Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY and is neither a solicitation announcement nor a request for proposals or quotes and shall not obligate the Government to award a contract. The purpose of this Sources Sought Announcement is for market research to gain knowledge of potential qualified sources interested and capable of performing the work and obtain their size classification (Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, 8(a), Hub Zone, Women Owned Small Business, Small Disadvantaged Business, etc.) relative to the North American Classification System (NAICS) Code 621512 with a size standard of $15.0 M. The Department of Veterans Affairs, Veterans Integrated Service Network (VISN) 6 is seeking potential sources to provide all labor, material, tools, equipment, maintenance and repair of all Contractor furnished equipment, specialized staff, training, required supplies to perform Mobile Cardiac Outpatient Telemetry and Event Monitor Services at Hunter Holmes McGuire VA Medical Center (VAMC). Contractor furnished equipment shall include but not be limited to a Mobile Cardiac Outpatient Telemetry and Event Monitoring equipment with staff monitoring and reporting to the VA. Monitoring devices shall be the most current level of knowledge and technology and have an algorithm program to recognize rhythms such as Ventricular Fibulation (VFib), tachycardia, bradycardia, and systole. Contractor monitoring services shall include as a minimum: "Function 24 hours per day, 7 days per week and 365 days a year. "Provide 24 hour Mobile Cardiac Outpatient Telemetry and Event Monitoring from a 14 to 28 day period. "Provide detail assessment of 24-hour ventricular ectopy count and burden during the 14 to 28-day period. "Be staffed with Certified Cardio Graphic Technicians, Emergency Medical Technician (EMT) with Advanced Cardiovascular Life Support (ACLS), or Registered Nurse (RN) with ACLS. "Receive and process both patient triggered and asymptomatic Electrocardiography (ECG) events and notifies physician immediately when an urgent event meeting the physician notification criteria is received. "Provide the following reports: oDaily Report sent to physician for the previous 24 hours and includes representative ECGs from the prior 24 hours, any ECGs meeting the physician notification criteria, a heart rate trend graph and if applicable an AF burden graph. oUrgent Report prepared when an ECG meets the physician notification criteria. oEnd of Service Summary Report within 3-5 days after the monitor is shipped back to contractor. The Contractor shall meet all Federal, State and Local codes and requirements for the operation of this service and equipment provided. Contractor personnel performing services shall be licensed and/or registered in a State, Territory or Commonwealth of the United States or the District of Columbia. Potential Sources who are interested and capable of providing the above services must demonstrate their experience and knowledge of the required services and the capability to provide such services in a capability statement to include salient characteristics of the Mobile Cardiac Outpatient Telemetry and Event Monitoring equipment. Potential Sources must be registered in SAM. Potential sources shall identify their business name, point of contact, business size, Federal Supply Schedule (FSS) contract number, applicable business socio-economic category (i.e. small business, SDVOSB, VOSB, SB, etc.), information for recent and relevant projects similar in nature, DUNS number, address and any additional information felt to be pertinent. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this Sources Sought Announcement, a solicitation announcement may be published at www.fbo.gov website or via GSA eBuy, depending on the status of the responses received. Responses to this Sources Sought Announcement are not considered adequate responses to any solicitation announcement that may result from this notice. All interested offerors will have to respond to the solicitation announcement in addition to responding to this Sources Sought Announcement. Written responses to this notice shall be electronically mailed in PDF format or Word format to larry.lapp@va.gov, with subject line: Sources Sought "Mobile Cardiac Outpatient Telemetry and Event Monitoring". Telephone responses will not be accepted. All responses must be received no later than 1:00 PM (EST), September 23, 2016. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued, it will be issued through GSA eBuy.
