Description

Network Contracting Office (NCO) 12 is seeking to contract for the services of up to two (2) FTEE, Board Certified/Board Eligible physicians in accordance with the pricing, specifications, terms and conditions contained in Request for Proposal (RFP VA69D-16-R-1259. All work will be performed at the Tomah VA Medical Center in Tomah, Wisconsin. These physicians are to provide coverage at the Acute Medical Inpatient Unit, the Urgent Care Clinic, the Community Living Centers (CLCs) and, when required, perform as Medical Officer of the Day (MOD). To fulfill these demands, contractor physician(s) shall preferably be trained in Family Practice, Internal Medicine, or Emergency Medicine. The period of performance shall be for a Base Period of six (6) months with three (3) six-month Option Periods to extend at the Government's discretion. Contractor is to provide firm-fixed (unit) pricing in the Price Schedule. Contractor will be paid only for actual hours worked and services performed as specified herein. Contractor shall provide the management and administrative support to enhance the rapport between its providers and the Government. Adequate management and administrative support may be demonstrated via accurate and timely responses, including the submittal of documents identified in the Addendum to 52.212-1, Instructions to Offerors, to be received as part of Contractor's proposal by 1:00 p.m. Central Time (CT) on September 13, 2016. The solicitation will be posted on Federal Business Opportunities (FBO) site on/about August 31, 2016 as an unrestricted full and open competition. Questions must be submitted to the Contracting Officer (CO) by 2:00 p.m. CT on Wednesday, September 7, 2016.
