The Department of Veterans Affairs (VA) will not accept costs incurred by vendors in responding to this RFQ; all costs associated with responding to this RFQ shall be borne solely by the vendor. Any information submitted by respondents to this RFQ is strictly voluntary. All submissions become Government property and will not be returned. This announcement is based upon the best information available and is subject to future modification. Information Requested from Industry: 1.Company Information / Socio-Economic Status A.Provide the company size and POC information. VA has identified the appropriate North American Industry Classification System (NAICS) Code 541380 If applicable, please identify and explain any other NAICS codes your company believes would better represent the predominant work included in the SOW, below. B.ISO 17025 certification is required and must be confirmed in your response. C.Indicate whether your company, subcontractors, teaming partners, joint ventures have an official, verifiable Federal Socio-Economic Status, e.g., Small Business, Service-Disabled Veteran-Owned Small Business (SDVOSB), Veteran-Owned Small business (VOSB), Women-Owned Small Business, Disadvantaged Small Business, and/or Hub Zone. SDVOSBs and VOSBs must be registered on www.vip.vetbiz.gov at time of response or will be considered to be other than small, unless registered with the SBA as Small at the time of response. If your company is Small but is not an SDVOSB or VOSB, it must be registered with www.SBA.Gov at the time of response to be considered as Small. D.If you plan to use a subcontractor, in order to be considered as having any SDVOSB, VOSB, Small or other official, verifiable Federal Socio-Economic Status, your company must be prepared to perform 50% or more of the work. Any company without a current and complete SAM registration at time of response shall not have their response considered. 2.Federal Contracts A.Identify any federal contract vehicles (ie GSA, SEWP, etc.) belonging to your company, or which your company has a legal right to quote under, using its own name, by contract number and expiration date. 3.Information Specific to the Quote A.This requirement is for a Base Year + 4 Option Years + 6-Month Extension Option for annual calibration and repair for radiation detection instruments which will be shipped to your laboratory at VA expense from various U.S. locations. B.The 6-month extension should be quoted following the fourth option year (the fifth year) but should not be added into the price quote total for the five years. (The contract Schedule will show the base year and each of the four option years. The 6-month extension will be shown in a line or paragraph below the Schedule and the price will not be added into the Schedule total.) C.See the attached RFQ, which includes the PWS. D.See the attached 'Equipment List' and 'Instruments by Location' list. E.The numbers of instruments listed on the Equipment List are the correct ones. F.The instruments will arrive in pelican cases and return shipping shall be insured for the value of the instrument(s). G.Include cost of insured return shipping in each calibration quote, not as a separate line item. H.The 'Instruments by Location' list does not show 1 x 2241-3 Kit and 4 x Polimaster 1703-1. All 5 of these will be returned to Martinsburg, WV. I.Each Polimaster 1703-1 will be shipped in a pelican case together with a Ludlum instrument kit. J.The Physician Serial Numbers beginning on Line 37 of the 'Instruments by Location' list represent 10 x Ludlum 2401-P Meters, as shown on the 'Equipment List', not 8 as shown on the 'Instruments by Location' List. The additional 2 will be returned to Martinsburg, WV. K.Include hourly repair labor rates at 40 hours/year (est.) as a separate line item which should be added into the total for each year. (The cost of 40 hours of repair labor should not include parts; repair labor and parts costs will be pre-approved as the need for repairs arises.) THE FOLLOWING SOW IS SUBJECT TO CHANGE: DRAFT STATEMENT OF WORK MERRT Radiation Survey Instrument Calibration and Repair 1.PURPOSE. VHA Office of Emergency Management (OEM), 10NA1, will contract for radiation safety instrument calibration services required to support the VHA Medical Emergency Radiological Response Team (MERRT). 2.BACKGROUND. The MERRT, as established by VHA Handbook 0320.05, is a specialized team of healthcare professionals with radiological expertise that can be rapidly deployed to a VHA or off-site medical provider to render patient treatment and technical advice during a radiological emergency. 3.Objectives. The contract, once established, will provide a mechanism for MERRT members, who are geographically dispersed throughout the United States, to have MERRT radiation safety instruments calibrated annually. 4.Scope. The Contractor shall provide annual calibration of all MERRT radiation survey equipment. The Contractor will provide instrument repair service as needed and specified in the contract. A base year plus four option years, plus a possible 6-month extension are required. 5.Contractor Requirements. The contractor shall be able to meet the following requirements: Factor #1: Ability to calibrate radiation detection instruments listed below, in-house. Factor #2: Ability to repair radiation detection instruments listed below, in-house. Factor #3: Must have alpha and beta emitting radiation sources for alpha/beta probe calibrations Factor #4: Must have a high energy gamma ray emitting source for standard instrument calibration. Factor #5: Must meet ISO 17025 compliance standards for portable radiological instruments. Factor #6: Ability to calibrate instruments within a 15 day turn-around time. Factors 1-5 are equally important; these are mandatory in order to be able to perform the services. Factor 6 is less important than the other factors but should not go over 30 days. 6.Mandatory Tasks and Deliverables. The contractor shall provide annual calibration of all MERRT radiation survey equipment. The contractor shall also provide instrument repair service as needed and specified in the contract. If for any reason, any deliverable cannot be completed on time according to the schedule below, the contractor shall provide a written explanation, a minimum of three days prior to the due date to the Contracting Officer Representative/Point of Contact (COR/POC). This written transmittal shall include a firm commitment as to when the work will be completed. This transmittal to the COR/POC shall cite the reasons for the delay, and include an assessment of the impact on the overall project. The COR/POC will review the facts with the program office and issue a response in accordance with the contract terms and conditions. Unless otherwise specified, an electronic copy shall be placed in the designated CBO SharePoint site or other CBO-designated site. Specifically, the contractor shall: 6.1Task One. Calibrate, using NIST traceable sources and methods approved in 17025 series of documents for radiation survey instruments, the following radiation survey equipment: Deliverable 6.1.1. Calibrate 20 ea. Ludlum Model 2241-3 survey kit. Each kit requires calibration of meter and 3 included probes. Deliverable 6.1.2. Calibrate 10 ea. Ludlum Model 2224-1 survey kit. Each kit requires calibration of meter and 2 included probes. Deliverable 6.1.3. Calibrate 10 ea. Ludlum Model 2401P or other model Geiger-Mueller survey meter. Deliverable 6.1.4 Calibrate 4 ea. Ludlum Model 52-5-1 Portable Portal Monitor. Deliverable 6.1.5 Calibrate 30 ea Polimaster PM1703-1. 6.2 Task Two. Radiation survey equipment repair, Contractor and COR/POC financial authority and liability. Deliverable 6.2.1. Contractor shall inspect instruments when received for calibration. If repairs are required, or are requested by the COR/POC, the contractor shall contact the COR/POC with an estimate of the repair/labor/parts cost and an estimated completion date in advance of commencing work. The Contractor shall repair instruments with COR/POC authorization up to, but not in excess of, the 'Not to Exceed' (NTE) amount of the contract. Expenditures authorized by the COR/POC, or incurred by the Contractor, in excess of the NTE amount will be disallowed unless approved in advance, in writing, by the Contracting Officer, in the form of an amendment to the contract increasing the NTE which has been distributed to all parties. Additionally, the COR/POC shall not obligate the government to, nor shall the Contractor incur, any expenditures not specifically covered in the contract. Costs incurred by the Contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor. 6.3Schedule of Mandatory Deliverables. 6.3.1 Table of Base Period Mandatory Deliverables Due Dates REFERENCEDELIVERABLEDUE DATE 6.1.1Calibrate 20 ea. Within 30 days of receipt of equipment by contractor 7.Period of Performance. The period of performance shall be from the effective date of the award for a base year plus 4 option years, plus a possible 6-month extension. Work will be performed at the contractor's facility with all instruments being shipped to the contractor at VHA expense. Holidays. None of the following holidays shall be counted in the Due Dates listed above. There are ten (10) Federal holidays set by law (U.S.C. Title 5 Section 6103): Under current definitions, four are set by date: New Year's DayJanuary 1 Independence DayJuly 4 Veteran's DayNovember 11 Christmas DayDecember 25 If any of the above falls on a Saturday, then Friday shall be observed as a holiday. Similarly, if one falls on a Sunday, then Monday shall be observed as a holiday. The Contractor shall perform the work at the contractor's facilities. 9.Type of Contract. A Fixed Price Not-to-Exceed/Contract will be issued for this requirement. 11.Changes to the SOW. Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project.
