Attachment 1 Volume 2 has been deleted and replaced with Attachment 1 Volume 2 Complete. AMENDMENT A001: Issued July 20, 2016 to change the room number for the pre-proposal conference/site visit scheduled for 9:00 a.m. CST on July 26, 2016. The Federal Aviation Administration (FAA) has a requirement for an Elevator Addition in Hangar 8 at the Mike Monroney Aeronautical Center (MMAC), Oklahoma City, OK. All work shall be accomplished in accordance to the project specifications and drawings. The work to be accomplished under this contract consists of the addition of an Accessible elevator reaching three floors of the western entry of Hangar 8. Addition is comprised primarily of an elevator shaft, landings at each floor, elevator equipment room, vestibule, corridor finishes improvements, mechanical & electrical additions/updates, and infill of an existing elevator shaft at each floor to create new storage rooms. Add approximately 1,025 gross square feet total to include the addition of a new entry vestibule and elevator equipment room at the first floor and new elevator and extended new landings at each upper floor. Renovate approximately 790 existing gross square feet total, for three floors, all interior finishes of the primary corridors accessing the new elevator, adjacent women 冱 toilets, and infill floor at removed elevator shaft to reclaim space as storage at each floor. New lighting design is included within the renovated spaces. It is anticipated that a firm-fixed-price type construction contract will be awarded as a result of this solicitation. Your attention is directed to Sections L and M of SIR/RFO DTFAAC-16-R-05694 for instructions for preparation and submission of proposals. The SIR includes an evaluation of past performance. Offerors are required to provide Past Performance Surveys to customers/contractors who are familiar with the work ethic, standards, performance, and deliverables of the prime/general contractor demonstrated through previous or ongoing contracts of a similar nature [Provision L.3(f)(1).] Offerors will complete Page 1 and Page 2, Section A of the Past Performance Surveys and then provide the surveys to their customers, who will in turn complete and return the surveys directly to the Contracting Officer by the due date specified on the surveys. Three (3) to five (5) Past Performance Surveys must be submitted directly to the Contracting Officer. Surveys received after the established due date may not be evaluated. This requirement is 100% set-aside for competition among Small Businesses. The NAICS code applicable to this requirement is 236220 Commercial and Institutional Building Construction. The business size standard for this NAICS is $36.5 million. A pre-proposal conference/site visit will be held on July 26, 2016 at 9:00 a.m. CST in the Headquarters Building (Building No. 1), conference room BS-21 at the MMAC, Oklahoma City, OK. Attendees should enter the Aeronautical Center's main west entrance gate on South Regina Avenue and proceed to the Security Command Center just inside this entrance. Guards at the Security Command Center will direct attendees to the Headquarters Building. Service trucks will need to enter through the Commercial Entrance on SW 74th Street. Contractors must be registered with the System for Award Management, www.sam.gov on the date established for receipt of offers (August 18, 2016).
