCT 2015) applies to this acquisition. Addenda to the following paragraphs of 52.212-1 are: (b)(10) Present/Past Performance Reference List: Present/Past Performance Reference List shall be submitted to 374 CONS/LGCB via facsimile at 042-530-3319: ATTN: Demarquis Bell (or Keisuke Kiyotaka), or email at demarquis.bell@us.af.mil (and/or keisuke.kiyotaka.jp@us.af.mil). Offeror shall submit the reference list of at least one (1) contract performed (on-going or completed) as prime Contractor, subcontractor, joint venture and/or teaming partner of the most relevant contract (similar in scope, size and complexity) for US Government agency, Government of Japan or its political subdivision, or commercial customer within the last three years of this solicitation release date. Furnish the following information for each reference on the list of contracts information, using Attachment 2 of this solicitation: a. Name of contracting activity (e.g. Government Agency/Company name) b. Point of Contact (POC), Contracting Officer and/or Contract Administrator's Phone/FAX number, and e-mail address (if available) c. Contract number and project title d. Contract Type (e.g. Firm-Fixed Price, IDIQ, Requirements type) e. Total contract value f. Performance period (e.g. date/month/year through date/month/year) g. Description of contract work performed h. Contracting Officer's name and phone number i. Government Inspector/commercial project manager and phone number j. List of major subcontractors if applicable In addition, the offeror may provide information on problems encountered on the contracts identified in b (10) above and corrective actions taken to resolve these problems. This may include a discussion of efforts accomplished by the offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. (b)(11) In submitting proposed prices and complete Offeror Representations and Certifications, the offeror accedes to the contract terms and conditions as written in this synopsis with all attachments. (c) Period for acceptance of offers -- The offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers." 9. FAR 52.212-2 Evaluation -- Commercial Items (OCT 2014) does not apply to this acquisition and offers will be evaluated based on price and past performance in accordance with procedures prescribed in FAR 13.106-2(b). An award will be made based on whether the lowest priced of the quotations or offers having the highest past performance rating possible represents the best value when compared to any lower priced quotation or offer. a. Past performance rating will be determeined based on the past performance information submitted by the offeror in view of the following criteria. (A) Recency : The Government will confirm whether contracts submitted by the offeror has been completed within the past 3 years from the solicitation release date or are currently in progress. (B) Relevancy: Relevancy is defined as similar scope, size and complexity when compared to this solicitation such as the number of visitors, the size of facilities, the number of portable toilets rented, duration of the events and etc. The Government is not bound by the offeror's opinion of relevancy. (C) Quality: The Government will determine whether performance met or exceeded contractual requirements by customer surveys, past performance questionnaire replies; the Governmentwide Past Performance Information Retrieval System (PPIRS) at www.ppirs.gov; or any other reasonable basis. b. Offerors with no relevant past performance history are treated neither favorably nor unfavorably, however, the Government may use information obtained from other sources to determine a final past performance rating. c. The government intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest. 10. FAR 52.212-3 Offeror Representations and Certifications -- Commercial Items applies to this acquisition. This means that the contractor must have valid Representations and Certifications filed in the System for Award Management (SAM) or provide the government a completed copy of the record. 11.
