- FAR 52.212-5 Description Requirements This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Oklahoma City Area Indian Health Service, Lawton Service Unit, Carnegie Indian Health Clinic has a need to procure Optometry equipment located at 214 E 4th Street, Carnegie, Oklahoma 73015. The combined synopsis solicitation number is 246-16-Q-0056, is issued as a request for quotation (RFQ). The combined synopsis solicitation RFQ and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89, dated 8/15/16. The combined synopsis solicitation is 100% set-aside for Small Business, the associated NAICS code is 334510 and small business size standard is 500 employees. Requirement: 1. Contractor shall provide Zeiss Cirrus 5000 IS, or equal The description of requirements for this item is attached. The Schedule of Prices/Costs Zeiss Cirrus 5000 IS, or equal 1 each at $________. Date required is 60 days after receipt of order and place of delivery is Carnegie Indian Health Clinic, Optometry Department, 214 E. 4th, Carnegie, Oklahoma 73015-2241. Acceptance and FOB point is the same address. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Addendum to FAR 52.212-1 Instructions to Offerors - Commercial Items: (a) Offerors are to e-mail complete quotes to cindi.rattler@ihs.gov no later than 4 p.m. CDT, September 14, 2016. All provisions and clauses applicable to this solicitation can be viewed in full text using the following web address: http://www.acquisition.gov/far/index.html. (b) New equipment only. No remanufactured for "gray market or refurbished item(s). The provision at 52.212-2, Evaluation - Commercial Items, applies to this solicitation. (a) The Government will award a contract resulting from this solicitation to the responsible offeror who offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluation offers: meeting the technical requirements, past performance and price. A best value award will be made utilizing the lowest price technically acceptable. Provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, applies to this solicitation and is to be included with its offer. Clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. The clause is attached.
