**********SHALL READ RFQ DOCUMENT********* ****Must Submit this Completed Form with Quote Submission**** ****Must Submit this Completed Form with Quote Submission**** ****Must Submit this Completed Form with Quote Submission**** ****Must Submit this Completed Form with Quote Submission**** ******SHALL READ RFQ ****** The Federal Bureau of Investigation intends to procure five projectors, one television, one projector screen and the installation of all items by a single vendor. Please see below for a list of item(s)/specifications. This is a total small business set aside. NAICS Code for this requirement is 238210 with a size standard (in millions of dollars) of $15.0. Quotes will be evaluated on lowest price, technically acceptable. All vendors shall be signed up and have an active DUNS number in SAM.GOV to quote. There will be a single award for all items. All quotes shall be firm fixed price. Quoted product shall meet all the required specifications below to be considered for award. Failure to do so will result in the vendor's quote being considered not technically acceptable. Only vendors located in the U.S. will be considered. Purchase cards and Purchase Orders shall be accepted for purchases. Delivery and installation shall be performed within 45 days or less. This Request for Quote falls under FAR Part 12 & 13. PLEASE MAKE SURE YOU FILL OUT THE SELF CERTIFYING DOCUMENT (ATTACHMENT NO. 1), ACQUISITION RISK QUESTIONS (ATTACHMENT NO. 2) AND KEY MANAGEMENT PERSONNEL LIST (ATTACHMENT NO. 3). If a vendor does not submit ALL four (4) required documents they will be considered nonresponsive to the RFQ.
