- This acquisition is a Prospective 100% set aside for Small Business. If Two or more interested Service Disabled Veteran Owned Small Business (SDVOSB) respond, then the solicitation to acquire services described below, shall be 100% Set-aside to SDVOSB. The NAICS Code is NAICS: 238110 Poured Concrete Foundation and Structure Contractors $14.0 Million size standard. Please note: That this is not a request for quotes or proposals; this is a pre-solicitation notice to inform all interested Small Businesses that the VA National Cemetery Administration, Pacific District as directed by the VA Zachary Taylor National Cemetery, will solicit a Set-Aside proposal. This Request for Proposal (RFP) shall be under No: VA786-16-Q-0261. A RFQ solicitation is currently not available. Subject to all terms, conditions, provisions, and schedules of the contract; contractor shall furnish all equipment, parts, materials, supplies, labor, and supervision necessary and incidental to the work required. The evaluation factors are as follows Lowest Price Technically Acceptable (LPTA). BACKGROUND: The VA National Cemeteries are a resting place for our nation's heroes and as such, the standards for management, maintenance and appearance of these cemeteries have been established by the VA National Cemetery Administration (NCA) to reflect this nation's concern and respect for those interred there. NCA mission is to honor veterans with a final resting place and lasting memorials that commemorate their service to our nation. National Cemeteries are National Shrines. The standards of maintenance and appearance at these cemeteries shall reflect this nation's concern for those interred. PROGRAM OBJECTIVES: The purpose of this Pre-solicitation is to obtain a contractor to support the National Cemetery administration. Location of Services: Zachary Taylor National Cemetery 4701 Brownsboro Road Louisville, KY 40207 Phone: 859-885-5727 (Camp Nelson National Cemetery, Nicholasville, KY) Participation in this effort is strictly voluntary. All costs associated with responding to this Presolicition Notice will be solely at the interested respondent's expense. The objective of this Presolicition Notice is to allow industry the opportunity to provide comments to the VA NCA Pacific District. Review of the responses to the Presolicition Notice will focus on the offeror's technical input and clarification of requirements. Presolicition Notice Response Due Date: The VA NCA Pacific District will only entertain written communication; no verbal communions will be accepted. Please submit written information via e-mail to Joseph Mendoza, Contracting Officer, at joseph.mendoza1@va.gov no later than 19 August 2016 4:00 p.m. Pacific time. Presolicition Response Contact: Respondents to this Presolicition Notice shall designate a primary and one alternate point of contact within the company (Name, Address, Email, and Telephone). Clarification of Presolicition Notice Responses: To fully comprehend the information contained within a response to this Presolicition Notice, there may be a need to seek further clarification from those responding. This clarification may be requested in the form of brief verbal communication by telephone; written communication; electronic communication; or a request for a presentation of the response to a specific VA NCA Pacific District group or groups. The VA NCA Pacific District reserves the right to seek additional information from those respondents identified with unique solutions that are determined to be beneficial to the VA NCA Pacific District. SCOPE OF WORK REPLACE COMMITTAL SHELTER CONCRETE SLAB ZACHARY TAYLOR NATIONAL CEMETERY LOUISVILLE, KY A.Scope of Work. Furnish all labor, tools, materials, equipment, testing, and site cleanup to replace the committal shelter concrete slab and concrete walks. The contractor shall replace all sod disturbed with new sod. Work shall consist of but is not limited to: 1.Removal and storage of the existing committal shelter and benches on-site during construction. 2.Remove and dispose of approximately 625 square feet of concrete slab and walks. The three (3) foot wide concrete walk up to the flagpole shall be removed and replaced with a six (6) foot wide concrete walk. This flagpole walk shall be extended to and tie-in to the new committal shelter concrete slab. 3.Prepare subgrade for new concrete slab and flagpole walks. Concrete slab underneath committal shelter Bier shall be level. Remaining areas of committal shelter slab shall be sloped 1% for drainage. Subgrade shall be adjusted so the committal shelter concrete slab and flagpole concrete walk tie into existing asphalt pavement and slope to drain. All final grades shall be ADA compliant. 4.Committal shelter concrete slab shall have four (4) inches of #57 drainage stone and four (4) mil poly vapor barrier beneath the concrete slab. Concrete slab shall be four (4) inches thick, 4,000 psi concrete reinforced with welded wire mesh, 6x6-W1.4xW1.4. Concrete slab shall have saw cut joints at five (5) foot centers. 5.Flagpole concrete walk shall be four (4) inches thick, 4,000 psi concrete reinforced with welded wire mesh, 6x6-W1.4xW1.4. Concrete walk shall be six (6) feet wide with saw cut joints at six (6) foot centers. 6.Concrete slab under committal shelter posts shall be thickened to a minimum of twelve (12) inches with new galvanized steel anchor bolts installed. 7.Modify committal shelter steel columns to provide a minimum of eight (8) foot clearance above new concrete slab. Reinstall committal shelter and repaint committal shelter using high quality DTM exterior grade paint. 8.Reinstall committal shelter benches. Repaint committal shelter benches using high quality DTM exterior grade paint. 9.Submit all proposed materials for approval prior to start of construction. Existing conditions. B.Cemetery Location. Zachary Taylor National Cemetery 4701 Brownsboro Road Louisville, KY 40207 Phone: 859-885-5727 (Camp Nelson National Cemetery, Nicholasville, KY) C.Specifications: 1.Specification - Section 32 05 23, Cement And Concrete For Exterior Improvements D.Project Information. 1.Period of Performance. The contractor shall complete all work within 30 calendar days after issuance of Notice to Proceed. Time stated includes final cleanup of the premises. 2.Work Hours. All work shall be performed between the normal business hours of 7:00 am to 4:30 pm, Monday thru Friday, excluding Federal Legal holidays. 3.Pre-bid Site Inspection. A pre-bid site inspection is recommended. Contact the Cemetery Director at Camp Nelson National Cemetery, 859-885-5727, to make site visit arrangements. 4.Preconstruction Meeting. The contractor shall not commence performance of work until the Contracting Officer has conducted a preconstruction meeting or has advised the contractor that a preconstruction meeting is not required. 5.Detailed Work Plan. The contractor shall provide in writing a detailed work plan which presents the contractor's plan for completing the work. The contractor's plan shall be responsive with the scope of work and describe, in detail, the approach to be used for each task required. For each task, the contractor shall identify all necessary subtasks (if any), associated costs by task, and milestone dates. Four copies of the contractor's work plan shall be provided to the Contracting Officer prior to issuance of the Notice to Proceed. 6.Changes to the Scope of Work. Any changes to the scope of work shall be authorized and approved only by the Contracting Officer. Any costs incurred by the contractor through directions of parties other than the Contracting Officer shall be borne by the contractor. E.Standards of Conduct. The National Cemetery Administration honors veterans with a final resting place and lasting memorials that commemorate their service to our Nation. National Cemeteries are national shrines. The standards of work, appearance, and procedures performed by the contractor at the cemetery shall reflect this nations concern for those interred there. Due to the sensitive mission of the cemetery, contractor personnel must exercise and exhibit absolute decorum, composure, and stability at all times. 1.The burial activities at a National Cemetery shall take precedence over construction activities. The Contractor shall cooperate and coordinate with the Cemetery Director in arranging construction schedule activities to cause the least possible interference with cemetery activities in actual burial areas. Construction noise during the interment services shall not disturb the service. Trucks and workmen shall not pass through the service area during this period. 2.The contractor shall cease work operations as necessary to ensure that no contract work causes any funeral, ceremony, procession or visitation be delayed, altered, or otherwise impacted in such a way that the dignity, serenity or security of the event is compromised. 3.The Contractor is required to discontinue his work sufficiently in advance of Easter Sunday, Mother's Day, Father's Day, Memorial Day, Veteran's Day and Federal holidays, to permit the contractor to clean up all areas of operation adjacent to existing burial plots before these dates. Cleaning up shall include the removal of all equipment, tools, materials and debris and leaving the areas in a clean, neat condition. 4.Workmen are subject to rules of the Cemetery applicable to their conduct. While on site at the National Cemetery, contractor personnel shall be required to adhere to the following standards of dress and conduct while performing work in the national cemetery. These standards and regulations are enforceable under Title 38, U.S.C., Section 218. 5.Contractor personnel appearance and conduct shall be professional and unobtrusive at all times. Shirts and full length pants must be worn at all times. 6.Smoking is not allowed in any building at the National Cemetery. 7.The playing of radios, cassette recorders, etc., is prohibited on site or in buildings. The contractor's work force is governed by the same rules governing cemetery employees. All individuals failing to comply shall be removed from the site. 8.Sexual harassment will not be tolerated and the offending individual(s) shall be removed from the site. 9.Contractor personnel shall not sleep on cemetery grounds, sit or stand on installed grave markers, or put, place or lean equipment or materials on grave markers. 10.Questions from cemetery visitors shall be politely referred or directed to cemetery personnel. 11.The contractor shall ensure that all employees behave with appropriate decorum, courtesy, and respect while within the cemetery or at its perimeter or entrances. Shouting, cursing, angry outbursts, sleeping, intoxication, unsafe driving of private or company vehicles, and violence or criminal acts of any kind will not be tolerated and is cause for immediate removal from the cemetery. 12.The contractor and subcontractors shall not fuel equipment in grass/turf areas. Petroleum based products will damage the grass/turf. Contractor shall bear all costs associated with removing any contaminated soil as a result of fueling in grass/turf areas and shall resod the area.
