Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation, a written solicitation WILL NOT be issued. Solicitation Number P16PS02543 is issued as a Request for Proposal (RFP) for the Contractor to provide all labor and materials to complete a Condition Assessment Report with drawings and provide a Monitoring Recommendation Report for the interior and exterior of the Cape Hatteras Light House brick shaft, Cape Hatteras National Seashore, Buxton, NC. The detailed Scope of Work for the project is provided in Attachment 1, Section C, Descriptions/Specifications/Work Statement, consisting of 3 pages. In addition to Attachment 1, two other Attachments are provided as follows: Attachment 2, entitled ¿Cape Hatteras Lighthouse, 13 Pages and Attachment 3, Photograph of Cape Hatteras Lighthouse ¿, 1 Page. The period of performance for completion of this work is October 3, 2016 through November 30, 2016. A draft of both reports shall be submitted for the Contracting Officer ¿s Representative (COR) review and approval by November 14, 2016. The Government anticipates award of a firm-fixed contract on or before September 19, 2016. In order to view all of the documents, drawings and attachments to the Solicitation posting, proceed to Fed Connect and use the ¿Public Opportunities Only ¿ search and enter the subject solicitation number under ¿Reference Number ¿ in the search criteria. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-88-1. Offeror ¿s doing business with the Federal Government must be registered in the System for Award Management at www.sam.gov. There is no fee to register in the system and the registration process can take several weeks. As part of the registration process a Dunn and Bradstreet Number will be provided. Confirmation of registration will be requested as part of the Business Proposal submission identified below. The North American Industry Classification System (NAICS) is 541350. The Solicitation is a total Small Business Set-Aside, Size Standard $7.5 million dollars average annual receipts for the last three years. Destination; FAR 52.249-8 Default (Fixed Price Supply-Service). Proposals shall be submitted in an original and two copies no later than 12:00 p.m., EST on August 30, 2016 to: Historic Preservation Training Center, 4801A Urbana Pike, Frederick, Maryland 21704. The following weighted criteria will be used to evaluate the proposals received. Proposals shall include the following information: (1) Technical Approach: The ability to perform the work outlined must be demonstrated in a brief narrative. Specifically address the following in detail: (a) Identify each of the key tasks required to successfully complete this project, whether performed by your staff or through the use of subcontractors. Explain how subcontracted work, if any, will be coordinated and carried out. (b) Provide details on the method of inspection for the interior and exterior of the brick shaft of the lighthouse. (c) Provide a schedule identifying dates of site work and submittal dates for draft and final reports. (d) Provide copy of certification through either the Society of Professional Rope Access Technicians or other similar certification. Maximum Score 40 Points; (2) Previous Work: The Offeror shall provide a portfolio including photographs of at least two completed projects which demonstrate the ability of the Offeror, personnel and designated subcontractors to complete the work specified for this project. For each project provide the following: (a) Name and location of the project; (b) Name of the client, contact person and telephone number; (c) Offeror ¿s role in the project (i.e. primary contractor, subcontractor, etc.); (d) Describe the specific work performed by the Offeror for each project; (e) Name and specific job responsibilities of proposed key personnel including subcontractors who worked on the project; (f) award price and actual completion price; (g) Explanation of any modifications; and (g) Date of contract award, scheduled completion date and actual completion date. Projects described shall demonstrate the capabilities of personnel proposed for use under this contract. If no project exists for the current employer, describe work produced under previous employer. Maximum Score 30 Points; and (3) Qualifications and Relevant Experience of Proposed Personnel: The Offeror shall identify all personnel proposed for use under this contract. The list shall identify each person by name, title, role in the contract, background, experience including with historic buildings and historic preservation activities, and certifications or specialized training. In this list clearly identify if personnel are part of your staff or a subcontractor. ¿ Maximum Score 30 Points. The following additional information shall also be provided as part of the Business Management submission: Pricing information broken out as follows: (1) Crack documentation of lighthouse tower 1 Job at $____________________; (2) Travel 1 Job at $_________________; (3) Crack Documentation and Monitoring Report Recommendations 1 Job at $______________ and Total Firm-Fixed Price $__________________. Also as part of your Business Management Cost Proposal submit the additional information as follows: (4) Completed copy of FAR 52.212-3, Offeror Representations and Certifications ¿Commercial Items, or a statement that Annual Representations and Certifications have been done in accordance with FAR 52.204-08. (3) Originally signed copy of Standard Form 1449 Form (SF) 1449 which is accessed via www.fedconnect.net by conducting a ¿Public Opportunities Only ¿ search and entering the subject solicitation number under ¿Reference Number ¿ in the search criteria. On the Standard Form 1449, the Offeror shall guarantee that pricing is good for 60 days after the closing date for receipt of proposals and (5) Confirmation of registration in SAM with Dunn and Bradstreet Number. Award of any contract resulting from this solicitation will be made to the responsible Offeror whose offer, conforming to the solicitation will be based on best value to the Government, cost or price and other factors considered. All evaluation factors other than cost or price, when combined are significantly more important than cost or price; however, cost/price may contribute in the selection decision. The Government reserves the right to make award without further discussion. The Government anticipates award of a firm-fixed price contract on or before September 19, 2016.
