Description

Auto Phoroptor and Installation The overall purpose is to provide and install Auto Phoroptor at the Southeast Louisiana Veterans Healthcare System (SLVHCS) Outpatient Ophthalmology Clinic, 2400 Canal Street, New Orleans, Louisiana 70119. Per, Statement of Work dated August 12, 2016. Responses are due no later than Thursday, August 18, 2016 @ 2:00 p.m. CDT. Instruction to Offerors Guidance: This Sources Sought Notice is not a Request for Quote (RFQ). No solicitation is being issued at this time. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the government to a particular acquisition approach. Any information provided to the Government as a result of this Sources Sought Notice is voluntary. Responses will not be returned. No entitlement to payment or direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses or the Government's use of such information. This Sources Sought Notice is for planning purposes only. INSTRUCTIONS FOR SUBMISSION After review of the above information, interested parties may submit a response in an electronic format only (via email) to Leslie Robinson, Contract Specialist leslie.robinson@va.gov. Respondents should include as part of their submission: a.Indicate whether or not you can support the procurement requirement based on the items identified in the Statement of Work; b.Any questions that you have reference this Sources Sought Notice; c.Company point of contact name, telephone number and email address; d.Socioeconomic status/Business Size (Other Than Small Business, Small Business, Service Disable Veteran Owned Small Business, etc.); and e.Provide DUNS number, CAGE code, and indicate whether or not you are registered in the System for Award Management (SAM) database (Website: www.sam.gov). (Registration in SAM is a requirement to do business with the Federal Government).
