Description
1.This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number VA262-16-Q-1393 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation. It is the offeror's responsibility to monitor this site for the release of amendments (if any). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-89, effective April 15, 2016. 2.This is a competitive solicitation, 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB). The North American Industry Classification System (NAICS) code for this acquisition is 562910- Remediation Services (Asbestos abatement/removal services) and the small business size standard is 500 employees. Offerors must be registered in the System Award Management (SAM) at https://www.sam.gov, in order to be considered for award. 3.Description of services. The Government is seeking for a contractor to perform preventive maintenance and cleaning services for catwalks and beams at VA San Diego Healthcare System (VASDHS), 3350 La Jolla Village Drive, San Diego, CA 92161. As specified in the attached Statement of Work (SOW), the Contractor shall provide all resources necessary to accomplish all requirements in performing inspection, repairs, maintenance and cleaning of catwalks and beams; examination of all beams in the interstitials along the catwalks including disposal of asbestos containing material (ACM) and non-ACM materials; seal penetration and ceiling tile replacement from asbestos containing areas to the adjacent areas; removal and abatement of vinyl asbestos tile (VAT), asbestos containing mastic and attached carpet. See attached SOW and Special Contract Requirements. 4.Period of performance. The base period of performance shall begin on September 25, 2016 through September 24, 2017 with option to exercise one-year option period. 5.Contract clauses. The clause at 52.212 - 4, Contract Terms and Conditions- Commercial Items (May 2015) applies to this acquisition and full text may be accessed electronically at http://farsite.hill.af.mil. See attached other contract clauses and addendum applicable to this acquisition. 6.Site Visit. The vendor may request a site visit to inspect the facility covered by this solicitation. A site visit is encouraged, but not mandatory. The intent of the on-site inspection is to give all interested vendors an opportunity to be fully cognizant with the participating VA medical facility where the required services will be provided. All interested vendors shall comply with the following requirements listed below: 6.1Site visit will be held on 08/30/2016 from 10:00 AM -11:00 AM PST. 6.2Vendors are required to confirm attendance by sending an email to the Contracting Officer (CO): Jenny Peters at jennelyn.peters@va.gov no later than 12:00 PM PST on 08/29/2016. Contracting Officer will provide acknowledgement receipt of the email request and details of the meeting location. If no request received by the required date, site visit will not be conducted. 6.3Vendors attending the site visit must show proof of Asbestos Occupation Certificate and must bring own respirator. 6.4 Vendors shall be responsible for all cost and travel expenses associated with the onsite inspection visit. Failure to conduct on-site inspection or walk-through of the participating VA medical facility shall not release the vendor from complying with the terms, conditions, specifications, and requirements of this solicitation. 7.Evaluation. The Government intends to issue a firm-fixed price; single award Indefinite Delivery Indefinite Quantity (IDIQ) contract resulting from this solicitation to the responsible Offeror whose offer represents best value to the Government, price and other factors considered. Award will be made on the basis of Lowest Price, Technically Acceptable (LPTA) offer that is determined by the Contracting Officer to be complete, accurate, responsive, and conforms to the solicitation requirements. Attached are the evaluation factors that shall be used to evaluate offers. 8.Submission of offers. Offeror(s) shall submit offer in response to this solicitation by providing and completing the following list below: 8.1As a minimum, cover page must include company name, point of contact, address, telephone number, email address, DUNS number and RFQ no.VA262-16-Q-1393. 8.2All documents required in the evaluation factors: 8.2.1Schedule of Supplies/Services and Price/Costs. The Contracting Officer may issue an order for less than the quantity on which the offer is based, at the quoted price, unless the Offeror will not accept an order on that basis. 8.2.2Technical Capability including sub-factor A through D 8.2.3Past Performance 8.3Signed copy of amendment(s), if any. 8.4Completed copy of provision at 52.212-3 Offerors Representations and Certifications - Commercial Items (July 2016). 9.Questions or inquiries: All questions/inquiries must be submitted to the Contracting Officer: Jenny Peters via electronic mail at jennelyn.peters@va.gov no later than 2:00 PM PST on 09/01/2016. Ensure to reference "Solicitation # VA262-16-Q-1393" within subject line of the email. All questions and answers pertaining to this solicitation, if any, will be posted to this site by issuance of an amendment prior to the closing date. Telephone inquiries will not be accepted. 10.Offer due date. Offers are due and must be received no later than 12:00 PM, PST on 09/07/2016. Submit your offer via email to the Contracting Officer: Jenny Peters at jennelyn.peters@va.gov. Email notification will be sent by the CO to confirm receipt of offer only. It is the Offeror's responsibility to ensure all required documents are included and completed as required by this solicitation. 11.Attachments.
