Description

The purpose of this amendment is to answer questions received and to provide information regarding the site visit on 8/23/16. Questions: 1)The RFP states on Page 10 that "Equipment must consist of Servers, PC's, Monitors, Mounts, and any other items necessary". Our experience with the VA in installing OR workflow systems is that the VA will require the software to run on an existing or virtual VA server and that the vendor will not be allowed to image or administer the server running behind the VA's firewall. Please advise if the RFP can be revised to state that the OR workflow system must run on an existing or virtual VA server that will be supplied, imaged, and administered by the VA? No, the RFP should not be revised to state this. We would like the contractor to provide the server(s) required for the OR Workflow System. 2)If relevant past performance history is available for a vendor's DUNs number, may the vendor also use the Past Performance Questionnaire to provide additional ratings? The Past Performance Questionnaire is to be completed for companies that the contractor has completed work for in the past relevant to the project being solicited. 3)(p. 30, VAAR 852.252-70) This paragraph indicates that the referenced FAR (852.246-71 Inspection) is to be submitted with the offer. The paragraph also says "must be completed by the offeror and submitted with the offer". What I pulled did not have anything to complete. Do we attach a copy of this provision to the executed RFP? This clause does not have anything to complete so it does not need to be returned with the proposal. 4)(p. 4, 10.1) "copy of SAM registration must be provided with Quote". Does this go in Volume I, Section1, Documentation? Yes please include this in Volume 1. SITE VISIT INFORMATION: The site visit will take place on Tuesday August 23, 2016 at 9am. We will meet in the lobby of the Pan Am Building located at 2400 Canal Street, New Orleans, LA 70119. This building is located on the corner of S. Rocheblave and Canal Street on the 1st floor. While there is nothing specific attending contractors need to bring please note that this hospital is still under construction and a reasonable number of attendees is requested. Attending contractors will need PPE in order to participate in this site visit. The following are required: Hard hat, safety glasses, safety vest, gloves, pants are required, closed-toe flat shoes (no heels), and a shirt with at least 4" sleeves. Contractors may take notes and measurements if needed, however no pictures will be allowed.
