Description
The Department of Veterans Affairs, Network Contracting Office 10, 24 Frank Lloyd Wright Drive, Lobby M, Suite 2200, Ann Arbor, MI 48105, intends to solicit offers the maintenance of the Evoqua Purification System with all labor, materials, equipment, supervision, and all necessary resources to maintain and test all systems. The contractor shall be authorized to maintain Evoqua Purification Systems as to Evoqua specifications. Work shall performed at the VA Ann Arbor Healthcare System (VAAAHS), 2215 Fuller Rd, Ann Arbor, MI 48105. Contractors that can provide this requirement and meets the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. business size under NAICS 811219, and 4. If the item(s) are available on a FSS contract or not and the FSS contract number if it is available on contract, 5. Notification if they are a VETBIZ registered VOSB/SDVOSB or not. Reverse Osmosis Equipment Maintenance SCOPE OF WORK: Provide periodic service on our Reverse Osmosis Equipment to ensure optimal performance. Test, Measurement and Service Reports will be provided to the VA. Immediate notification will be provide if any deficiencies are discovered during a service interval. Ann Arbor VA Medical Center is seeking one (1) base year with two (2) one year options. CONFORMANCE STANDARDS: Services provided in this contract will conform to current GMP's and reference the AAMI recommends for the water quality necessary for the reprocessing of medical instruments. PREVENTIVE MAINTENANCE: Preventive Maintenance will be performed on the following components with the interval listed: quarterly replacement of all consumable filters; quarterly service and inspection of equipment; 6-month replacement of Carbon Bed; quarterly replacement of Mixed Bed; annual replacement of 0.2 micron cartridge filter; 6-month replacement of UV Bulbs and Quartz Sleeve; annual replacement of 0.05 micro Ultra Filters; 6-month replacement of TOC UV Bulb; annual replacement of TOC Filter; 6-month replacement of TOC Solution Set. REPORTING REQUIREMENTS: The Contractor shall be required to schedule an appropriate service date with Facilities Management in order for the equipment to be taken out of service with little or no disruption to the normal operation of the SPS Department. Contractors will check in with the Logistics Department currently located in the basement of Building 1W. Scheduled service will be performed during normal business hours which considered to be Monday-Friday 8:30am to 4:30pm. This check in is mandatory. When the service is complete, the Contractor shall document services rendered on a legible field service report. The Contractor shall log out at the Facilities Management Office and submit the field service report to the Contracting Officer Representative for a signature, during normal working hours. SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as OSHA may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default.
