- NJ-Facilities Information Pricing Example Statement of Work Map NY-Facilities Information NY-Wage Determination NJ Wage Determination Synopsis: "PLEASE READ THIS NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED." This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is W912K6-16-Q-0082 and is issued as Request for Quotation (RFQ) unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. The associated North American Industrial Classification System (NAICS) code for this procurement is 541620 with a small business size standard of $15.0 M. This requirement is a [Small Business] set-aside and only qualified offerors may submit quotes. This requirement will be awarded on an all or none basis as a commercial Service Fixed Price purchase order under procedures on FAR Parts 12 and 13. Offer must be valid for 30 days. The work covered under this requirement is subject to the Service Contract Act of 1965, and New York Wage Determination No. 2015-4143, Revision No.: 2, Date of Revision: 12/29/2015, and New Jersey Wage Determination No. 2005-2345, Revision No. 19, Date of Revision 12/29/15, are applicable to and will be incorporated into this requirement.. Please provide detailed specifications on how you will accomplish the requested services for a Technical Evaluation to ensure your offer will meet our needs. Requirements: The contractor will provide the necessary personnel and materials required to conduct baseline, annual and special Industrial Hygiene (IH) Surveys and to prepare a comprehensive report for the specified facilities in accordance with (IAW) the Statement of Work (SOW). The contractor should be able to perform IH Surveys and submit final reports IAW the SOW. Scheduling of surveys must be performed within six (6) weeks after contracts has been awarded. All final reports and supporting documents must be completed no later than (NLT) sixty (60) days after the last survey has been completed. Contract Type and Evaluation Criteria: (See Section 1.3 of SOW) Basis for award is "Best Value" with the quote that provides the greatest overall benefit to the government with consideration of price, past performance and technical capability and acceptability.. The associated North American Industrial Classification System (NAICS) code for this procurement is 541620 with a small business size standard of $15.0 M. Format of Quotation: The offeror must submit their quotes based on the format (each facility for each state must be quoted individually as per enclosed (Enclosure No. 3 to the announcement) Excel document titled "Pricing per Facility (Example)" for quote submission. Destination/Delivery: See enclosed SOW (Enclosure No. 1 to the announcement) for facilities/description as surveys will be conducted at multiple locations. Period of Delivery: Sixty Days after completion of surveys, not to exceed six (6) month in its entirety. Notes: Interested offerors must be registered in SAM.
