Credit Authorization Form ACH Vendor Payment Form Place of Performance Provision Business Mgt Questionnaire CID No A-A-55095A dated 01122004 Solicitation Document & Cover Letter CT2218-16 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and the solicitation will be available at the Federal Business Opportunities website. The solicitation number is CT2218-16 and this solicitation is issued as a Request for Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89-1. The North American Industry Classification System code is 339993 - Fasteners, button, needle and pin manufacturing, the SBA small business size standard is 750 employees. This requirement is UNRESTRICTED. UNICOR, Federal Prison Industries, Inc. intends to enter into a three (3) year firm-fixed price Requirements type contract with an EPA clause for the following commercial item: Slide Fasteners, Brass for use in the manufacture of general purpose utility coveralls, blue 3329 and green 3454 for military personnel.
