- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA248-16-Q-1687 is issued as a (Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-89-1/08-15-2016. Interested contractors may contact Christopher Brailer at 352-381-5730; emailed RFIs are preferred. Responses should be sent to christopher.brailer@va.gov. To receive a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to receive a contract award. NAICS and Set-aside: The NAICS is 811219 and the small business size standard is $20.5 million. This procurement is set aside for Small Businesses. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation and represents the best value to the government. The Service Contact Act, Wage Determination number 2005-2113, Rev. 18, Date 12/29/2015 is applicable to this contract. For further details, access the relevant wage determination at www.wdol.gov. Description of Services: This contract is to provide preventative maintenance services on the Tuttnauer model#69E-1A, Serial #13042102 located at the NF/SG VA Medical Center, Gainesville, FL. Includes cost of parts & labor (includes gaskets), travel, 4 preventative maintenance (PM) visits done quarterly, includes costs for all services calls performed during regular business hours. Services shall be performed by manufacturer trained service engineers who are familiar with all aspects of this equipment. The service engineer will work on site as needed. This will be a for a firm fixed price contract for one base year plus 4 Option Years. REQUIREMENTS: Contractor shall provide maintenance/certification services in accordance with manufacturer's specifications and as outlined below: 1.Ultimate Preventative Coverage - Provides full service support for the instrument. 2.Unlimited emergency service with no additional charges for on-site labor, travel times, or travel expenses during regular business hours. 3.Scheduled preventive maintenance - PM's increase instrument reliability and performance. Thorough PM procedures include cleaning, inspecting, adjustments, and calibration to published factory specifications. Minor problems are corrected to prevent catastrophic failures that jeopardize valuable samples and labs productivity. 4.Replacement parts are provided with this agreement. 5.Engineering modifications are included and are installed without charge during the preventative maintenance visit. 6.Factory trained and certified service engineers perform all service repairs. 7.Organization and delivery of service activity reports (education, troubleshooting, process improvements) and damage frequency reports. PROGRESS AND COMPLIANCE: 1.Vendor shall provide evidence of factory training for each service technician upon request. 2.Contractor shall provide certification worksheets on each machine tested. 3.The contractor shall provide certification in the form of technical documentation, original Invoice, or service documentation that the quality of updates and upgrades is sufficient to guarantee continued performance and operation of the system listed in this contract. PAYMENT: CONTRACTOR REMITTANCE ADDRESS: Effective January 1, 1999 all payments by the Government to the contractor will be made in accordance with Federal Acquisition Regulation (FAR) Clause 52.232-34, Payment by Electronic Funds Transfer-- Other than Central Contractor Registration (31 U.S.C. 3332). DESIGNATION OF CONTRACT REPRESENTATIVE: A VA representative of the Contracting Officer will be designated to represent the Contracting Officer in furnishing technical guidance and advice regarding the work being performed under this contract. The foregoing is not to be construed as authorization to interpret or furnish advice and information to the Contractor relative to the financial or legal aspects of the contract. Enforcement of these segments is vested in and is the responsibility of the Contracting Officer. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include price and quantity of each labor category proposed, price of all anticipate indirect costs, and total price; any offered discounts; name, address, and telephone number of the offeror; terms of any express warranty; completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. Interested contractors must submit a copy of any relevant certifications and licenses along with their quote. Quotes must be received September 16, 2016 by 1300 EST. All quotes must be e-mailed referencing to VA248-16-Q-1687 to christopher.brailer@va.gov; Offers received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition. All questions regarding this solicitation must be submitted to the Contracting Specialist in writing (email is preferred).
