- CSS - W911QY-16-R-0061 Attachment 4 - 52.212-4 Addendum Attachment 3 - 52.212-3 Brand Name J&A Attachment 1 - Requirements List 1. The US Army Natick Soldier Research, Development, and Engineering Center has a requirement for communications headsets, push-to-talk devices, associated cables, and miscellaneous parts to these items. 2. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 3. This solicitation is issued as a request for quotation (RFQ). The solicitation number for this RFQ is W911QY-16-R-0061. 4. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. 5. This solicitation is issued as a 100% total small business set-aside under associated NAICS code 334220 - Radio and Television Broadcasting and Wireless Communications Equipment Manufacturing, with a small business size standard of 1,250 employees. 6. This acquisition will be awarded to the lowest price, technically acceptable offer, however the Government reserves the right not to make an award if it is in its interest. This solicitation does not constitute as a commitment on behalf of the Government. Any cost associated with the preparation of a quotation cannot be charged to the Government. 7. The vendor must provide all supplies found in Attachment 1 - Requirements List. The exact items are required. Third party, non-OEM items cannot be accepted. A brand name justification pursuant to FAR 6.302-1 can be found with this solicitation as Attachment 2 - Brand Name J&A. New items must be provided; the Government will not accept used equipment. The vendor must provide pricing for all items found in Attachment 1 - Requirements List. Attachment 1 - Requirements List contains all contract line item numbers (CLINs), quantities, and units of measure for all required items. Offeror must fill in the unit price and total price amounts for all items found therein. 8. All items must be delivered F.O.B. Destination, 90 days after date of contract (ADC) to: USSOCOM SOFSA 5749 Briar Hill Rd Bldg 221, Door 12 Lexington, KY 40516 Partial and/or early shipments are encouraged if no additional expense to the Government is incurred. 9. Responses to this solicitation must include: A letter, signed by an authorized official of the Offeror, formerly submitting the proposal and containing company name and address, Cage Code, DUNS Number, TIN, business size and type (i.e., large, small, small and disadvantaged, 8(a)certified, woman-owned small, HUB Zone certified small, or service disabled veteran-owned small business) for the applicable NAICS code, and Point of Contact information. The letter shall also include acknowledge of amendments, if any, to this solicitation. Interested parties must be registered with System for Award Management (SAM) under the NAICS 334220. Representations and certifications must be on record. These registrations can be done through the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/. The Government will not delay award for the purpose of allowing a contractor time to register in SAM. 10. The provision at 52.212-1, Instructions to Offerors - Commercial applies to this acquisition, and Addenda to this provision are contained in Paragraph 9 of this solicitation. 11. Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. See Attachment 3 for FAR 52.212-3. 12. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. See Attachment 4 for the 52.212-4 Addendum. 13. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The clauses cited at Paragraph 15 of this Solicitation are those that are applicable to this acquisition. 14. Offers are due on: 13:00 EST 2 September 2016 Quotes will be only be accepted by email.
