- SOW This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this requirement is NIH-O D4289798 and this is a Request for Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1 dated June 15, 2016. The NAICS code for this requirement is 541380 for $15.0M. The Statement of Work is attached. The Period of Performance will be October 1, 2016 through May 31, 2017 for the base year and three (3), twelve month option periods. FAR Pt. 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and a statement regarding any addenda to the provision. FAR Pt. 52.212-2, Evaluation-Commercial Items is applicable. Deliverables •· Samples for analysis will be collected and frozen by NINR and sent to the contractor in large batches for analyses approximately every six months. Testing results will be returned within 14-30 days from receipt of sample(s). Results should be provided in both hard copy and electronic format. •· Contractor must notify the Project Officer promptly following receipt of any sample that is not viable for testing with an explanation/reason as to why Proposals will be evaluated on : Evaluation Criteria The government will be evaluating the proposals on a best value basis using Tradeoffs. The evaluation factors are listed below, with the weight of each factor attributed each Non-Price Factor. Non-Price Factors, when combined, are significantly more important than Price Factor. 40% Key Personnel qualifications and experience in administering similar programs 20%, Technical Approach to implementation of tasks and deliverables stated 40%, Price will be rated only among those proposals that are deemed Technically Acceptable after review of their Non-Price Factors. Offerors shall include a completed copy of the provision at 52.212-3, Offerors Representations and Certifications-Commercial Items, with its offer or submit a printout to their Online Reps and Certs. An invoice is the Vendor's bill or written request for payment under the contract for supplies delivered or services performed. A proper invoice is an "Original" which must include the items listed in bullets 1 through 12 below. The due date for making invoice payments for meat and meat food products, perishable agricultural commodities, dairy products, and edible fats or oils, shall be in accordance with the Prompt Payment Act, as amended. III. Interest Penalties A. An interest penalty shall be paid automatically if payment is not made by the due date and the conditions listed below are met, if applicable 1. A proper invoice was received by the designated billing office; 2. A receiving report or other Government documentation authorizing payment was processed and there was no disagreement over quantity, quality, or contractor compliance with a term or condition; 3. In the case of a final invoice for any balance of funds due the contractor for supplies delivered or services performed, the amount was not subject to further settlement actions between the Government and the Contractor. (2) While not required, the contractor may include in its subcontracts for commercial items a minimal number of additional clauses necessary to satisfy its contractual obligations. (End of clause) Alternate I (Feb 2000). As prescribed in 12.301 (b)(4)(i), delete paragraph (d) from the basic clause, redesignate paragraph (e) as paragraph (d), and revise the reference to "paragraphs (a), (b), (c), or (d) of this clause" in the redesignated paragraph (d) to read "paragraphs (a), (b), and (c) of this clause." Alternate II (Dec 2014). As prescribed in 12.301 (b)(4)(ii), substitute the following paragraphs (d)(1) and (e)(1) for paragraphs (d)(1) and (e)(1) of the basic clause as follows: (d)(1) The Comptroller General of the United States, an appropriate Inspector General appointed under section 3 or 8G of the Inspector General Act of 1978 ( 5 U.S.C. As prescribed in 17.208 (f), insert a clause substantially the same as the following: Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor prior to the end of the period of performance. 52.217-9 Option to Extend the Term of the Contract. As prescribed in 17.208 (g), insert a clause substantially the same as the following: Option to Extend the Term of the Contract (Mar 2000) (a) The Government may extend the term of this contract by written notice to the Contractor prior to the end of the period of performance provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 4 years.
