- Performance Work Statement - Draft - July 07 2016 Notice of Intent to Award Sole Source for Accreditation Surveys for Inpatient Medical Treatment Facilities. THIS IS NOT A REQUEST FOR PROPOSALS - THIS IS A NOTICE OF THE GOVERNMENT'S INTENT TO AWARD SOLE SOURCE. The Air Force Medical Operation Agency (AFMOA) has a requirement for the surveying of the United States Air Force Inpatient Medical Treatment Facilities (MTFs). The locations include 23 on-site hospital surveys at 13 inpatient MTFs located throughout the Continental United States (CONUS) and outside the Continental United States (OCONUS). The survey schedule in the Performance Work Statement (PWS) indicates which locations are surveyed each year. The anticipated period of performance is for a one (1) year base period with four (4) option years to commence on 01 Jan 17. The applicable NAICS code is 813920, Professional Organizations, at a size standard of $15.0 million. The Department of Defense (DoD) Medical Treatment Facilities (MTFs) requires a hospital accreditation survey for the purpose of "Accreditation" by an accreditation source that has been approved by the Assistant Secretary of Defense Health Affairs (ASD (HA)). All other inpatient MTFs will receive a hospital accreditation survey with Primary Care Medical Home (PCMH) certification once every three (3) years, with the exception of Osan AB, Korea which receives an accreditation survey every two (2) years. For specific requirement details, please see attached draft PWS. This notice of intent is not a request for competitive proposals and no solicitation is available. Capability statements are not required; however all responsible sources may submit a capability statement, proposal, or quotation which shall be considered by the agency. If any interested responsible party believes it is equally or otherwise uniquely capable of meeting the requirements described herein may submit a capability statement (to include contact information, size status under NAICS 813920, DUNS Number, and CAGE Code) at no cost to the Government. A draft Performance Work Statement (PWS) dated 07 July 2016 is provided for your review. All responses must be provided in writing to the Contracting Officer not later than 10:00 AM Local San Antonio, Texas time (Central) on 30 August 2016. A determination by the Government not to compete this proposed contract is solely within the discretion of the Government. The Government will not be responsible for any costs incurred by responding to this notice. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
