- Attachment D - Wage Determination Attachment C - Tax Exemption Letter Attachment B - Business Associates Agreement Attachment A - Rate Schedule RFP Scope of Work, Terms and Conditions, Instructions Contractor shall provide two (2) positions for Registered Nursing (RN) services with specialized experience in the Emergency Department at the Hopi Health Care Center, located in Polacca, AZ on an as needed basis in accordance with the terms specified in the attached statement of work and contract terms and conditions. Period of Performance is estimated to start on October 15, 2016 through October 14, 2017. This requirement is being solicited for a base and four (1 year) option years. This is a small business set aside. Contractor shall state in their proposal that the quote shall remain effective for 120 calendar days after close of solicitation. Closing Date is September 22, 2016 at 1700 hours (ET time). Deadline for questions is on September 9, 1600 hours (ET time). See attached pages for additional information, terms and conditions.
