Description
The Department of Veterans Affairs, Network Contracting Office 10, 8888 Keystone Crossing, Suite 1100, Indianapolis, IN 46240, intends to solicit offers for one (1) 120H Holmium Laser and staff training for proper use of this laser. This system is used to perform urological surgeries. Laser shall be capable of dusting kidney stones into a fine powder using setting of high frequency and low-pulse energy. Equipment to be installed at the Saginaw VAMC, 1500 Weiss Street, Saginaw, MI 48602. Training to take place at the VA facility or online. Contractors that can provide a system, installation and training that meets the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. business size under NAICS 339112, and 4. If the item(s) are available on a FSS contract or not and the FSS contract number if it is available on contract, 5. Statement of Work Saginaw VA Medical Center 120H Holmium Laser 1. The Saginaw VA Medical Center has a requirement for surgical laser equipment to be used in performing urological surgeries in our facility located at 1500 Weiss Street, Saginaw, MI 48602. 2. The equipment being requested is a brand name or equivalent procurement. The selection criteria is based on the best overall value to the government as stated below: a. Best overall value to the government when considering price and functionality. Saginaw is a Basic Ambulatory Surgery Center which means we are only able to treat cases of low surgical complexity. The equipment being requested has the capability to "DUST" kidney stones into a fine powder using settings of high frequency and low-pulse energy. 3. Required Specifications: "120 Hertz Holmium laser to provide the ability to "DUST" kidney and ureteral stones into a fine powder, eliminating the need for stone retrieval (helps to eliminate ureteral irritation from passing of fragmented stones, decreased return visits to surgery) Opertating ranges below 120 hertz will not adeqately pulverize stones. "Pulse width adjustment with up to 120 hertz/6 Joules to facilitate stone retropulsion into the kidney "Integrated suction pump "Ability to eliminate large bladder stones "Ability to perform laser vaporization of the prostate (advantages are low reoperation rate, safety, minimal bleeding) "Minimal or reduced anesthia time (Below industry standard) "Dual foot pedals allowing for operation by a single surgeon "220 Volt Electical Power "One year warranty parts and labor "Training package: oOnsite training for Operating Room members, oLaser Safety Officer training, oBasic vaporization and enucleation procedureal training, oOngoing techinical support oAbility to request additional onsite training per needs of the facility
