- Request for Quote #813252 Notes to Quoters: A. This is a combined "Synopsis/Solicitation" for one firm‐fixed‐price purchase order. Quoted price shall include all costs necessary to supply, repair, including and shipping FOB Destination to the delivery address in the Statement of Work (SOW). B. Interested vendors shall submit price and a confirmation that delivery of services can be made within the required time frame listed in the SOW. C. Service Contract Act Wage Determination Applicable Service Contract Act Wage Determination(s) will be required under this contract. The contractor shall be responsible for obtaining the wage determination applicable to the location of their facility or the location where services are performed. TYPE OF CONTRACT: A Firm-Fixed Price (FFP) contract vehicle will be used for the purchase of repair services to repair United States Department of Agriculture (USDA) Animal Plant Health Inspection Service (APHIS) Plant Protection and Quarantine (PPQ) Aircraft and Equipment Operations (AEO) Cessna U206 (N9583M). Quoted price shall include all costs necessary to supply, repair, including and shipping FOB Destination to the delivery addressed in the Statement of Work (SOW). 2. SCOPE OF WORK: Aircraft must be inspected for all corrosion. Once inspected all corrosion must be cleaned and treated in accordance with the aircraft manufacturer approved methods. All parts that have damage must be annotated. Damaged parts that are considered unrepairable must be removed and replaced in accordance with aircraft manufacturer approved methods. All contractor services must be in accordance with FAA Regulations. The location shall be an enclosed hangar with ample space for working with various tools and equipment required. 3. BACKGROUND: The Aircraft for United States Department of Agriculture (USDA) Animal Plant Health Inspection Service (APHIS) Plant Protection and Quarantine (PPQ) Aircraft and Equipment Operations (AEO) Cessna U206 (N9583M). The corrosion has started to affect the Fuselage, Wings, Empennage, and Cowling. If the corrosion and sheet metal damage to the aircraft is not repaired soon, it will make the aircraft UNAIRWORTHY and is incapable of completing any missions without putting a pilot's life at risk. 4. DELIVERALBLE SCHEDULE: Vendor will supply a complete list of all parts and repairs required and scope of damage in hidden areas. After plane repairs are complete the plane is required to be in anodized primer prior to release for painting. Any estimated repair costs above and beyond the initial scope of work must be reported to the COR and any additional repairs or extensions may only be approved by the Contracting Officer. 5. Invoicing - USDA, APHIS uses the Invoice Processing Platform (IPP) for electronic submission and tracking of invoices and payment information to its suppliers of goods and services. Invoices shall be submitted electronically to the IPP invoicing system. Invoices may be submitted weekly. Vendor may only invoice for weeks worked. The contractor may register at https://ipp.gov. An invoice must include- (1) Name and address of the Contractor; (2) Invoice date; (3) BPA number, contract line item number and, if applicable, the order number; (4) Description, quantity, unit of measure, unit price and extended price of the items delivered; (5) Terms of any prompt payment discount offered; (6) Name and address of official to whom payment is to be sent; and (7) Name, title, and phone number of person to be notified in event of defective invoice. Invoices will be handled in accordance with the Prompt Payment Act (31 U.S.C. 3903) and Office of Management and Budget (OMB) Circular A‐125, Prompt Payment. Contractors are encouraged to assign an identification number to each invoice. (8) Invoicing shall not exceed hours worked as noted on Price Sheet (pg 2, paragraph #3): 560 hours worked. 7. SECURITY REQUIREMENTS AND CONTRACTOR IDENTITY PROOFING, REGISTRATION, AND E-VERIFY PROCESS a. The contract employee is responsible for safeguarding all government property provided for use. The contractor shall ensure that all card keys/keys issued are not lost or misplaced and are not used by unauthorized persons. b. Contractors shall comply with the personal identity verification (PIV) policies and procedures established by the AGAR Advisory No. 81, Revision 02 dated April 28, 2009 - Common Identification Standard for Contractors and Departmental Regulation (DR) and Departmental Manual (DM) 4620-002. Copies of the DR and DM 4620-002 can be downloaded from the website at http://hspd12.usda.gov. 1) All applicable contractor employees requiring routine physical access to Federally-controlled facilities and/or routine access to Federally-controlled information systems must go through the identity proofing, registration, and e-verify process, and have a successful background investigation conducted by OPM. The level of investigation required is dependent on the duties and may require a low-risk non-sensitive National Agency Check with (Written) Inquiries, an Office of Personnel Management (OPM) Public Trust or an OPM National Security background investigation to serve on the contract. E-Verify is an electronic system which checks employee information obtained from the Employment Eligibility Verification form (I-9 form) against records maintained by both the Department of Homeland Security (DHS) and the Social Security Administration (SSA). (See Attachment 1 - Security and Contractor Identity Proofing, Registration, and E-verify Process). 2) Contractor ID credentials will be issued after successful identity proofing of the contractor employee applicant and upon a successfully adjudicated background investigation. 3) In accordance with the USDA regulations, a Background Investigation of each employee is `required prior to starting work under this contract as well as maintaining it throughout the duration of the contract. Prior to start of work, the CONTRACTOR shall provide only fully qualified and properly cleared employees for the work to be performed under this solicitation. The contractor shall, through their own internal background investigation protocol and pre-employment investigation, ensure that each employee submitted for a background investigation is reasonably expected to receive a favorable adjudication of their respective investigations and training. 4) The USDA, APHIS will pay for the required background investigations. The government shall have and exercise full and complete control over granting, denying, withholding, or termination the background investigation clearance for employees. 5) All contractor personnel that will have access to the APHIS facilities will be required to have a Background Investigation (BI) with no less than favorably adjudicated returned results. Once approved and working at the facilities, contractor personnel shall not be removed or diverted from the site without prior notification to the designated COR. It has been determined by APHIS personnel that a Low Level- National Agency Check with Inquiries (NACI) BI is required for all personnel. The COR will work with the contractor to submit all required documents required to complete the BI. 6) The contractor shall not employ persons for work on this contract if such employee is considered by the contracting officer to be a potential threat to health, safety, general well-being or operational mission of USDA and its employees. If the contracting officer finds a prospective employee to be unsuitable for his or her assigned duties, the contractor will be advised immediately that such employee cannot continue to work or be assigned to work under the contract. 7) Contractors shall comply with the personal identity verification (PIV) policies and procedures established by the AGAR Advisory No. 81, Revision 03 dated July 19, 2010 - Common Identification Standard for Contractors. The level of investigation required is dependent on the duties and may require a low-risk non-sensitive National Agency Check with (Written) Inquiries, an Office of Personnel Management (OPM) Public Trust or an OPM National Security background investigation to serve on the contract. E-Verify is an electronic system which checks employee information obtained from the Employment Eligibility Verification form (I-9 form) against records maintained by both the Department of Homeland Security (DHS) and the Social Security Administration (SSA). If the information input matches the DHS and SSA records, an ‘employment authorized' message will be received. However, any discrepancy between the information input and the records at DHS and SSA will result in an alert being issued. The alert is called a Notice of Tentative Non-Confirmation (TNC) and further action is required. All background investigations or clearances must be done one week prior to commencing work. Where this is not practical and the prime or subcontractor requires intermittent access to federally controlled facilities or are under the aggregate 180 day limit, a temporary guest ID badge will be issued and/or such laborer may be under constant supervision and their access to areas or data may be more restricted. Laborer's who will NOT have routine unaccompanied physical access to a Federally-controlled facility and/or routine unaccompanied access to a Federally-controlled information system may be excluded from coverage by AGAR Advisory 81 based on the LincPass Distribution Risk Assessment. The LincPass Distribution Risk Assessment will be conducted by a USDA Sponsor for laborer's (Contracting Officer's Representative, or other Government designated program/project officers) as part of the PIV II registration, identity proofing, and credential issuance procedures. Examples of probable exclusion are couriers, express mail and package or other delivery persons. The Contractor shall provide to the USDA PIV Sponsor and the Personnel Security Specialist listed below, a list of all contractor and subcontractor employees full name, job title, employer (prime or subcontractor), contract number, performance period and/or the days employee is expected to be on-site for base year and each period of performance, if applicable, and information on OPM's background investigation or clearance conducted by the prime or subcontractor and that contractor employees pass the background check.
