- VA786-16-R-0357 Pre Solicitation Notice for Sewage Treatment Services for National Cemetery of the Alleghenies 1158 Morgan Road, Bridgeville, PA 15017 RFP VA786-16-R-0357 - Contractor shall furnish all materials, labor, testing / sampling, transportation / lodging, and services necessary to assist National Cemetery of the Alleghenies in the operation of its sewage disposal treatment system. 001Furnish all parts, labor, testing/sampling, transportation/lodging and services necessary to provide operational assistance of a subsurface sewage disposal treatment system to include two (2) site visits per month, in accordance with all terms, conditions, schedules & specifications of the contract.12MO 002Cleanout of Wastewater Treatment system. Remove all debris from tanks and dispose of off-site.2EA 003Complete and file Annual Maintenance Report (AMR) with PA DEP1EA 004ADDITIONAL SERVICES: Services provided beyond the two (2) monthly site visits. This hourly charge includes transportation and labor only. All parts will be quoted separately. AS NEEDEDHOURLY 005Additional water sample collections & analysis.AS NEEDEDEA Fixed Monthly Rates: Invoices shall be submitted monthly (or quarterly as otherwise agreed upon) in arrears to the Director, National Cemetery of the Alleghenies, and include an original and one copy. It is agreed and understood that the above monthly rate includes all supplies, services, labor, and transportation (including per diem and lodging), necessary and incident to the provision of complete Water Operational Assistance Services as specified herein. Additional Services: 1. Hourly rates shall include all supplies, transportation and labor (including per diem and lodging). Prior to starting any additional services the Contractor must receive approval from the COR. 2. Additional water sample collections and analysis will be provided at the request of the COR or at the recommendation of the Contractor with the approval of the COR. A.1GENERAL REQUIREMENTS: The Department of Veterans Affairs desires the services of a contractor to assist in the operation of their sewage disposal treatment system. Contractor shall provide the required licensed sewage disposal treatment system Operator for assistance in the operation of the National Cemetery of the Alleghenies subsurface sewage disposal treatment system. Contractor Operator responsibilities include, but not limited to, the taking and testing of water samples to meet the Pennsylvania Department of Environmental Protection (DEP) requirements and the performance of ordinary Preventive Maintenance. The services to be provided do not include any maintenance of the sewage disposal treatment system other than ordinary Preventive Maintenance, but shall include the following: (a)Two (2) monthly site visits including a monthly routine visual inspection of the system's facilities; (b)Comply with all Terms, Conditions or Effluent Limitations related to: NPDES Permit No. PAG046344 and WQM Part II Permit No. WQG0161140. (c)Register the outfall/headwall by obtaining a General Permit 4 from Bureau of Soils and Waterways; (d)Keeping abreast of changes in the Water Quality Regulations, and informing the Director, National Cemetery of the Alleghenies of those changes; (e)Maintaining Operator certification; (f)Advising as to proper operation and maintenance of pumps, and other mechanical components of the sewage system, if applicable; (g)Accompanying regulatory agencies during sanitary surveys; (h)Assisting in the development and implementation of a preventive maintenance schedule, as necessary; (i)Overseeing the calibration of instrumentation associated with the operation of the system including monitoring equipment, pumping equipment, and flow meters, if applicable; (j)Being available on a regular and emergency basis to include after normal working hours and weekends. (k)Recommending actions to maintain the subsurface sewage disposal treatment system. (l)Performing a semi-annual cleanout of the Wastewater Treatment System which entails removing all debris from the tanks and disposing off site, complying with all State and Federal requirements. A.3OPERATIONS AND MAINTENANCE The contractor shall provide: (a)A primary and secondary Operator, both holding the required Pennsylvania DEP Certified Sewage Treatment Licenses. (b)Minimum of two site visits per month to provide oversight on the proper operation and compliance requirements for the cemetery sewage disposal treatment system. (c)Collection of all required routine samples for analysis by a Pennsylvania DEP Certified Water Laboratory. (d)Completion of monthly performance reports indicating water usage, flow readings, sample results. (e)Filing of the Annual Maintenance Report (AMR) with the PA DEP. (f)Performance of ordinary Preventive Maintenance. (g)Additional services beyond the two (2) monthly site visits will be charged on an hourly basis. This hourly charge will include the supplies, transportation and labor for the service. If additional work is required to complete the repairs such as replacing pumps, the contractor will submit a quote which will include the transportation/labor/parts to complete the repairs. The contractor must receive permission from the COR prior to proceeding with any additional repairs. Project will be for one year, plus 4, one-year options, if exercised. This acquisition is for a 100% Small Business Set-Aside. North American Industrial Classification System (NAICS) Code 221320 is applicable. Please reference Solicitation Number RFP VA786-16-R-0357 on remittances. An electronic solicitation will be available for download from www.FedBizOpps.gov on or about August 10, 2016. Responses to the solicitation are due by 2:00 p.m., September 2, 2016. Offerors are advised that they are responsible for obtaining amendments to the solicitation which is available at Fed Biz Opps.
