- Statement of Work This a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; a written solicitation document will not be issued. The solicitation number is NIH-CC-16-015410. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-89. The North American Industrial Classification System (NAICS) code is 339113. 1.STATEMENT OF WORK 1.1Storage racks with stainless steel surfaces, adjustable 1.1.1Specific Requirement 1.1.2Stainless steel surfaces 1.1.3Adjustable to accommodate custom configurations and additions 1.1.4Surfaces resistant to chemical disinfection 1.1.5Autoclavable and/or cart wash compatible. High temperature resistant. 1.1.6Swaged stainless steel rack posts 1.1.7Provision for casters to configure a mobile rack system 1.1.8Available in the following dimensions: 18"x48"x68", 18"x60"x68", 18"x36"x68", 24"x36"x68", 24"x60"x68" 1.2Tables with stainless steel surfaces, mobile and immobile configurations 1.2.1Specific Requirement 1.2.2Stainless steel surfaces 1.2.3Seamless work surface 1.2.4Surfaces resistant to chemical disinfection 1.2.5Available in the following dimensions: 30"x60", 30"x48", 30"x30" 1.3Case cart, stainless steel surface 1.3.1Specific Requirement 1.3.2Stainless steel surfaces 1.3.3Swaged posts 1.3.4Available in the following dimension: 24"x36" 1.4Gowning bench, stainless steel 1.4.1Specific Requirement 1.4.2Stainless steel surfaces 1.4.3Provision for floor mounting but not required 1.4.4Available in the following dimension: 16"x48"x18" 1.5Assembly and Installation service 1.5.1Vendor shall offer this service which includes assembly and installation into final location onsite. 2.Deliverables 2.1 Items shipped to location of specification. Available either knock down for assembly or assembled. 2.2Assembly and installation service In addendum to FAR 52. 212-2, quotations will be evaluation on 1) technical, 2) past performance and 3) price. Technical is more important than price. The scoring method is a numerical rating. 3.Selection Criteria 3.1.Product compatible with department's specifications. 20 points 3.2.Vendor can supply all items necessary for complete order. 10 points 3.3.Vendor manufacturers and supplies items designed for use in the healthcare industry. 10 points 3.4.Past performance of manufacturer as evidenced from previous clients going back 3 years. 30 points 3.5.Best value to the NIH including purchase price, freight or shipping costs, terms and conditions, supply item pricing and any applicable discounts. 30 points 4. Each offeror will be evaluated on its past performance under existing and prior contracts for similar products and services. Performance information will be used for both responsibility determinations and as an evaluation factor against which offeror's relative rankings will be compared to assure the best value to the Government. The Government will focus on information that demonstrates quality of performance relative to the size and complexity of the acquisition under consideration. QUESTIONS DEADLINE: All question requests for NIH are to be submitted via email to michael.falzone@nih.gov by September 8, 2016, 10:00 a.m. EST. QUOTATIONS DUE: All quotations are due, no later than, 12:00 pm, EST on September 12, 2016 email to micahel.falzone@nih.gov. The solicitation does not commit the Government to pay any cost for the preparation and submission of a quote or proposal. PROVISIONS and CLAUSES: The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this solicitation. The following agenda has been attached to this provision: see above. Certifications Commercial Items FAR 52.212-3 Offeror Representations and Certifications-Commercial Items. (APR 2012).
