- STATEMENT OF WORK Camp Butler National Cemetery in Springfield, IL 62707 is looking for a weekly uniform rental and laundering service. Two sizes of floor mats are to be included in the rental and cleaning. These uniforms have four different patches on them and there is one uniform in a non-standard size. ITEMQUANTITY 3x5 Gray Mat3 4x6 Gray Mat3 Perma Lined JKT16 Comfort Pant22 Cargo Pant110 Comfort Shirt121 SIZE PREMIUMQUANTITY Perma Lined JKT2 Cargo Pant11 Comfort Shirt11 1.There are currently 8 permanent employees and 3-4 summer hires (Jun-Sep). This number is predicted to remain very stable. 2.Employees receive 11 short sleeve shirts for summer and 11 long sleeve shirts for winter. The summer temp hires do not receive long sleeve shirts. 3.Each employee receives 2 jackets. The summer temp hires do not receive jackets. 4.Every week a week's worth of uniforms are picked up for laundering and the previous' week's laundered uniforms returned. The mats are also picked up this way. a)Garments should be laundered and pressed, free of spots, stains and odors when returned. b)Repairs shall be made at no additional cost to the Government and shall be returned and repaired on schedule. c)Garments for new employees requiring standard sizes shall be delivered within 1 week of request. 5.Patches are sewn on, employee name over right pocket and cemetery over left. Camp Butler Emblem on left arm and flag on right, sewn on. The employee name and cemetery name are on patches, then sewn to material. 6.The mats have no logo/emblem. 7.All costs incurred should be included in the final price. METHOD OF AWARD - CASCADE PROCEDURES All proposals will be evaluated for responsiveness. Failure to submit all documentation required as required so may result in the submission being determined non-responsive and removed from further consideration. Award will be made to a responsible offeror on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors; only where adequate/sufficient competition is received at a given tier. Remaining proposals/quotes will be reviewed using cascade procedures. This is a competitive Request for Quotes (RFQ) using a Three (3)-Tier cascading set-aside procedure. The set-aside requirement will be as follows: 1st tier- SDVOSB 2nd tier- VOSB 3rd tier- All small businesses
