COMBINED SYNOPSIS/SOLICITATION Indoor Air Quality Testing Services at Michael E. DeBakey Veterans Affairs Medical Center in Houston, Texas (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number: VA256-16-Q-1123. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. (IV) This solicitation is being issued as a Total Service Disabled Veterans Owned Small Business Set-Aside. The associated NAICS code is 541620, Environmental Consulting Services. The small business size standard is $7.0 Million. (V) This combined solicitation/synopsis is for purchase of the following commercial services: Contractor shall furnish all labor, equipment, skilled and certified personnel to conduct Vivarium Balance Service at the Michael E. DeBakey Veterans Affairs Medical Center main campus located at 2002 Holcombe Blvd, Building 100. Houston, Texas 77030. See Attached SOW (VI) Description of requirements is as follows: Contractor shall furnish all labor, equipment, skilled and certified personnel to conduct Vivarium Balance Service at the Michael E. DeBakey Veterans Affairs Medical Center main campus located at 2002 Holcombe Blvd Houston, Texas 77030.. See attached Statement of Work which applies to the one time performance of Vivarium Balance Service, a Tentative award date of 09/09/2016 and completion date before 12/31/2016. (b) The place of acceptance will be: Michael E. DeBakey VAMC, 2002 Holcombe Blvd, Houston, Texas 77030 Period of performance The period of performance of this contract is tentatively set for September 09, 2016 through December 31, 2016. Quotes are required to be received in the contracting office no later than 2:30 P.M. CST on September 8, 2016. All quotes must be emailed to the attention of Kever L. Swafford.
