Unify Control System's Performance Work Statement - Revised 17 Aug 2016 Wage Determination # 05-2531 Revision 17 for State of Utah Combined Synopsis/Solicitation •(i) This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this announcement. (vii) Description of items to be acquired: Background: The room in which the unify console is located is receiving new flooring and furniture in the room where the unify console is located. Unify console, wiring, and peripheral equipment to be removed and stored prior to the government refurbishment effort described above. The removal effort will be proposed under contract line item number (CLIN) 0001. Re-installation of the unify equipment to be proposed under CLIN 0002. Please see attachment 1 (Revised-17 Aug 2016 )Performance Work Statement (PWS, for a detailed description of all services and supplies required. The successful OEM authorized/approved offeror shall perform and provide all services, materials, products, labor, and shipping; as identified within the attached PWS. Note: Proposals received by non-OEM authorized service companies and/or not able to be timely OEM approved to meet the government's performance schedule will be determined non-responsive for this requirement. Additionally, work is to be performed on a military installation, offeror's employee(s) performing work under this order must be able to pass a local police background and obtain the installation commander's permission to enter the base in a timely manner; so as to delay performance. ( viii) Date(s) and place of delivery/Performance (estimated): Performance Period: Site work commence: within 30 calendar days from award date (coordinated start date with program office) Completion: on or before 30 Oct 2016 Work to be performed on-site at Hill Air Force Base (DODAAC-F2DTAE); at the following building location: Anticipated requirement is separate on-site work days, 1 day to remove the console system, the 2nd to re-install the console system; once the room refurbishment has taken place. Room refurbishment is not a part of the requirement. The basement of Building 593, 75 ABW/CE Headquarters 5713 Lahm Lane, Hill AFB, UT 84056-5410 [ X ] All required products and materials shall be delivered on or before commencement of the 1 st on-site work day. Freight on Board (FoB) for all products shall be: Destination (ix) Technical point(s) of contact will be provided on the award document. Final inspection shall occur at destination. •(x) T he provision at 52.212-1, Instructions to Offeror(s) -- Commercial, applies to this acquisition. Offers are due by 1000 hours MST, 24 Aug, 2016 via electronic mail to jerry.meakin@us.af.mil. All questions regarding this synopsized solicitation should be directed as soon as possible to the above same individual. Submit proposals on company letterhead or pricing document, contractor format acceptable. Include as a minimum: •1. The solicitation number •2. The name, address, email, and telephone number of the offeror •3. A technical description of the items (services and supplies) being proposed; in sufficient detail for the government to evaluate the proposals compliance in meeting all requirement contained in the PWS •4. Price (fixed) with any proposed discount terms •5. Identify any applicable GSA contract; state part numbers of CLINs quoted from a GSA pricing schedule; and identify any that are proposed as open market pricing. If proposing open market pricing, state difference in prices from existing GSA scheduled pricing; or state if price is equal to or less than pricing offered most favored customers •6. Include 'remit to' address for payment •7. State period of acceptance for submitted proposal •(xi) The government reserves the right to make award, based on submitted original proposals; without further discussion. (xii) Far Clause: 52.212-2: Evaluation -- Commercial Items applies. •(a) The Government will award a contract resulting from this solicitation to the responsible, responsive offeror whose offer, conforming to this solicitation, is the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: •(i) Price •(ii) Technical Acceptablility -proposed items (supplies and services*) must fufill to meet the Government requirement •(iii) Past Performance •(b) Technical Acceptability of the services, when combined with past performance, is approximately equal to price. (c) Options: N/A •(d) Offeror and proposal evaluation to be OEM approved/authorized service companies proposals, found to have a satisfactory past performance record, will be evaluated against Performance Work Statement, dated 2 Jun 2016, see attachment 1 of this posting, in order of price, starting with the Lowest Priced proposal, until an offeror's proposal is found to be technically acceptable. At that point no further proposal evaluations will occur; as the resulting award will be made to this lowest priced technically acceptable offeror with a satisfactory past performance record. If technical evaluations of more than one of the remaining proposals is required; they will continue to be be accomplished again by price from lowest to highest proposal from offeror's with an acceptable by past performance, until an acceptable proposals is found. All remaining proposals will not be reviewed nor will they receive any futher consideration for award. For an offeror's proposal to be determined technically accetable the proposal must clearly and adequately show the contractor has identified all PWS requirements; and its proposed personnel are capable of satisfactorily performing all requirements identifed under the PWS. •(e) Only one award will be made under this solicitation. Award will be conducted under the provisions of FAR Part 12, Commercial Items, and FAR 13, Simplified Acquisition Procedures. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered and active in the System for Award Management (SAM). (f) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. •(vi) Offerors shall include a completed copy of the provision at 52.212-3, Offeror representations and Certifications -- Commercial Items, with its offer; or include cage code or DUNs number; and affirm certifications are complete and on file at https://www.sam.gov/portal/SAM/ •(vii) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
