- Pictures of the section of ducting that needs replaced Performance Work Statement Wage Rates for this Requirement This is an As-build of the HVAC system Combined Synopsis- Solicitation for Commercial Items (IAW FAR 12.603) 31 August 2016 Contracting Office Address Department of the Air Force, Pacific Air Forces, 766 SCONS - Joint Base Pearl Harbor - Hickam (JBPHH), 90 G Street, JBPHH, HI, 96853-5230, UNITED STATES Description This is a combined synopsis/solicitation prepared in accordance with Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is not to be construed as a commitment by the Government. All information is to be submitted at no cost to the Government. The solicitation will be issued as a Request for Proposal (RFP) under the solicitation number F1W6CC6127A001, HVAC Duct Cleaning and Duct Repair, Bldg. 310 Wheeler Army Air Field, Oahu HI. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) FAC 2005-89, as of 18 August 2016, Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20160811 and Air Force Acquisition Circular (AFAC) 2016-0603. This acquisition is a Small Business Set Aside in regards to size standards. The applicable NAICS is 561790, and the small business size standard is $7.5 Million. This RFP contains one (1) line items on a firm fixed price basis. The line items are as follows: CLIN DESCRIPTION OF SERVICE COST 0001 The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to provide items as defined in this Performance Work Statement (PWS) and IAW current industry standards, appropriate statutory guidance and the requirements listed in the PWS dated 18 August 2016 rev 2 Period of Performance: 19 September 2016-18 October 2016 FFP QTY Unit Price Total Lump Sum $ $ ADDITIONAL REQUIREMENTS When submitting a proposal on this requirement please include the information below: Cage Code: DUNS No: TIN: Prompt Payment Terms: Period of Performance : Date offer expires: The delivery location is the 169 AOC Bldg. 310 at Wheeler Army Air Field, Oahu, HI. Acceptance will be performed by the Government. The FOB Point is destination. The FAR provision 52.212-1, Instruction to Offerors-Commercial Items (Oct 2015) applies to this acquisition. Addenda to the following paragraphs of 52.212-1 are: (a) A site visit will be held on the 9 September 2016. We will meet at Bldg. 310 on Wheeler Army Air Field. To access the installation you must have rapid gate pass. If you intend to participate in the site visit please respond by email to mark.barber.2@us.af.mil no later than 6 September 2016 by 1300 HST. (b) All proposals are due on or before 4:00 PM, Hawaii Standard Time (HST), 14 September 2016. Submission may be made via email to the Contracting Administrator, TSgt Mark A Barber at mark.barber.2@us.af.mil. Proposals must reference the solicitation number F1W6CC6127A001. The Government intends to evaluate all proposals and award without Discussions. (c) System for Award Management (SAM). Offerors must be registered in the SAM database to receive a contract award. If the offeror is not registered in the SAM database, the Contracting Officer will proceed to award to the next otherwise successful registered offeror. Instructions to register your entity to be eligible for contracts in SAM are located at www.SAM.gov. In addition, 52.212-2(a), The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) technical capability of the item offered to meet the Government requirement; (ii) price; Offeror must ensure the provision FAR 52.212-3, Offeror Representation and Certifications-Commercial Items (this must be completed on http://orca.bpn.gov), DFARS 252.212-7000, Offeror Representation and Certifications-Commercial Items. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
