Description

The Department of Veterans Affairs, Network Contracting Office 10, VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105, is seeking sources for hazardous waste disposal services. Services will be completed with the VA Ann Arbor Healthcare System at the address noted above. Contractors that can provide the items that meet the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. business size under NAICS 492110, 4. If the item(s) are available on a FSS contract or not and the FSS contract number if it is available on contract, 5. Notification if they are a VETBIZ registered VOSB/SDVOSB or not. REQUIREMENTS: Establish a contract for small package express and ground delivery services. The organization will be responsible for conducting and paying for shipments of Category A Infectious Substances and/or Radioactive Yellow Label Substances in support of VA's mission and must provide Hazardous Waste Disposal Services within consignee's premises, to the participating Veterans Affairs Healthcare System specified herein. The Contractor shall provide all labor, supervision, materials, equipment, sanitized, disinfected, transportation, facilities, supplies, and waste manifests. Contactor shall collect, analyze, prepare, package, label, remove, transport, store, treat and dispose of Hazardous waste under the resulting contract in accordance with the terms and conditions, specifications and in accordance with the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), in accordance with all Federal, State, and Local (U.S. Environmental Protection Agency - EPA, Department of Transportation) laws, policies and guidelines, and VA infectious medical waste requirement. The Contractor shall maintain all necessary hazardous waste permits and licenses for the disposal of and treatment of such waste. The Contractor shall also have an assigned U.S. Environmental Protection Agency (EPA) authorization and identification number, necessary Department of Transportation permits, and insurance prior to transport and/or disposal of subject waste. If contractor requires additional information, they may contact U.S, EPA regional office. This sources sought is for the period of October 1, 2016 -September 30, 2017.
