- The Gulf Coast Veterans Healthcare System (GCVHCS), Network Contracting Office (NCO) 16, Biloxi, MS, is issuing a combined synopsis / solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. The GCVHCS intends to negotiate and award a sole source contract to GE Medical Systems for full maintenance and repair services, to include preventive maintenance inspections, on one GE XR Optima Digital X-ray unit, located at the G.V. (Sonny) Montgomery VA Medical Center, 1500 Woodrow Wilson Drive, Jackson, MS 39216. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number VA256-16-Q-1311 is issued as a Request for Quote (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88 effective May 16, 2016. The NAICS code is 811219 and the Size Standard is $20.5 million. This requirement is unrestricted, sole sourced, and will be firm-fixed-price. The closing date for this notice is September 22, 2016 at 3:00 p.m. EST. The contractor shall provide all tools, labor, transportation, supervision, and parts necessary to provide full maintenance and repair services, including preventive maintenance inspections, for the above-mentioned equipment located at the G.V. (Sonny) Montgomery VAMC facility. (See below Statement of Work for additional detailed information.) Contractor to provide services in accordance with the terms and conditions of the contract for the period of October 1, 2016 through September 30, 2017. To be eligible for award of a contract, vendor must be actively registered in the SAM database (https://www.sam.gov). Vendor is also required to complete representations and certifications found in the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. An amendment will be prepared in response to all questions submitted. STATEMENT OF WORK (SOW) 1. Contract Title. GE Optima XR220 AMX Digital Mobile X-Ray System Service Contract. 2. Background. G.V. (Sonny) Montgomery VA Medical Center is requesting the maintenance and repair service from GE Healthcare for one GE Optima XR220 AMX Digital Mobile x-ray unit in Jackson, MS. This contract shall be for one year. The services are based from GE Healthcare's VA NAC ID/IQ contract vehicle V797P-6019B. The full terms and conditions are discussed in V797P-6019B Modification P00104. 3. Scope. The equipment covered under this service agreement is listed in the table below and located at the G.V. (Sonny) Montgomery VA Medical Center. Service is to include automatic exposure, battery replacement, and tube coverage for the equipment listed below. The equipment covered under this service agreement is listed in the table in Section 3. Scope. Service is to include automatic exposure, battery replacement, and tube coverage for the equipment listed. Expectations of the contractor are detailed below. 1.DEFINITIONS/ACRONYMS: A.Biomedical Engineering - Supervisor or designee. B.CO - Contracting Officer C.COR - Contracting Officer's Representative D.PM - Preventive Maintenance Services, which are periodic in nature and are, required to maintain the equipment in such condition that it may be operated in accordance with its intended design and functional capacity with minimal incidence of malfunction or inoperative conditions. E.FSE - Field Service Engineer, a person who is authorized by the contractor to perform maintenance (corrective and/or preventive) services on the NMVHACS premises. F.ESR - Vendor Engineering Service Report, a documentation of the services rendered for each incidence of work performance under the terms and conditions of the contract. G.Acceptance Signature - VA employee who is authorized to sign-off on the ESR which indicates that the PM has been concluded or is still pending completion, or that the Emergency Repair has been accomplished or is still in a pending status. H.Authorization Signature - COR signature; indicates COR accepts work status as stated in ESR. I.GVSM VAMC: G.V. (Sonny) Montgomery VA Medical Center, Jackson, MS. J.OEM - Original Equipment Manufacture 2.CONFORMANCE STANDARDS: A.Contract service shall ensure that the equipment functions in conformance with the specifications used when the equipment was procured by the VAMC, and any upgrades/updates, as well as following most current published standards/ specifications/regulations: (1)Manufacturer's specifications, Association for the Advancement of Medical Instrumentation (AAMI), The Joint Commission (TJC), National Fire Protection Agency 99 (NFPA-99), Center for Device and Radiological Health (CDRH), Original Equipment Manufacturer (OEM), American Hospital Association (AHA), Institute of Electrical And Electronic Engineers (IEEE), Occupational Safety and Health Administration (OSHA), College of American Pathologists (CAP), Memphis VAMC standard operating procedures, and any other Federal, State, and Local regulations pertaining to the equipment listed for this contract. 3.HOURS OF COVERAGE: A.Normal hours of coverage are Monday through Friday, 8:00 a.m. to 5:00 p.m. Service will be performed during normal hours of coverage unless requested or approved by COR. B.Federal Holidays observed by the VAMC are: New Years' DayLabor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial DayThanksgiving Day Independence DayChristmas Day 4.SCHEDULED MAINTENANCE: GVSM VAMC shall perform initial inspection of equipment. If the equipment is not in proper operating condition outlined by the OEM then the Biomedical Equipment Support Specialist at GVSM VAMC will call the vendor to schedule repair services. A.Contractor shall provide PM procedures and checklists with worksheet originals indicating work to be performed and actual values obtained, if applicable. PM coverage shall be performed during normal work hours: M-F 8AM-5PM CST. Scheduled PM shall include an itemized list of the procedures performed, including electrical safety, and shall include, but need not be limited to, the following: 1.Cleaning of equipment. 2.Reviewing operating system software diagnostics to ensure that the system is operating in accordance with Equipment Performance Section 2. 3.Calibrating and lubricating the equipment. 4.Performing remedial maintenance of a non-emergent nature. 5.Testing and replacing faulty and worn parts and/or parts likely to become faulty, worn, or failed. 6.Inspecting and replacing, where indicated, electrical wiring and cables for wear and fraying. 7.Inspecting and replacing, where indicated, all mechanical components which may include, but is not limited to, X-ray tube mounting hardware; cables and mounting hardware, chains, belts, bearings and tracks, interlocks, clutches, motors, keyboards, and safety, and performance. 8.Returning the equipment to the operating condition as stated in Equipment Conformance Section 2. 9.Providing documentation of services performed. B.The Contractor shall provide software upgrades and updates and software and quality updates. Hardware/firmware/software updates/upgrades shall be scheduled and performed during normal hours of coverage. C.Contractor shall provide repair and maintenance on: automatic exposure, battery replacements, and tube coverage. The response time for repair/maintenance calls is within 24 consecutive hours that the call is made. D.Contractor shall provide FSE Coverage Weekdays: M-F 8AM-5PM CST. E.FSE on-site response time must be no longer than 4 hours. F.Contractor shall provide InSite/Tech phone support G.The Contractor's uptime commitment shall be 97% 6.PARTS - ITEMS: The contractor shall furnish parts. The contractor shall maintain quantity of parts including unique and/or high mortality replacement parts in such a manner as to be available for shipment to arrive at the GVSM VAMC by 10:30AM CST the following morning. All parts supplied shall be compatible with existing equipment. The contract shall include all parts with the following exclusions: detector, peripheral devices, printers, uninterrupted power supply, and workstation. Contract parts coverage is to include: automatic exposure, battery replacement, and tube coverage. The contractor shall use new and/or refurbished parts. All parts shall perform identically to the original equipment specifications. 7.SERVICE MANUALS/TOOLS/EQUIPMENT: A.The GVSM VAMC shall not provide tools, test equipment, service manuals, or service diagnostic software to the contractor. Contractor shall provide all tools, test equipment, service manuals, and/or service diagnostic software. Contractor shall obtain, have on file, and make available to its FSE all operational and technical documentation (such as operational and service manuals, schematics, diagnostic software, and parts lists) which are necessary to meet the performance requirements of this contract. The location and listing of the service manuals, by name and/or the manuals themselves shall be provided to the CO upon request. B.Contractor shall provide the GVSM VAMC Biomedical Engineering Support Specialists copies of all documentation that pertains to any hardware, software, or firmware upgrade or repair. Contractor shall provide to the GVSM VAMC Biomedical Engineering Support Specialist copies of the latest version(s) of all technical documentation and licensing agreements, which shall include operational and service manuals, schematics, diagnostic software, and parts lists. GVSM VAMC Biomedical Engineering Support Specialist shall have access to the same operation, service, and documentation as the OEM's FSE. Contractor shall provide to the GVSM VAMC Biomedical Engineering Support Specialist OEM Service Bulletins for the equipment listed in Part 1. 8.DOCUMENTATION/REPORTS: A.At the completion of each service call (scheduled or unscheduled), Contractor/FSE shall provide a detailed ESR. The ESR shall, at a minimum, document the following information legibly and in complete detail: 1.Name of contractor and contract number. 2.Name of the FSE performing services. 3.Contractor Service ESR number/log number. 4.Date, time (starting and ending), equipment downtime and hours on-site for service call. 5.Identification of equipment to be serviced: I.D. number, Manufacturer's name, device name, model number, serial number, and any other Manufacturer's identification numbers. 6.Description of problem reported by Biomedical Engineering Support Specialist. B.Signatures: 1.FSE performing services described. 2.Authorized VA Employee who witnessed service described. C.Any additional charges claimed must be approved by Biomedical Engineering Support Specialist before service has commenced. 9.REPORTING REQUIREMENTS: Contractor shall be required to report to Biomedical Engineering Support Specialist to check in. This check in is mandatory. The FSE shall check out with Biomedical Engineering Support Specialist and submit the ESR(s) to Biomedical Engineering Support Specialist. All ESR shall be submitted to Biomedical Engineering Support Specialist for an "acceptance signature". If Biomedical Engineering Support Specialist is unavailable, a signed, authorized copy of the ESR will be sent to Biomedical Engineering Support Specialist. If you have question about the e-invoicing program or Tungsten, please contact the FSC at the phone number or email address listed below: oTungsten e-Invoice Setup Information: 1-877-489-6135 oTungsten e-Invoice email: VA.Registration@Tungsten-Network.com A.REPORTING REQUIRED FOR SERVICES BEYOND THE CONTRACT SCOPE: The Contractor shall immediately, but not later than twenty-four (24) hours after discovery, notify the CO and the COR, IN WRITING of the existence or the development of any defects in, or repairs required to the schedule of equipment which the Contractor considers not to be responsible for under the terms of the contract. The Contractor shall furnish the CO and COR with a written estimate of the cost to make necessary repairs. B.ADDITIONAL CHARGES: There shall be no additional charge for time spent at the site (during, or after the normal hours of coverage) awaiting the arrival of additional FSE and/or delivery of parts 11.CONDITION OF EQUIPMENT: Contractor shall accepts responsibility for the equipment described in Part I in "as is" condition. Failure to inspect the equipment prior to contract award will not relieve Contractor from performance of the requirements of this contract. 12.COMPETENCY OF PERSONNEL SERVICING EQUIPMENT: A."Fully Qualified" is based upon training and on experience in the field. For training, the FSE(s) has successfully completed a formalized training program, for the equipment identified in the Part I. For field experience, the FSE(s) has a minimum of two years of experience (except for equipment newly on the market) with respect to scheduled and unscheduled preventive and remedial maintenance, on equipment listed in Part I. B.The FSE shall be authorized by Contractor to perform the maintenance services. All work shall be performed by "Fully Qualified" competent FSE. Contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSE for each make and model Contractor services at the GVSM VAMC. The CO may authenticate the training requirements, request training certificates or credentials from Contractor at any time for any personnel who are servicing or installing any GVSM VAMC equipment. The CO and/or COR specifically reserve the right to reject any of the Contractor's personnel and refuse them permission to work on the GVSM VAMC equipment. C.If subcontractor(s) are used, they must be approved by the CO; Contractor shall submit any proposed change in subcontractor(s) to the CO for approval/disapproval. 13.TEST EQUIPMENT: Prior to commencement of work on this contract, Contractor shall provide the GVSM VAMC with a copy of the current calibration certification of all test equipment to be used by the Contractor on GVSM VAMC equipment. This certification shall also be provided on a periodic basis when requested by the GVSM VAMC. Test equipment calibration shall be traceable to a national standard. 14.IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS: A.Contractor FSE shall wear visible identification at all times while on the premises of the GVSM VAMC. It is the responsibility of Contractor FSE to park in the appropriate designated parking areas. Information on parking is available from the VA Police Service. The GVSM VAMC will not invalidate or make reimbursement for parking violations of Contractor's FSE under any conditions. Smoking is prohibited inside any buildings at the GVSM VAMC. Possession of weapons is prohibited. B.Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. 15.INSURANCE: A.Contractors are required to comply with applicable Federal and State Worker Compensation and occupational disease statutes. B:Contractors are required to have Bodily Injury Liability Insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence. C.Contractors are required to have Property Damage Liability insurance coverage of at least $500,000. 16.EQUIPMENT MODIFICATIONS/UPGRADES/UPDATES: A.Equipment modifications are divided into two kinds; 1) Normal functional improvements that assure that the equipment is operating at the peak of performance and 2) Enhancing improvements are optional improvements that change the equipment's original design to allow for supplementary or additional functions. B.Normal functional improvements shall be provided at no cost to VA within six (6) months after each version is released. Installation shall be done at no additional cost to the government during normal working hours, unless requested by the COR C.Enhancing improvements are optional to VA. If the COR has determined that VA requires such enhancing improvements, it shall be considered outside of the scope of this contract and processed as a separate procurement. D.All modifications, upgrades, updates, enhancements, etc., must be scheduled in advance with the COR. CO and COR must receive notice at least five (5) full business days in advance before any equipment updates, upgrades, enhancements, and modifications are begun along with justification (if Vendor-suggested modification), estimated costs (if applicable), probable effect on equipment operation, and needs for new or additional in-service training. E.Documentation must be provided to Biomedical Engineering Support Specialist when software upgrades/updates are installed. Documentation will include what was installed, how it was installed, and where it was installed on the system. Documentation will include parts list, calibration procedures, and test procedures, if applicable. 17.SPECIAL CONDITIONS: A.The Contractor shall support all system and third party software of the equipment, providing onsite support and repairs during normal hours of coverage. The Contractor will provide telephone support during normal hours of coverage. The Contractor will provide provision revisions and updates of all software that corrects functional flaws. B.Contract labor coverage to include all system options, third party pass thru devices, such as, compact disk devices, remote workstations, etc. and will include all Philips supplied software including that used by third party devices. 5. Performance Monitoring. Vendor performance will be monitored by Biomedical Engineering, the COR, and using service to ensure conformance to stated contract requirements and expectations. Work completed will be reported to the COR to ensure that contractual conditions are met. 6. Security Requirements. The C&A requirements do not apply and a Security Accreditation Package is not required. 7. Place of Performance. Service and maintenance work will be completed on site at the G.V. (Sonny) Montgomery VA Medical Center, Jackson, MS. 8. Period of Performance. Vendor will provide services for the period October 1, 2016 through September 31,2017 on the equipment listed in Part 1.
