- STATEMENT OF WORK (SOW) A.GENERAL INFORMATION 1.Title of Project: Rental Service of Four (4) 500-Two Chillers. 2.Background: A catastrophic failure occurred in the air conditioning system's cooling towers that provide services to Building 200, Building 200N, Community Living Center 3, Community Living Center 4, and Building 9. As a result of this failure, chillers No 1, 2, 3, and 4 had to be taken out of service leaving these locations without air conditioning and dehumidification services. The chillers will remain out of service, for an indefinite period of time, until a permanent solution can be found to replace the cooling towers. Currently four portable chillers are in place to cover chillers No. 1, 2, 3, and 4 during summer time. However, current contracts will end as of September 30, 2016. The number of chillers required varies throughout the year as the demand for cooling capacity decreases during winter and increases during summer. The chillers are required until new cooling towers are put in place. 3.Scope of Work: The contractor shall provide rental, mobilization, and demobilization of 500-ton chillers including other related items described below to the Chiller Plant located at the Northport VA Medical Center: 3.1Rental: "The minimum quantity of service the Government will acquire is one 500-ton chiller rental service for one month. Chiller must be 460 volt and must comply with energy efficiency requirements in Section B.3. "The maximum quantity of service the Government will acquire is four 500-ton chillers rental service for 30 months. Chillers must be 460 volt and must comply with energy efficiency requirements in Section B.3. "The rental service much include 6 inch 200 feet host kits, 4/0 cable box 100 feet or 50 feet sections, and trailers for chillers. 3.2Mobilization: "Delivery of two Chillers (Chiller 1 and 2) to the Chiller Plant located west side of building 200 and north of loading dock at Northport VAMC for an estimated period of performance of October 1, 2016 through September 30, 2017. "Delivery of trailers for two Chillers (Chiller 1 and 2) west side of building 200 and north of loading dock at Northport VAMC for an estimated period of performance of October 1, 2016 through September 30, 2017. "Delivery of two Chillers (Chiller 3 and 4) to the Chiller Plant located north of loading dock and south to building 200 at Northport VAMC for an estimated period of performance of June 1, 2017 through September 30, 2017. "Delivery of trailers for two Chillers (Chillers 3 and 4) located north of loading dock and south to building 200 at Northport VAMC for an estimated period of performance of June 1, 2017 through September 30, 2017. "Delivery of all necessary chilled water hoses to these Chillers. "Delivery of all necessary temporary electrical cables to the Chillers. "Transportation of all necessary equipment to the Chiller Plant located at the Northport VAMC from the contractors equipment storage location. "Hook-up and installation of all equipment to the Chiller Plant. 3.3Demobilization: Demobilization shall consist of the following services: "The demobilization shall occur within 24 hours at the end of the period of performance of each chiller. "Removal of chiller(s) from the Chiller Plant. "Removal of trailers from the Chiller Plant. "Removal of all chilled water hoses. "Removal of all temporary electrical cables. "De-installation and removal of all equipment. "Transportation of all equipment from the Chiller Plant from the Northport VAMC to the contractor's facility. "Contractor shall restore site to condition as found at the beginning of contract. 4.Performance Period: "Two Chillers (Chiller 1 and 2) Base Year: October 1, 2016 through September 30, 2017. "Two Chillers (Chiller 3 and 4) Base Year: June 1, 2017 through September 30, 2017. "Option Year One for Chillers 1 and 2: October 1, 2017 through September 30, 2018. "Option Year One for Chillers 3 and 4: June 1, 2018 through September 30, 2018. 5.Place of Performance: Chillers 1 & 2Chillers 3 & 4 Northport VA Medical Center Chiller Plant, West Side of Bldg. 200 & North of Loading Dock 79 Middleville Road Northport NY 11768. Northport VA Medical Center Chiller Plant, North of Loading Dock and South to Bldg. 200 79 Middleville Road Northport NY 11768. 6.Type of Contract: Firm Fixed Price (FFP) Indefinite Delivery Indefinite Quantity (IDIQ) 7.Site Visit: Vendors are highly encouraged to attend a site visit to verify existing conditions and to get full understanding of the work to be performed. a.Date and time: Wednesday, September 7, 2016 at 10:00 am. Engineering Main Office Bldg. 10, Room 216 Northport, NY. 11768 c.Point of Contact: John Derudder, Contracting Officer Representative (COR). d.All questions must be submitted in writing via email to Delfo.Saco-mizhquiri@va.gov by Friday, September 9, 2016 by 1:00 pm E.T. B.GENERAL REQUIREMENTS 1.Chillers must meet the energy efficiency requirements. See table in section B.3. 2.The mobilization, installation, and demobilization of two chillers shall occur between 8am-4:30pm. 3.The mobilization shall occur at the beginning of the period of performance and throughout the life of the contract as needed. 4.The demobilization of Chillers shall occur at the end of the period of performance of each chiller. 5.Upon mobilization, installation, and rental of the chillers, the equipment shall be inspected by VA. 6.Report of acceptance and delivery shall be submitted to the designated Contracting Officer Representative (COR). 7.The contractor shall coordinate all necessary inspections with the COR. 8.All contractor personnel must have VA-issued badges at all times while on VA property. C.CHANGES TO STATEMENT OF WORK 1.Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer (CO). Costs incurred by the vendor through the actions of parties other than the Contracting Officer shall be borne by the vendor. D.CONTRACTOR PERSONNEL SECURITY REQUIREMENTS 1.The vendor shall not have access to the Department of Veterans Affairs' computer systems. The certification and accreditation (C&A) requirements do not apply and a Security Accreditation Package is not required. 2.All contractor employees and subcontractors under this contract or order are required to complete the VA's on-line "VA Privacy and Information Security Awareness and Rules of Behavior Training" - TMS Code "10176." 3.Contractors must provide signed certifications of completion to the Contracting Officer Representative (COR) during each year of the contract. This requirement is in addition to any other training that may be required of the contractor and subcontractor(s)."
