This attachment has complete details about this requirement and contains all forms that need to be returned with your quote. END This is a Brand Name or Equal requirement for DFAS-Cleveland for Telephone Headsets (quantity 250 each) and Cords (quantity 250 each). This is a 100 percent Small Business Set Aside. Deliver FOB Destination to the listed address at DFAS-Cleveland no later than 9/30/2016. You must complete and return the following forms, both included on the last two pages of the attached RFQ document. 1) Technical Certification Form--if you are quoting an Equal item you must complete and return this form. 2) Provision 252.209-7991, answer questions (b)(1) and (b)(2) and return the form with your quote. Questions about this requirement will be accepted until 9:00 a.m. Friday, September 9, 2016. Please see the attached document for complete details. END
