Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-16-Q-0774 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. SET-ASIDE: This requirement is Total Service-Disabled Veteran-Owned Small Business Set-Aside. The NAICS is 561730 and the small business size standard is $7.5 Million. In accordance with VAAR 852.219-11, any SDVOSB submitting a quote for this action must be verified for ownership and control and is so listed in the vendor information pages database (http://www.vetbiz.gov) - upon submission of quote. Only qualified offerors may submit bids. Introduction: The government anticipates awarding a Firm Fixed Price award, lowest price technically acceptable (LPTA). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation requirements. Description of Services: The Louisville VAMC has a need for a contractor to provide all management, supervision, labor, skiffs, transportation, equipment, supplies, materials, and quality control necessary to provide live plant maintenance for 31 units that contain approximately 67 live plants on a weekly basis. The live plants are utilized within the VA Medical Center to provide environmental, psychological, personal health, and productivity benefits for visitors and staff. See attached Statement of Work (SOW) and Line Item CLIN for complete details. Period/Place of Performance: Louisville VAMC, 800 Zorn Avenue, Louisville KY 40206. Base Year - October 1, 2016 thru September 30, 2017 with (4) one-year option periods to renew through September 30, 2021. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an "Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement" to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement. The offeror shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the resulting contract. The offeror shall provide past performance evidence. This assessment is based on the offeror's record of relevant and recent past performance information that pertains to services outlined in the solicitation requirements. The offeror shall provide evidence that all workers (engineers, technicians and helpers) must have certification/documentation as evidence of their degree of training and skills in order to perform the requirements in the solicitation. Certification/documentation must be submitted at the time of offer. All quotes received without this documentation will not be considered. STATEMENT OF WORK 1.SCOPE OF WORK: The contractor shall provide all management, supervision, labor, skills, transportation, equipment, supplies, materials, and quality control necessary to provide live plant maintenance for 31 units that contain approximately 67 live plants on a weekly basis for the VA Medical Center Louisville located at 800 Zorn Avenue, Louisville, Kentucky 40206. The live plants are utilized within the VA Medical Center to provide environmental, psychological, personal health, and productivity benefits for visitors and staff. 2.REQUIREMENTS: "The contractor shall provide maintenance for live plants located in the Emergency Room, Main Lobby, West Entrance (OIF/OEF clinical entrance), Pod, Eye, D125 and Audiology Waiting areas. "The contractor shall provide maintenance services that includes but is not limited to watering, pruning &trimming, removal of dead foliage, disease control, cleaning, quality control, fertilizing and replacement as needed. "The contractor shall be responsible for ensuring that all contractor employees providing work on this contract are fully trained and completely competent to perform the required work at all times during the performance of this contract. "The contractor shall ensure that the highest standard of professional capability and workmanship is maintained throughout the life of this contract. "The contractor shall provide any and all replacement plants as needed for the Emergency Room, Main Lobby, West Entrance (OIF/OEF clinical entrance), Pod, Eye, D125 and Audiology Waiting areas. "All Contractor employees shall wear a uniform with the company name and logo and wear a badge in plain view above the waist bearing the company name, logo and employees name. 3.OTHER REQUIREMENTS: "Watering - Many factors influence the amount of water needed by a plant-temperature, light, the size of growing container, etc. The contractor shall project the amount of water each plant will use in a week. "Fertilizing - The contractor shall fertilize plants to keep them green and healthy. Over-fertilizing can be a serious problem and difficult to correct. The contractor shall s conduct soil tests as necessary to determine the nutrient needs of plantings. "Pruning & Trimming - The contractor shall remove the leaves as soon as they turn yellow. The tips of leaves will often turn brown, particularly on older leaves, so the contractor shall also trim the brown tips and provide pruning as required when plants grow too tall or become spindly. "Insect Control - The contractor shall replace any plant that does not remain in a healthy, attractive condition. The contractor shall notify the Contracting Officer Representative (COR) in a prompt manner when the need for a replacement occurs due to a plant's appearance becoming deteriorated. 4.PLACE OF PERFORMANCE: Robley Rex VA Medical Center, 800 Zorn Ave, Louisville, Kentucky 40206 5.PERIOD OF PERFORMANCE: Base Year: 1 October 2016 - 30 September 2017 Option Year 1: 1 October 2017 - 30 September 2018 Option Year 2: 1 October 2018 - 30 September 2019 Option Year 3: 1 October 2019 - 30 September 2020 Option Year 4: 1 October 2020 - 30 September 2021 6.DOCUMENTATION The contractor shall furnish to the Contracting Officer Representative (COR) legible hard copies and electronic copies of service reports indicating details of the work performed upon completion of all work. 7.SECURITY/ IDENTIFICATION The contractor's employees shall check in and out with the VA Police obtain visitor identification each day. Employees must have a valid state or government identification. All contractor/vendor personnel shall display a VAMC Louisville issued identification badge while performing work on any VAMC property. Smoking is not prohibited on property except in designated areas. Possession of weapons is prohibited (in vehicle or on person). Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable in the United States (Federal) District Court. Contractor employees (crew) when on VAMC Louisville grounds; shall not bring visitors into the facility; shall not bring into the facility, any form of weapons or contraband; shall not bring in any alcohol or drugs or be under the influence of alcohol/drugs; shall conduct themselves in a professional and polite manner at all times; shall not cause any disturbances in the facility, are subject to all the rules and regulations of the VAMC Louisville. Contractor personnel shall present a neat appearance and be easily recognized. Contractor shall furnish and require every on-site company employee to wear a company shirt and safety shoes. All company workers while on duty shall be identified by uniform shirt or blouse indicating the company logo. The print should be large enough to be easily read. In addition all company staff assigned to work at the VAMC Louisville. 8. SITE REGULATIONS: All work shall be performed in accordance with VAMC Louisville rules & regulations. 9. CONTRACT TYPE: Firm-Fixed Price - Base plus four year options.
