Network Contracting Office 16 is Requesting For Quote, regarding respirator fit testing and training in accordance with the Occupational Safety and Health (OSHA), code of federal regulations CFR 1910.134 at the Oklahoma City VA Medical Center (OKC VAMC) employees are required to be included in a comprehensive respiratory protection program. See Statement of Work for specifics. Please communicate via email to eric.tuck@va.gov by Sept 16, 2016, 11:00 AM CST as to your company's ability to perform service per the attached Statement of Work. Questions must be addressed in writing to the Solicitation POC (email eric.tuck@va.gov) no later than two (2) business days prior the closing date. BRIEF SCOPE OF WORK Posted in the RFQ (SF 1449) on FBO SERVICE In accordance with the Occupational Safety and Health (OSHA), code of federal regulations CFR 1910.134 some Oklahoma City VA Medical Center (OKC VAMC) employees are required to be included in a comprehensive respiratory protection program. Project Scope The contractor shall provide instructors for respirator fit testing and training for N95 respirators; and, training for Powered Air Purifying Respirators (PAPRs) for individuals who cannot be fitted with an N95. A minimum of one fit tester will be provided for Monday through Friday from 7am-5pm. The VA has the option of requesting a 2nd fit tester to work from 10am-2pm, or other time up to 4 hours during 7am-5pm, as needed. All work shall be conducted in accordance with Occupational Safety and Health (OSHA) respirator standard 29 CFR 1910.134. The fit testing schedule shall be in accordance with a schedule determined by the contract and based on employee availability. Testing days shall be scheduled for Monday through Friday from 7:00 am - 5:00 pm in order to give both shifts the opportunity to get fit tested. Fit tester shall follow OSHA requirements for fit testing and train each employee on the respirator they will be wearing. The contractor shall insure each employee has been medically cleared to wear a respirator within the past 6 months. Fit test will insure the following documentation: "The name of each employee's to be fit tested. "Service and Department "The date an employee's medical questionnaire was approved by the LHCP. "The type of respirator each employee was fitted with, results, fit tester identification and the date trained on the respirator. "Employee signature and the date of testing The contractor shall complete fit testing notification cards (supplied by the VA COR; this card lists the respirator the employee is to wear and the date fit testing was completed) and provide this card to each employee upon the completion of fit testing. On-site maintenance Location: Oklahoma City VAMC 921 N.E. 13th St., Oklahoma City, Ok 73104 Testing All testing materials will be supplied by the VA COR at the OKC Medical Center. The contractor shall keep the COR informed of level of supplies needed for the week. The contractor shall be required to periodically clean the fit testing equipment to ensure that saccharin is flowing through the nebulizer.
