Functional/Performance/Specification June 16, 2016 Ergonomic Task Chairs for HCS TASK CHAIRS VA Gulf Coast Health Care System 400 Veteran Ave Biloxi, MS 39531 Introduction The government requires ergonomic chairs for employees that have received ergonomic evaluations in the Health Care System. This scope of work will be performed at VA Gulf Coast Health Care System, 400 Veterans Ave, Biloxi, MS 39531. Project Overview This is a requirement for ergonomic task chairs. All furniture within project scope is to be installed by vendor. The RFQ is for contract furniture and the vendor is responsible for all final measurements, final designs, pending approvals, transportation of said product. Product Specifications A.Allseating You Ergonomic Task Chair with Hard Surface Casters or equal item with the following technical specifications: (QTY: 30) "Adjustable Height, U-shaped frame "Multifunctional Arm "Syncro Tilt Mechanism "Ratchet back height adjustment "Lumbar support: mesh back support bows and concaves via a turn knob that increases pressure to the lumbar area by increasing or decreasing the curve in the mesh "Frame Finish: Polished Aluminum "Mesh Back Available in Light Gray "Seat Pan Slider "Casters: soft for hard floor "Seat Fabric: Momentum, Beeline, color: Sisal "Minimum 10 year comprehensive warranty "Greenguard Gold certified "80% or greater of the chair must be recyclable at the end of its useful life. Provide a photograph of item. Delivery scheduling will need to be coordinated with VA COR and end users. Preferred form of communications is email. Products will need to be scheduled for delivery once arrived and inspected. Upon award, the VA will require that chairs are tagged according to caster type for ease of installation. Chairs should be delivered assembled and blanket wrapped. Miscellaneous Furniture Information "Furniture must have warranty noted-this will be a determining factor "Please provide photographs of each item "Upholstery must be commercially rated and meet NFPA 701 "VA designer will select finishes with awarded vendor "Furniture order needs to be coordinated with VA interior designer "Delivery must occur during business hours, 7:30 am - 2:30 pm, Monday - Friday
