Attachment 6, EMS Flyer Attachment 5, Past Performance Questionnaire Attachment 4, Applicable Clauses and Provisions Attachment 3, Historical Workload Data for Repair Calls Attachment 2, Performance Work Statement Attachment 1, Pricing Schedule (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FA5613-16-R-0016, and the solicitation is issued as a request for proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. (iv) This is not a set-aside. The North American Industry Classification System (NAICS) code for this solicitation is 811310. (v) The 700 CONS/LGCA Flight has a requirement to purchase preventive maintenance, safety inspections and repairs of pop up barriers, tire shredders, bollards, arm barriers and sliding gates in the Kaiserslautern Military Community (KMC), for the period 01 February 2017 through 31 January 2018, with options to extend the performance through 31 January 2022. Offerors shall use Attachment 1, Pricing Schedule, which includes all contract line item numbers, quantities and units of issue, to fill in all unit prices and total amount blocks, for the base period and all option periods. (vi) Attachment 2, Performance Work Statement (PWS), contains the d escription of the requirement for preventive maintenance, safety inspections and repairs of pop up barriers, tire shredders, bollards, arm barriers and sliding gates in the KMC. Attachment 3, Historical Workload contains the historical workload data for repair calls. (vii) Places of performance and acceptance are identified in Appendix B of the PWS, contained in Attachment 2. All offerors must propose FOB (free on board) destination. Required period of performance: Base Period: 01 February 2017 - 31 Janaury 2018 1 st Option Period: 01 February 2018 - 31 Janaury 2019 2 nd Option Period: 01 February 2019 - 31 Janaury 2020 3 rd Option Period: 01 February 2020 - 31 Janaury 2021 4 th Option Period 01 February 2021 - 31 Janaury 2022 (viii) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items (Oct 2015), applies to this acquisition. Addenda to this provision are included in Attachment 4, Applicable Clauses and Provisions. (ix) The provision at FAR 52.212-2, Evaluation - Commercial Items (Oct 2014), applies to this acquisition. Specific evaluation criteria contained in paragraph (a) of that provision are included in Attachment 4, Applicable Clauses and Provisions. (x) Offerors shall i nclude a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Apr 2016), Alternate I (Oct 2014), with their offer, or complete paragraph (b) of 52.212-3, if the offeror has completed the annual representations and certifications electronically at http://www.sam.gov. The provision is included in Attachment 4, Applicable Clauses and Provisions. (xi) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Jun 2016), applies to this acquisition; Additional FAR (Federal Acquisition Regulation), DFARS (Defense FAR Supplement) and AFFARS (Air Force FAR Supplement) clauses are included in Attachment 4, Applicable Clauses and Provisions. (xiii) The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition. (xiv) In order to be eligible for award the contractor is required to be registered in the System for Award Management (SAM), http://www.sam.gov, and Wide Area Workflow (WAWF), https://wawf.eb.mil. (xv) Offers are due 12 September 2016 not later than 14:00 hrs (2:00 pm) Central European Summer Time (CEST). Offers may be sent via email to erica.morris.4@us.af.mil and ingeborg.jenkins.1.de@us.af.mil, or hand-delivered to the following address; 700th Contracting Squadron/LGCA, Kapaun Air Station, Bldg 2767, 67661 Kaiserslautern. POCs for hand deliveries are Erica Morris room 215 or Inge Jenkins room 219. Postal mail and FAX shall not be used to submit offers. Emailed proposals must be received in the stated addressees' email inboxes on time and the addressee must be able to open the email and all attachments. It is the offerors' responsibility to confirm the timely receipt of the proposals by the stated addressees. Note: Kapaun Air Station is an Air Force controlled installation. Delays at the gate will not be considered if offers are late. Further details can be found in the addendum to 52.212-1 - Instructions to Offerors, in Attachment 4. (xvi) A pre-proposal conference and site visit will be held on 23 August 2016 at 09:00 hrs. Offerors shall provide the names of the attendees no later than 18 August 2016 to the points of contact identified in AFFARS clause 5352.215-9001, Attachment 4, Applicable Clauses and Provisions, in order to allow sufficient time for approval of base entry.
