- Equipment List Wage Determination Solicitation This is a Solicitation notice for a procurement of Two (2) Mobile and Walk-in Freezer Units as a 100% small business set aside requirement. FOB destination is Tinker AFB OK. Please make sure all provisions and clauses are appropriately filled in (where applicable), when submitting a quote in. Please send in the actual solicitation completed along with any other documentation to support your company's quote. Please make sure information stated is readable and legible. This is a Firm Fixed Price procurement, so please state the price on your quote as all inclusive of everything. ( i.e. shipping, taxes, fees, etc.) Quotes will be accepted on or before the closing date stated on page 1 of the solicitation. When returning this form, please complete: •1. 1. Please complete Company Information (Name & Address) and CAGE Code in Block 17a. 2. Please wet sign, put the name and title of signer, and date signed in blocks 30a., 30b., and 30c. respectively. 3. Please insert pricing on Page 3 along with net amount & delivery times 4. Please complete information in all "Fill-in" provisions and clauses, especially : •a. FAR 52.212-3 •b.
