- Statement of Work RFQ FA5215-16-Q-8007 STATEMENT OF WORK (SOW) FOR IRONWOOD TREE REMOVAL AND PRUNNING BELLOWS AIR FORCE STATION (AFS), WAIMĀNALO, HAWAI‘I 1.0 SCOPE OF WORK: The purpose of this project is to remove and prune ironwood trees at Bellows AFS. Contractor shall complete the service no later then 120 days upon notification of contract award (to include hauling off all debris). Contractor shall submit weekly projected work schedule to POC so that Bellows AFS personnel can ensure the safety of guests. All trees will be clearly marked for removal or pruning by Bellows AFS staff. 2.0 SPECIFIC TASKS: TREE PRUNNING: A- Small Tree up to 6" DBH (DIAMETER AT BREAST HEIGHT = 4.5FT) Complete Tree Pruning: The Contractor shall prune trees according to their natural growth characteristics leaving them well shaped and balanced, and to promote proper health and growth. Retain typical growth habit of individual plant with as much height and spread as is practical. Contractor must remove all dead wood and insect-infected limbs as well as excess water sprouts. No heading or topping cuts will be made. Thin and clean crowns, reduction cuts that bring back branch to greater than 1/3 diameter and remove dead branches. Pruning shall be accomplished by or under the guidance of a certified Arborist. Work shall be completed within 2 calendar days. B- Medium Small- Above 7" up to 14" DBH (DIAMETER AT BREAST HEIGHT = 4.5FT) Complete Tree Pruning: The Contractor shall prune trees according to their natural growth characteristics leaving them well shaped and balanced, and to promote proper health and growth. All removed tree material will be hauled off of Bellows AFS by contractor within 30 calendar days of removal. I- Medium Small- Above 7" up to 14" DBH (DIAMETER AT BREAST HEIGHT = 4.5FT) Complete Tree Cutting: Trees must be cut down in sections and safely lowered to ground to prevent damage to surrounding areas. The remaining tree stumps should be no more than 6 Inch in height. If tree debris cannot be hauled off Bellows AFS immediately, contractor must move it to the CE waste hold area within 48 hours of removal. All removed tree material will be hauled off of Bellows AFS by contractor within 30 calendar days of removal. J- Medium - Above 15" up to 24" DBH (DIAMETER AT BREAST HEIGHT = 4.5FT) Complete Tree Cutting: Trees must be cut down in sections and safely lowered to ground to prevent damage to surrounding areas. The remaining tree stumps should be no more than 6 Inch in height. If tree debris cannot be hauled off Bellows AFS immediately, contractor must move it to the CE waste hold area within 48 hours of removal. The remaining tree stumps should be no more than 6 Inch in height. If tree debris cannot be hauled off Bellows AFS immediately, contractor must move it to the CE waste hold area within 48 hours of removal. All removed tree material will be hauled off of Bellows AFS by contractor within 30 calendar days of removal. N- XX-Large- Above 55" up to 90" DBH (DIAMETER AT BREAST HEIGHT = 4.5FT) Complete Tree Cutting: Trees must be cut down in sections and safely lowered to ground to prevent damage to surrounding areas. The remaining tree stumps should be no more than 6 Inch in height. If tree debris cannot be hauled off Bellows AFS immediately, contractor must move it to the CE waste hold area within 48 hours of removal. All removed tree material will be hauled off of Bellows AFS by contractor within 30 calendar days of removal. NOTE: This statement apply to all above paragraph. Contractor will have no more than 120 days to complete contracted services (to include hauling off all debris). Contractor must provide weekly projected work schedule so that Bellows personnel can ensure the safety of guests. All trees will be clearly marked for removal or trim by Bellows AFS staff. Contractor should make all efforts to maintain high standards of safety and cleanliness. Contractor should make every effort to minimize the amount of disruption to the environment and operations as possible. Additionally, the contractor is responsible for any damages to property, equipment or environment they cause. The contractor must replace or repair damages as appropriate. 4.1 PERIOD AND PLACE OF PERFORMANCE AND WORKING HOURS: 4.2 PERIOD OF PERFORMANCE: The period of performance shall not exceed 120 days from date contract is signed. 4.3 PLACE OF PERFORMANCE: Various locations across Bellows AFS (437-acre military recreational facility) Waimānalo, HI 96795. 4.4 WORKING HOURS: Tree removal/trimming can be accomplished seven days a week and on holidays (at contractor's discretion) but cannot be conducted any earlier than 0900 or any later than 2000 on any given day. 4.5 EQUIPMENT: Contractor shall provide all tools, personal protective, gear, labor and equipment for services rendered. Bellows AFS will provide a staging area for contractor vehicles and/or large equipment if required. 5.0 NOTIFICATION REQUIREMENTS: The Contractor is required to notify the Contracting Officer (CO) of critical issues that may affect the contract performance and/or human health and the environment. The types of issues that require notification include, but are not limited to, health risks, spills, and changes in critical personnel, and finding Munitions and Explosives of Concern (MEC). As an example, if MEC were discovered during field activities, the Contractor would be required to immediately stop work, report the discovery to Bellows AFS Security Forces and implement the appropriate safety precautions. Field activities could not continue until clearance was received from the CO. On critical issues, verbal notification should be made immediately, followed by written notification as soon as practical. 6.0 SOLID WASTE DISPOSAL. Contractor shall provide Bellows AFS with data to identify the type, tonnage and destination (e.g. landfill, recycling, waste to energy, etc.) of all solid waste (including green waste) that is recycled or disposed on a monthly basis. Copies of manifests and weight tickets shall be provided to the Bellows AFS EPM. 7.0 HAZARDOUS MATERIAL AND HAZARDOUS WASTE ACTIVITIES. The Contractor shall handle all hazardous materials and waste in accordance with applicable federal, state, and local requirements. The Contractor shall provide a listing of hazardous chemicals/materials and estimated usage (quantities) to Bellows AFS EPM for any material requiring a Material Safety Data Sheet (MSDS) and brought or used on station. It is anticipated that nonhazardous solid waste will be generated during the project. No hazardous waste is anticipated as a result of this project. 8.0 MANAGEMENT, PLANNING, AND REPORTING REQUIREMENTS. The Contractor shall supply all necessary supervision, labor, equipment, vehicles and materials necessary to accomplish the work. The Contractor shall perform management and planning functions, including performance measurement and fund status reporting, through the course of this effort. The Contractor shall document for government review and approval the results of the project efforts for the SOW. 2.1 Meetings 2.1.1 Meetings/Teleconference Support. The Contractor shall attend and/or support meetings and teleconferences as required by the Requiring Activity and/or 766th Specialized Contracting Squadron (SCONS) representative(s). The purpose of the meetings includes, but is not limited to, contract discussions, progress reviews, project scoping, planning, project status, and the general exchange of information concerning current activities. 2.1.2 Kick-Off Meeting. This SOW provides for activities required for the on- site kick-off meeting. The contractor will present a Project Work Plan that will include a brief description of activities of the required tasks and finalized schedule. 9.0 The contractor shall perform all tree work in accordance with contract specifications and the following tree care standards, latest addition, unless otherwise directed by the Contracting Officer. ANSI A300 - Standard Practices for Tree Care Operations including referenced Combined Federal Regulations (Utility Pruning and Emergency Service Restoration) ANSI A300 (Part 1) - Tree Pruning ANSI A300 (Part 3) - Tree Support Systems (Cabling, Bracing, and Guying) ANSI Z60.1 - American Standards for Nursery Stock ANSI Z133.1 - Safety Requirements for Tree Care Operations 10. CONSERVATION. Activities shall be planned and implemented in a manner that protects existing site utilities, surface features, and the general site environment. This includes the protection of trees, shrubs and other vegetation not in the affected zone from dust damage, soil compaction, and physical contact with machines and equipment. All reasonable measures shall be taken to minimize and suppress fugitive emissions of dust, vapors, and other site materials during site work. The Contractor shall conduct all operations and activities with the intent of reducing the amount of pollution generated. Specific areas to be focused on are generation of solid waste, use of hazardous materials, use of ozone depleting chemicals, generation of hazardous waste, and use of energy and water. 11.0 SECURITY REQUIREMENTS: Contractor is responsible to ensure background checks and Form B-180's are accomplished for any laborer that will require base access for 3 days or more. These two documents must be submitted to visitor control center no less than 5 duty-days in advance of access. All heavy equipment and commercial contract delivery vehicles are subject to be searched by Security Forces personnel prior to entry.
