- RFP #IHS-16-236-SOL-00039 The Indian Health Service (IHS), Division of Acquisition Policy (DAP) is issuing this RFP for the purpose of entering into a Firm-Fixed Price contract (one 12-month Base Period with 4 (four) 12-month Option Periods) to establish a structured home visiting program developed specifically for implementation by Native American Indian paraprofessionals to support Native families from the pregnancy stages to 36 weeks post-partum as described in Section C: Description/Specifications/ Performance Work Statement. Interested parties may participate by submitting your response in accordance with the following instructions: 1. Offerors must be registered under SAM (System for Award Management - www.sam.gov ) in order to be considered for award. Provide your nine-digit DUNS Number (Data Universal Numbering System), which uniquely identifies your individual business in SAMs. 2. Refer to Section L "Instructions, Conditions, and Notice to Offerors" for solicitation instructions. 3. Refer to Section M "Evaluation Factors for Award" for evaluation information. The following criteria will be used to technically evaluate the proposal: - Technical Approach; - Management Approach/Qualification of Proposed Key Personnel; - Adequacy of Management Plan and Adequacy of Manpower and Facilities/Resources; - Section 508 Compliance; - Special Consideration - American Indian (AI)/Alaskan Native (AN). 4. An award will be made on a best value basis; both price and other factors considered. An award will be made only in the event that the Offeror's proposal is found to be technically acceptable and is determined responsive and responsible in accordance with the terms and conditions of this solicitation. 5. The IHS DAP will evaluate proposals on the basis of information furnished by the Offeror. The IHS DAP is not responsible for locating or obtaining any information not identified in the quote. The proposal must be signed by an official authorized to bind your organization. The proposal must be submitted no later than Friday, September 2, 2016 at 10:00 am (EST) via e-mail to vito.pietanza@ihs.gov.
