- Equipment Listing for Naval Hospital Guam Scope of Work This is a combined synopsis/solicitation for commercial items and/or Services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. This Solicitation is issued as a Request for Quote (RFQ). The US Naval Hospital Guam intends to award a Firm Fixed Priced Contract for Fiscal Year 2017 Service and Preventive Maintenance on ten (10) Government owned Ice machines. Please see attachment for SOW and Equipment Listing of Ice Machines. Anticipated Start date: SEP 2016 - AUG 2017 with the possibility of Option Years) This procurement is set-aside to small businesses only. NAICS code for this solicitation is 811412. Contractor must be registered in SAM prior to award. The website address for SAM registration is www.sam.gov. Responsible sources must be active in the CCR (Central Contractor Registration), provide DUNS number, Cage Code. All responsible sources may submit a quotation, which shall be considered by the agency. Inquiries submitted via telephone calls will be re-directed to an e-mail submission. Basis for Award: The Government will award a single purchase order contract resulting from this RFQ to the responsible offeror whose offer represents the best value in terms of technical requirement and lowest price technically acceptable. Technical requirement is defined as the ability of the products to meet the Scope of Work essential to meet the needs of the Government. Lowest price technically acceptable is defined when best value is expected to result from selection of the technically acceptable proposal with the lowest evaluated price. Technical requirements and lowest price technically acceptable when combined are significant evaluation factors that will be most advantageous to the Government. Contractors who do not meet both of the factors will not undergo a price evaluation. Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Item. Responsible sources must be registered and active in the System for Award Management (SAM) database, website accessed through http://www.acquisition.gov. Responsible offerors must be able to utilize the Wide-Area-Work-Flow (WAWF) electronic invoicing as a method of payment. WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site. Offerors who fail to complete and submit the requirements above may be considered non-responsive. Applicable Clauses: The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-75. It is the contractors' responsibility to be familiar with the applicable clauses and provisions.
